A comparison of brachial artery-brachial vein arteriovenous fistulas with arteriovenous grafts in patients with poor superficial venous anatomy.

OBJECTIVE: The autogenous arteriovenous fistula (AVF) has been shown to be superior to the arteriovenous graft (AVG) with respect to cost, complications, and primary patency. Therefore, the National Kidney Foundation Disease Outcomes Quality Initiative guidelines recommend reserving AVGs for patients who do not have adequate superficial venous anatomy to support AVF placement. The brachial artery-brachial vein arteriovenous fistula (BVAVF) has emerged as an autologous last-effort alternative. However, there are limited data comparing BVAVFs and AVGs in patients who are otherwise not candidates

Selective venography versus nonselective venography before vena cava filter placement: evidence for more, not less.

OBJECTIVE: We undertook this study to determine whether additional use of selective venography, compared with nonselective venography alone, reveals more abnormal anatomic venous findings that lead to changes in vena cava filter (VCF) position. METHODS: From January 1998 to June 2002, 94 patients underwent VCF placement by vascular surgeons at a university tertiary care center. Indications, techniques, decision analysis, and complications were reviewed. Nonselective venography and selective venography of the inferior vena cava (IVC) were evaluated for image quality, abnormal findings, aberran

Biased T-cell receptor repertoires in patients with chromosome 22q11.2 deletion syndrome (DiGeorge syndrome/velocardiofacial syndrome).

Morbidities in patients with Transfusion dependent thalassemia (TDT) have received more attention in the past years than those in patients with Non Transfusion Dependent β-Thalassemias (NTDT). Both the disease categories are widely prevalent in our country. Many NTDT patients present at a later age and it has been found that age at disease presentation correlates with disease severity. Significant proportion of NTDT patients show craniofacial deformity and also suffer from complications related to iron overload. There is scarcity of data on quantitative dimensions of the craniofacial anatomy i

Cerebral venous drainage in patients with dural arteriovenous fistulas: correlation with clinical presentation.

OBJECTIVE: Dural arteriovenous fistulas (DAVFs) are abnormal, acquired arteriovenous connections within the dural leaflets. Their associated symptoms may be mild or severe and are related to the patient's venous anatomy. With the hypothesis that the patient's venous anatomy determines the development of symptoms, the authors aimed to identify which venous anatomy elements are important in the development of major symptoms in patients with a DAVF. METHODS: A multicenter study was performed based on the retrospective analysis of cerebral angiographies with systematic assessment of brain drainag

Triangle tilt and steel osteotomy: similar approaches to common problems.

BACKGROUND: Each year, thousands of children worldwide suffer obstetric brachial plexus nerve injuries resulting not only in primary nerve injury, but also in development of secondary muscle and bone deformities of the shoulder. The triangle tilt surgery has been developed and shown to effectively address these deformities. The triangle tilt procedure was initially designed by the lead author (RKN) to follow the concepts of joint normalization featured in the Steel pelvic osteotomy used to correct developmental dysplasia of the hip joint, and indeed ultimately bears a striking resemblance to t

On the need to revise the arm structure in stylized anthropomorphic phantoms in lateral photon irradiation geometry.

Distributions of radiation absorbed dose within human anatomy have been estimated through Monte Carlo radiation transport techniques implemented for two different classes of computational anthropomorphic phantoms: (1) mathematical equation-based stylized phantoms and (2) tomographic image-based voxel phantoms. Voxel phantoms constructed from tomographic images of real human anatomy have been actively developed since the late 1980s to overcome the anatomical approximations necessary with stylized phantoms, which themselves have been utilized since the mid 1960s. However, revisions of stylized p

Intensity-modulated proton radiotherapy spares musculoskeletal structures in regional nodal irradiation for breast cancer: a dosimetric comparison.

BACKGROUND AND PURPOSE: Regional nodal irradiation (RNI) for breast cancer delivers radiation in proximity to the shoulder and torso, and radiation exposure may contribute to long-term upper extremity and postural morbidity. To date, no studies have assessed the differential dosimetric impact of proton versus photon radiation on shoulder and torso anatomy. This study examined clinically relevant musculoskeletal (MSK) structures and assessed the dose delivered with each modality. PATIENTS/MATERIAL AND METHODS: Ten MSK structures were contoured on IMPT (intensity-modulated proton therapy) and V

Influence of morphology and material properties on the range of motion of the costovertebral joint - a probabilistic finite element analysis.

The motion of the costovertebral joint (CVJ) is governed by the material properties and its morphology. The goal of this numerical study was to identify the material and morphology parameters with the greatest influence on the motion of the CVJ. A fully parametric finite element model of the anatomy and material properties of the CVJ was developed. The impact of five morphology and thirteen material parameters was investigated and compared to in vitro data. The motion was influenced in particular by the rotational stiffness of the articulatio capitis costae and the lateral position of the fove

Surgical treatment of transposition of the great arteries with bilateral intramural coronary arteries.

A 33-day-old neonate with transposition of the great arteries, ventricular septal defect, and intramural course of both coronary arteries underwent successful arterial switch repair. At a follow-up time of 36 months, left ventricular function and coronary arterial anatomy at angiography were normal. Optimal treatment of such rare coronary arterial abnormality may allow functional and anatomic results comparable to usual coronary arterial patterns in transposition of the great arteries.

Multiparametric MR Urethrography: Dynamic Comprehensive Evaluation of the Male Urethra.

Urethral diseases in males, such as strictures, traumarelated injuries, and neoplasms, are significant clinical challenges that can greatly impact urinary function and quality of life. The complex anatomy of the male urethra, which traverses structures such as the prostate and corpus spongiosum, make it particularly susceptible to injuries and strictures compared with the female urethra. The clinical assessment of urethral abnormalities is often limited, necessitating the use of imaging studies for thorough evaluation. While traditional radiographic techniques such as retrograde urethrography

The Use of a Transforaminal Lumbar Interbody Fusion (TLIF) Cage Following Inadequate Access to the Disc Space During an Anterior Lumbar Interbody Fusion Procedure.

Non-specific lower back pain caused by degenerative lumbar disease, such as disc and facet joint degeneration or spondylolisthesis, significantly impairs quality of life of patients and is associated with higher pain scores and reduced function. Patients that fail to respond to conservative treatment may require surgical intervention, such as lumbar interbody fusion (LIF). Compared to other approaches, an anterior approach to lumbar interbody fusion (ALIF) has advantages regarding efficacy of fusion, visualization of relevant anatomy, and a larger allowable size of the interbody fusion device.

Microstructure of the Surface of the Tongue and Histochemical Study of the Lingual Glands of the Lowland Tapir (Tapirus terrestris Linnaeus, 1758) (Perissodactyla: Tapiridae).

Although the anatomy of the gastrointestinal tract has been characterized in the lowland tapir (Tapirus terrestris), the exact anatomy of its tongue has not been studied. Samples of the lingual papillae and lingual glands were collected from the tongue of an adult female lowland tapir. The microscopic analysis of the structure of the lingual papillae and the histochemical analysis of the secretion of the lingual glands were analyzed. The tongue of the tapir is divided into the apex, body with a distinct lingual prominence, and the root. Its ventral surface is smooth. The most numerous of the m

Review of the Anatomy of the Distal Anterior Cerebral Artery and Its Anomalies.

The anterior cerebral artery (ACA) varies considerably and this complicates the description of the normal anatomy. The segmentation of the ACA is mostly agreed on by different authors, although the relationship of the pericallosal and callosomarginal arteries (CmA) is not agreed upon. The two basic configurations of the ACA are determined by the presence or absence of the CmA. The diameter, length and origin of the cortical branches have been measured and described by various authors and display great variability. Common anomalies of the ACA include the azygos, bihemispheric, and median anteri

European position paper on the anatomical terminology of the internal nose and paranasal sinuses.

The advent of endoscopic sinus surgery led to a resurgence of interest in the detailed anatomy of the internal nose and paranasal sinuses. However, the official Terminologica Anatomica used by basic anatomists omits many of the structures of surgical importance. This led to numerous clinical anatomy papers and much discussion about the exact names and definitions for the structures of surgical relevance. This European Position Paper on the Anatomical Terminology of the Internal Nose and Paranasal Sinuses was conceived to re-evaluate the anatomical terms in common usage by endoscopic sinus surg

Hepatic and pancreaticobiliary MRI and mr cholangiopancreatography with and without secretin stimulation in normal cats.

Magnetic resonance (MR) cholangiopancreatography is useful in humans to diagnose biliary and pancreatic diseases. Some of these protocols incorporate the use of secretin, which stimulates the exocrine pancreas to release bicarbonate with secondary dilation of the pancreatic duct. We compared the utility and quality of multiple hepatic-pancreaticobiliary MR imaging sequences before and after secretin stimulation of the pancreatic duct in five healthy cats. Multiple MR sequences were evaluated, including fast Spoiled Gradient Recalled in- and out-of-phase, Single Shot Fast Spin Echo (SSFSE), T2

Anorectal morphology and function: analysis of the Shafik legacy.

Following the untimely death of Professor Ahmed Shafik, the authors decided to analyse his published work on the anatomy and physiology of the anorectal area, reconciling this body of data with current imaging and physiological techniques. All available papers as indexed by PubMed pertaining to the anorectal area were obtained and reviewed. The anatomical data was compared with historical representations and current new imaging modalities. The original 'triple-loop' theory of the anatomical disposition of the external anal sphincter has been confirmed by coronal and coronally reconstructed ima

A cadaveric study of the serratus anterior muscle and the long thoracic nerve.

The anatomy and function of the serratus anterior muscle and the long thoracic nerve have not been fully elucidated. The purposes of this investigation were (1) to clarify which nerve roots of the cervical spine supply each part of the muscle and contribute to the long thoracic nerve and (2) to investigate the anatomy of the 3 parts of the muscle to understand the function of each part. We collected specimens from 70 dissections of 35 cadavers (11 men and 24 women). The serratus anterior muscle consisted of the upper, middle, and lower parts. The upper part was supplied mainly by the C5 nerve

Assessment of the arteries in living kidney donors: correlation of magnetic resonance angiography with intraoperative findings.

Comprehensive imaging evaluation of kidney donor anatomy is crucial for selecting candidates for living kidney transplantation and for determining the surgical technique to procure the renal graft. In 76 living renal donors we compared the results of preoperative magnetic resonance angiography (MRA) with the intraoperative findings of arterial anatomy. Donors were evaluated for the number of main renal arteries and the presence of any polar arteries. A total of 80 main renal arteries and five polar arteries were observed at MRA. At surgery, 90 main renal arteries and eight polar arteries were

Anatomic comparison of the Roy-Camille and Magerl techniques for screw placement in the lower cervical spine.

The Roy-Camille and Magerl techniques for screw placement in the lower cervical spine were compared under simulated operating room conditions. Three surgeons with varying years of spine surgery experience participated. The anatomic morbidity risks of each screw insertion technique were evaluated by anatomic dissection and radiographic examination. The spinal cord and vertebral arteries were not threatened by either method. Analysis of the pooled data revealed that the Roy-Camille technique had less risk of nerve root injury, nerve roots placed "at risk" for injury, and errors in "zone" of plac
