Introduction Ophthalmic segment aneurysms may present with visual symptoms due to direct compression of the optic nerve. Treatment of these aneurysms with the Pipeline embolization device (PED) often results in visual improvement. Flow diversion, however, has also been associated with occlusion of the ophthalmic artery and visual deficits in a small subset of cases. Case report A 49-year-old Caucasian female presented with subarachnoid hemorrhage due to a ruptured anterior communicating artery aneurysm. On follow-up imaging, the patient was found to have a right asymptomatic ophthalmic segment

Introduction In case of cavernous sinus dural arteriovenous fistula (CSDAVF), transvenous embolization (TVE) of the cavernous sinus (CS) via the inferior petrosal sinus (IPS) is generally performed. However, various approach routes have been reported when the accessibility of the IPS is challenging. We herein report a case of CSDAVF treated by TVE with direct puncture of the facial vein. Case report A 70-year-old woman who suffered from tinnitus, chemosis, diplopia and bruit was referred to our hospital. Digital subtraction angiography (DSA) demonstrated CSDAVF. We initially attempted to perfo

Accurately recognising a supraclinoid internal carotid artery (ICA) fenestration, despite its rare location, is important as it can mimic an intracranial aneurysm, intraluminal thrombus or focal dissection on non-invasive imaging. The overwhelming majority of reported supraclinoid ICA fenestrations are associated with intracranial aneurysms; however, a concomitant arteriovenous malformation (AVM) remote from the fenestration site should also be considered. We present the case of a 26-year-old woman with a Spetzler-Martin grade I right frontal AVM in whom a left supraclinoid ICA fenestration wa

BACKGROUND AND PURPOSE: The artery of Bernasconi and Cassinari is an important infraclinoid branch of the internal cerebral artery. It is of neuroendovascular relevance in view of its supply to complex lesions such as meningiomas and arteriovenous malformations in the tentorial and falcotentorial regions. The present microanatomic study attempts a morphometric elucidation of this slender but important branch of the meningohypophyseal trunk.
MATERIALS AND METHODS: The origin, course, dimensions, and related variations of the tentorial artery were studied in 10 formalin-fixed human cadaveric si

It is a fact that image noise influences for image qualities of the brain CT are known widely. The same principle applies to 3D-CTA, image noise significantly influences the depiction of blood vessels. So we evaluated the use of CT-AEC for 3D-CTA to optimize the scan dose and control the depiction of cerebral vessels by CT-AEC. We decided to optimize the noise index (NI) for cerebral 3D-CTA through the use of an imitation blood phantom. In the evaluation of the depiction of the anterior choroidal artery that was able to be confirmed with DSA, the detection rate in high resolution mode: NI 6.0

In a 41-year-old woman with a ruptured anterior communicating artery aneurysm, cerebral angiography incidentally showed an absence of the right common carotid artery. The right internal and external carotid artery originated from the ipsilateral inominate artery. The absence of the common carotid artery is extremely rare and association with a ruptured cerebral aneurysm is even less common. A description of the case and review of the literature are reported.

OBJECTIVE: The diagnosis of isolated central nervous system angiitis often requires both cerebral angiography and biopsy for diagnosis.
CLINICAL PRESENTATION: We present a case of gliomatosis cerebri with demonstrated angiographic changes suggestive of a central nervous system vasculitis.
INTERVENTION: A 47-year-old woman presented with a 2-week history of worsening headache and progressive drowsiness. Magnetic resonance imaging scans revealed confluent nonenhancing T2-weighted hyperintensity involving the frontal white matter bilaterally, hypothalamus, and both thalami. A four-vessel angiog

A 37-year-old man with Behçet's disease suffered secondary subarachnoid hemorrhage and intracerebral hematoma in the right temporal lobe caused by a ruptured aneurysm of the right middle cerebral artery. The aneurysm was successfully clipped. Aneurysm formation is common in the visceral arteries in Behçet's disease, but extremely rare in the intracranial arteries. Vasculitis may be involved in the etiology of intracranial aneurysms in patients with Behçet's disease.

BACKGROUND: Intrapancreatic accessory spleens are frequently confused with primary pancreatic tumors, and differentiation from neoplastic lesions is important to avoid an unnecessary laparotomy. We present three cases of intrapancreatic accessory spleen evaluated by computed tomographic arteriography (CTA) and discuss the characteristic findings.
METHODS: CTA was performed, followed by digital subtraction angiography, with an injection of contrast material through a 4-F catheter placed in the celiac artery. Single-level dynamic CTA was also performed in two patients with a 30-s continuous sca

Clearly identifiable intraoperative landmarks render the placement of intraoperative stents difficult. Preoperative use of quantitative digital angiography helps the surgeon accurately insert endovascular stents intraoperatively. By using defined points of reference, we were able to carefully select the size and lengths of stents before the operation and precisely place these stents in the operating room. Furthermore, we have been able to redilate these stents using the same techniques at subsequent operations. Our results reflect the efficacy of this technique.

The case of a 21-year-old female patient with neck pain and brain stem symptomatology after appendectomy is reported. Duplex sonographic findings were compatible with right vertebral artery dissection and occlusion of the vessel. Angiography confirmed the diagnosis. The patient recovered within weeks and was free of symptoms 6 months after the acute episode. Dissection of the vertebral artery can occur with or without minor trauma. Associations with fibromuscular dysplasia, arterial hypertension and the use of oral contraceptives have been reported. As in our own patient, dissection can appear

The origin of the vertebral artery may be difficult to show on sonography or conventional angiography. Our aim was to evaluate the accuracy of CT angiography (CTA) in detecting arteriosclerotic changes in the first segment (V1) of the vertebral artery. We performed CTA and intra-arterial digital subtraction angiography (DSA) on 24 patients with vertebrobasilar insufficiency. The ostium and the V1 segment were examined. Stenosis was assessed on a three-grade scale, and calcification and the degree of kinking were recorded. DSA and CTA results were compared. The ostium of the artery was seen in

BACKGROUND AND PURPOSE: It is important to recognize a perimesencephalic pattern of hemorrhage in patients with subarachnoid hemorrhage (SAH), because in 95% of these patients the cause is nonaneurysmal and the prognosis is excellent. The purpose of this study was to investigate whether CT angiography can accurately exclude vertebrobasilar aneurysms in patients with perimesencephalic patterns of hemorrhage and therefore replace digital subtraction angiography (DSA) in this setting.
METHODS: In 40 patients with posterior fossa SAH as shown on unenhanced CT, 2 radiologists independently evaluat
