Anesthetic Dilemma in a Full-term Parturient with Facial Trauma and Suspected Head Injury Presenting as Difficult Airway Posted for Emergency LSCS in a Limited Resource Setting: Spinal versus General Anesthesia.

Trauma frequently occurs during pregnancy and occurs 1 in 12 pregnant women affecting maternal mortality, morbidity, and also pregnancy outcome. Hence, it is essential to consider pregnancy in every female trauma patient of reproductive age. Motor vehicle accidents and domestic/intimate partner violence account for most cases of major maternal trauma. Most of the causes mentioned above can lead to facial injuries/head injuries making it challenging for the anesthesia provider. We report a parturient who presented to us with multiple facial injuries with a suspected head injury following a road

Comparative Effect of Intravenous Ketamine and Tramadol on Hemodynamic Parameters, Pain, Sedation, and Postoperative Nausea and Vomiting in Patients Undergoing Urological Surgery Under Spinal Anesthesia: A Triple-Blind Randomized Clinical Trial.

BACKGROUND: Spinal anesthesia is widely used in urological surgeries but is often associated with hemodynamic instability, insufficient sedation, and postoperative nausea and vomiting (PONV). While ketamine and tramadol have been employed as adjuncts, their comparative effects on perioperative parameters remain underexplored. This study aimed to compare the efficacy of intravenous ketamine and tramadol in modulating hemodynamic stability, sedation, pain, and PONV in patients undergoing urological surgery under spinal anesthesia. METHODS: In this triple-blind randomized clinical trial, 90 pati

Effect of warmed ropivacaine versus room temperature ropivacaine administered spinal anesthesia on recovery of lower limb muscle strength following total knee or hip replacement: a prospective, double-blind, randomized controlled study.

BACKGROUND: With the aging of the population, the number of hip and knee replacements worldwide continues to increase. Postoperative enhanced recovery has become a clinical focus, and an increasing number of hip and knee replacement surgeries are completed on an outpatient basis. Spinal anesthesia is the most widely used anesthetic method in clinical practice, but its disadvantages, such as slow recovery of lower limb muscle strength after operation, have hindered the rapid recovery of such patients. We found that the duration of lower limb motor block under spinal anesthesia with warmed ropiv

Our experience regarding patients with headache, vomiting, and urinary retention following endothermal ablation of the greater saphenous vein under spinal anesthesia: Gender type, age interval, and procedural risk factors are important.

OBJECTIVES: The endovenous thermal ablation of the greater saphenous vein under spinal anesthesia is still a popular technique. Although this procedure is considered to be fast and simple, side effects such as headaches, vomiting, and urinary retention could occur. These side effects make the recovery period difficult for the patient. The patient's age, gender, and procedural risk factors such as needle sizes and types are important parameters that affect the occurrence and rate of undesirable outcomes. This retrospective study aims to evaluate the endovenous thermal ablation method for the ma

Effects of Prophylactic Infusion of Equivalent Doses of Norepinephrine and Phenylephrine in Preventing Spinal Anesthesia-Induced Hypotension During Cesarean Delivery on Fetal and Maternal Outcomes: A Dual-Center, Non-Inferiority Controlled Trial.

Источник пометил публикацию как отозванную

BACKGROUND: Numerous studies have compared the effects of norepinephrine and phenylephrine on maternal and neonatal outcomes during cesarean delivery. However, the infusion rates are often based on clinical experience, resulting in non-equivalent doses. We aimed to compare the effects of norepinephrine and phenylephrine at equivalent doses on fetal and maternal outcomes, and assess their efficacy at the 90% effective dose (ED90). METHODS: A total of 100 parturients scheduled for cesarean delivery were randomly allocated to receive either 0.10 μg/kg/min norepinephrine (Group NE) or 0.60 μg/kg/

Reducing by 50% the incidence of maternal hypotension during elective caesarean delivery under spinal anesthesia: Effect of prophylactic ondansetron and/or continuous infusion of phenylephrine - a double-blind, randomized, placebo controlled trial.

BACKGROUND: Prophylactic administrations of ondansetron or phenylephrine have been reported to provide a protective effect against hypotension in women undergoing cesarean delivery under spinal anesthesia (SA). The main hypothesis is that ondansetron improves the hemodynamic response, especially combined with phenylephrine infusion. METHODS: This prospective, double-blind, randomized, placebo-controlled study included 265 healthy pregnant women scheduled for elective cesarean delivery under SA. Women were randomly allocated into four groups to receive either placebo (control), ondansetron (O)

Impact of being lateral and sitting position for spinal anesthesia administration on maternal hemodynamic variability and the timing of hypotension among cesarean section patients in a resource-constrained area: a prospective observational cohort study.

BACKGROUND: Type of anesthesia used for cesarean section can be general, spinal, or epidural. However, peripheral nerve blocks, such as the lower and upper extremity nerve blocks, can be referred to as other types of anesthesia. Spinal anesthesia is the most frequently used method because of its ease, dependability, and rapidity in attaining proper anesthesia. The purpose of this study was to determine the effects of spinal anesthesia induction position on maternal hemodynamics and the timing of hypotension. METHOD: A prospective observational cohort study design was employed. A simple random

The impact of low-dose hypobaric unilateral spinal anesthesia combined with pericapsular nerve group block on safety and rehabilitation quality in frail older adults undergoing hip fracture surgery: a retrospective case series.

Hip fracture is one of the common types of fractures among the older people. Early and active surgical treatment can significantly alleviate patients' pain symptoms and reduce mortality. Low-dose hypobaric unilateral spinal anesthesia is considered capable of significantly minimizing hemodynamic fluctuations and ensuring the stability of patients' vital signs during the perioperative period. As a novel regional nerve block technique, pericapsular nerve group block has garnered widespread attention. However, the actual effects of the combined application of these two methods in older person und

Effects of prophylactic infusion of equivalent doses of metaraminol and norepinephrine for preventing spinal anesthesia-induced hypotension during cesarean delivery on fetal and maternal outcomes: a dual-center, non-inferiority randomized clinical trial.

BACKGROUND: Few studies directly compared the effects of metaraminol and norepinephrine on maternal and neonatal outcomes during cesarean delivery. Moreover, the infusion rates are often based on clinical experience rather than high-quality evidence, leading to non-equivalent dosing. This study aims to validate and compare metaraminol and norepinephrine at their 90% effective dose (ED 90 ) in obstetric anesthesia. MATERIALS AND METHODS: One hundred parturients undergoing cesarean delivery were randomly assigned to receive either 2.00 μg/kg/min of metaraminol (Group ME) or 0.10 μg/kg/min of no

Comparative Evaluation of Analgesic Efficacy of Ultrasound-Guided Pericapsular Nerve Group Block and Femoral Nerve Block During Positioning of Patients With Hip Fractures for Spinal Anesthesia: A Prospective, Double-Blind, Randomized Controlled Study.

INTRODUCTION: Hip fractures cause severe pain during positioning for spinal anesthesia (SA). Intravenous systemic analgesics can lead to various complications in elderly patients, hence peripheral nerve blocks are emerging as a standard of care in pain management for hip fractures, among which femoral nerve block (FNB) is widely known and practiced. Pericapsular nerve group (PENG) block is a recently described technique that blocks the articular nerves of the hip with motor-sparing effects and is used to manage positional pain in hip fractures. This study aims to evaluate the analgesic efficac

Minimum effective concentration of ropivacaine for ultrasound-guided pericapsular nerve group block to relieve spinal anesthesia positioning pain in elderly patients with femoral intertrochanteric fracture: a double-blind biased-coin dose-finding study.

BACKGROUND: The optimal ropivacaine concentration for ultrasound-guided pericapsular nerve group (PENG) block to relieve spinal anesthesia positioning pain in elderly patients with femoral intertrochanteric fracture remains unclear. We aimed to estimate the minimum effective concentration (MEC) and explore concentration-related motor impairment. METHODS: In this double-blind biased-coin up-and-down study, elderly patients scheduled for spinal anesthesia underwent ultrasound-guided PENG block with 20 mL ropivacaine. The initial concentration was 0.10%. For a successful block, the subsequent pa

Impact of the infusion line on co-loading and highly diluted norepinephrine administration through a single peripheral venous catheter: An in vitro simulation of the infusion protocol used during cesarean delivery under spinal anesthesia.

BACKGROUND: Rapid crystalloid co-loading and prophylactic norepinephrine may share a peripheral intravenous catheter during cesarean delivery. We assessed whether infusion-system architecture alters fluid and drug delivery. METHODS: Six infusion-system configurations were tested in vitro. Norepinephrine 8 μg/mL was infused at 30 mL/h (4 μg/min) while 1000 mL saline was delivered from a pressure bag initially inflated to 300 mmHg. Outcomes were crystalloid volume delivered during the first 10 min, time within the norepinephrine target range of 3.5-4.5 μg/min, and the proportion of measurements
