Subclassification and Clinical Management of Extraction Sockets with Labial Dentoalveolar Dehiscence Defects.

Immediate implant therapy involving implants placed into intact Type 1 extraction sockets has become a consistent clinical technique. The classification of Type 2 extraction sockets, where the mucosal tissues are present but there is a midfacial osseous dehiscence defect, has been described according to the extent of the buccal bone plate absence. The literature has offered different techniques in the treatment of Type 2 sockets; however, the extent of the defect has never been defined or delineated. In this article, the authors describe a subclassification of Type 2 sockets: Type 2A presents

Vertical Bone Augmentation with an Autogenous Block or Particles in Combination with Guided Bone Regeneration: A Clinical and Histological Preliminary Study in Humans.

BACKGROUND: Vertical ridge augmentation with the use of solid bone blocks or particulate bone autograft, exposed or covered by a nonresorbable expanded polytetrafluoroethylene (ePTFE) membrane, are well known in the literature and have been shown to be effective in treating bone atrophy. PURPOSE: The aim of our study was to assess the two techniques in respect to biological properties of transplanted bone in graft revascularization and bone remodeling in conjunction with dental implants. MATERIALS AND METHODS: Ten patients were treated within the study, with a total of 12 sites with posterio

Prospective study of soft tissue contour changes following chin bone graft harvesting.

This study sought to evaluate changes in the soft tissue contour after chin bone graft harvesting. Thirty selected patients underwent chin bone graft harvesting and evaluations were made using lateral cephalograms preoperatively and postoperatively at 30 and 180 days. Fixed points and lines were established on cephalometric tracings and used to measure the selected vertical and sagittal parameters. Results showed statistically significant alterations to the vertical position values of the vermilion (V-VPV) which increased from 9.70 to 11.01 and the exposure of lower incisors (V-ELI) which incr

Autogenous femoral head as grafting material for mandibular augmentation.

Bone grafting is commonly used for augmentation of the atrophic edentulous maxilla and mandible. Although bone substitutes and allogeneic frozen bone grafts have been applied successfully, fresh autogenous bone grafts remain the 'gold standard' in maxillofacial reconstructive surgery. A disadvantage of harvesting autogenous bone is the resulting donor-site morbidity. The authors present a case in which an autogenous femoral head, which was removed because of a prosthetic hip replacement, was used for augmentation of the extreme atrophic mandible. Using this procedure avoids donor-site morbidit

Segmental sandwich osteotomy and tunnel technique for three-dimensional reconstruction of the jaw atrophy: a case report.

BACKGROUND: A three-dimensionally favourable mandibular bone crest is desirable to be able to successfully implant placement to meet the aesthetic and functional criteria in the implant-prosthetic rehabilitation. Several surgical procedures have been advocated for bone augmentation of the atrophic mandible, and the sandwich osteotomy is one of these techniques. The aim of the present case report was to assess the suitability of segmental mandibular sandwich osteotomy combined with a tunnel technique of soft tissue. Based on our knowledge, nobody described before the sandwich osteotomy with tun

AI-assisted preoperative surgical planning for dental implant.

BACKGROUND: An accurate preoperative assessment of alveolar bone morphology is essential to the success of dental implant surgery. Although Cone Beam Computed Tomography (CBCT) provides high-resolution volumetric imaging for this purpose, interpreting it requires substantial expertise and remains inherently subjective. Integrating artificial intelligence (AI), specifically three-dimensional convolutional neural networks (3D-CNNs), could automate and standardize CBCT interpretation. However, there has been no systematic development of AI-driven preoperative tools capable of objectively predicti

Bone Quality in Resorbed Posterior Maxilla Affects Osteogenesis After Sinus Floor Augmentation: A Retrospective Analysis.

INTRODUCTION AND AIMS: Compromised bone quality and quantity in the posterior maxilla are widely recognised as significant risk factors for implant failures. This study aimed to (1) assess the bone quality of the resorbed posterior maxilla both radiographically and histologically and (2) evaluate the impact of native alveolar bone quality on osteogenesis following maxillary sinus floor augmentation (MSFA). METHODS: Patients presenting advanced posterior maxillary atrophy (residual bone height ≤ 4 mm) underwent MSFA via a lateral window approach using deproteinised bovine bone matrix (DBBM). B

A retrospective study of the risk factors for ridge expansion with self-tapping osteotomes in dental implant surgery.

PURPOSE: To analyze the risk factors associated with the survival of self-threading implants placed with the ridge expansion technique using self-tapping osteotomes after at least 24 months. MATERIALS AND METHODS: This retrospective study followed patients for at least 2 years in whom implants had been inserted by means of the ridge expansion technique. The patient was taken as the unit of analysis, and the loss of any implant as a result of mobility at any time during the follow-up period was considered as a failure. A number of patient variables were recorded: sociodemographic (age/sex), an

Use of 4 immediately loaded zygomatic fixtures for retreatment of atrophic edentulous maxilla after complications of maxillary reconstruction.

This article reports the 20-month clinical outcome of the use of 4 zygomatic implants with immediate occlusal loading and reverse planning for the retreatment of atrophic edentulous maxilla after failed rehabilitation with autogenous bone graft reconstruction and maxillary implants. The intraoral clinical examination revealed mispositioned and loosened implants underneath a maxillary complete denture. The panoramic radiograph showed 6 maxillary implants. One implant was displaced into the right maxillary sinus, and the implant anchored in the region of tooth 21 was fractured. The other implant

A novel approach to the antral sinus bone graft technique: the use of a prototype cutting guide for precise outlining of the lateral wall. A case report.

The antral sinus bone graft has become one of the most predictable and commonly used surgical procedures to augment bone in the posterior maxilla and thereby accommodate implant placement. Positioning the lateral wall during this technique has traditionally been an intuitive process, whereby the surgeon relies on mental navigation to achieve proper identification. The purpose of this article is to introduce a prototype cutting guide that is developed through the use of computerized tomographic imaging, computer software, and the stereolithographic process to precisely position the lateral wall

Assessment of maxillary sinus lifting procedure in the presence of chronic sinusitis, a retrospective comparative study.

INTRODUCTION: Chronic sinusitis can be considered a relative contraindication for sinus lifting procedure. However, its specific effects on bone height, infection and healing have not been as well investigated as its incidence as a post-operative complication. METHODS: A retrospective comparative investigation was executed to evaluate the impact of chronic sinusitis on sinus lifting procedures regarding bone height, infection, and healing scores. Pre-operative and 6-month postoperative records (CBCT or CT and panoramic radiographs) of 40 patients who underwent sinus lifting procedure with gra

Anatomical Landmarks of the Facial Artery and Vein for Intraoral Anastomosis: A Cadaveric Study.

BACKGROUND: Intraoral anastomosis is a widely used technique for microvascular alveolar ridge augmentation and midface reconstruction. However, the predictable anatomical positioning of facial structures, such as the vessels, parotid duct, and facial nerve in the buccal region, has remained unclear. Therefore, we aimed to obtain the anatomical characteristics of these locations to establish surgical landmarks for the intraoral anastomosis of facial vessels. METHODS: A total of 26 sides from 13 formaldehyde-fixed cadavers approximately a month after fixation with a mean age at death of 86.6 ± …

Scintigraphic, histologic, and histomorphometric analyses of bovine bone mineral and autogenous bone mixture in sinus floor augmentation: a randomized controlled trial--results after 4 months of healing.

PURPOSE: To test our null hypothesis stating that the mixture of autogenous cortical bone scrapings and bovine bone mineral (BBM) in a ratio of 1:4, compared with BBM alone, would have no significant effect on new bone formation 4 months after maxillary sinus floor augmentation. PATIENTS AND METHODS: Twenty-four patients presenting with alveolar bone height of less than 5 mm in the narrowest zone between the sinus floor and alveolar crest were randomly assigned to 2 treatment groups in this randomized controlled trial. We augmented 12 maxillary sinuses with a mixture of BBM and cortical autog

Sinus augmentation using human mesenchymal stem cells loaded into a beta-tricalcium phosphate/hydroxyapatite scaffold.

OBJECTIVE: Implant placement in the posterior maxilla may often be contraindicated because of insufficient bone volume and presence of the maxillary sinus. In these situations, sinus floor augmentation frequently has been proposed as the best treatment. This clinical study was based on the hypothesis that the clinical effectiveness of adult mesenchymal stem cells (MSCs) loaded to the biphasic scaffold. METHODS: In this report, the clinical and radiographic results are presented on 6 consecutively treated patients using MSCs in combination with biphasic hydroxyl apatite/ beta-tricalcium phosph
