Dexmedetomidine in the emergency department: assessing safety and effectiveness in difficult-to-sedate acute behavioural disturbance.

OBJECTIVES: To investigate the safety and effectiveness of dexmedetomidine for sedating patients in whom previous attempts at sedation in the emergency department have failed. METHODS: A study was carried out on dexmedetomidine for sedation of patients with acute behavioural disturbance for whom at least two previous attempts at sedation with other drugs had failed. Either a loading dose of dexmedetomidine was administered or a loading dose then an infusion. Administration was titrated to the sedative effect and vital signs. The sedation assessment tool was used to assess effectiveness, and a

Predictors of short- and long-term adherence to a daily walking program in persons with Alzheimer's disease.

OBJECTIVE: To examine factors associated with adherence to a walking program in community-dwelling individuals with Alzheimer's disease (AD). METHODS: Data were analyzed for 66 participants with AD asked to walk 30 continuous minutes per day. Adherence data (number of days walked, minutes walked, days walked 30+ minutes) were obtained from daily logs. Predictor variables included age, spousal relationship, health limitations, depression, participant dementia severity and behavioral disturbance, and caregiver stress and dementia management style. RESULTS: During week 1, participants significa

Cross-gender violence perpetration and victimization among early adolescents and associations with attitudes toward dating conflict.

This study examined gender differences in cross-gender violence perpetration and victimization (ranging from mild, e.g., push, to severe, e.g., assault with a knife or gun) and attitudes toward dating conflict, among an urban sample of 601 early adolescents (78% African-American). Comparisons across gender groups for cross-gender (e.g., female-to-male) violence perpetration and victimization indicated higher levels of perpetration for girls and higher levels of victimization for boys. Girls also reported higher levels of verbal and physical violence toward partners with regard to attitudes tow

The Brief Child and Family Phone Interview (BCFPI): 1. Rationale, development, and description of a computerized children's mental health intake and outcome assessment tool.

BACKGROUND: This study describes the development of the Brief Child and Family Phone Interview (BCFPI) - a computer-assisted telephone interview which adapts the revised Ontario Child Health Study's (OCHS-R) parent, teacher, and youth self-report scales for administration as intake screening and treatment outcome measures in children's mental health services. It focuses on the factor structure of the BCFPI's hypothesized parent-reported child mental health scales describing attention-deficit hyperactivity disorder (ADHD), oppositional defiant disorder (ODD), conduct disorder (CD), separation a

Psychiatric Disorders and Function in Adolescents with Tetralogy of Fallot.

OBJECTIVES: To assess psychiatric disorders and function in adolescents with repaired tetralogy of Fallot (TOF) without and with a genetic diagnosis and to evaluate associations of functioning with medical factors, IQ, and demographics. STUDY DESIGN: Adolescents with TOF (n = 91) and 87 healthy referents completed a clinician-rated structured psychiatric interview, parent-/self-report measures of psychopathology, and brain magnetic resonance imaging. Twenty-three of the adolescents with TOF had a known genetic diagnosis. RESULTS: The prevalence of anxiety disorders did not differ significant

Neuroimaging the neural correlates of increased risk for substance use disorders in attention-deficit/hyperactivity disorder-A systematic review.

BACKGROUND/OBJECTIVES: Children with attention-deficit/hyperactivity disorder (ADHD) are nearly three times more likely to develop substance use disorders (SUD) than their typically developing peers. Our objective was to review the existing neuroimaging research on high-risk ADHD (ie, ADHD with disruptive behavior disorders, familial SUD and/or early substance use), focusing on impulsivity as one possible mechanism underlying SUD risk. METHODS: A PubMed literature search was conducted using combinations of the keywords "ADHD," "substance use," "substance use disorder," "SUD," "addiction," "de

Disrupted expected value signaling in youth with disruptive behavior disorders to environmental reinforcers.

OBJECTIVE: Youth with disruptive behavior disorders (DBD), including conduct disorder (CD) and oppositional defiant disorder (ODD), have difficulties in reinforcement-based decision making, the neural basis of which is poorly understood. Studies examining decision making in youth with DBD have revealed reduced reward responses within the ventromedial prefrontal cortex/orbitofrontal cortex (vmPFC/OFC), increased responses to unexpected punishment within the vmPFC and striatum, and reduced use of expected value information in the anterior insula cortex and dorsal anterior cingulate cortex during

Stimulus context and motor preparation in attention-deficit/hyperactivity disorder.

AIM: To investigate (1) whether and how local stimulus context variation may modify behavioural and preparatory motor processes in children, and (2) if these effects differ between healthy children and children with attention-deficit/hyperactivity disorder (ADHD) aged 9-12 years. METHODS: Behavioural parameters and contingent negative variation (CNV) at cortical motor electrodes were recorded during a cued continuous performance task (AX-CPT) in three stimulus context conditions (Go, NoGo, neutral). Stimulus context was varied on the basis of stimulus types preceding the cue letter A. RESULT

Neurodevelopment in patients with repaired tetralogy of Fallot.

Neurodevelopmental sequelae are prevalent and debilitating for patients with congenital heart defects. Patients born with tetralogy of Fallot (TOF) are susceptible for abnormal neurodevelopment as they have several risk factors surrounding the perinatal and perioperative period. Some risk factors have been well described in other forms of congenital heart defects, including transposition of the great arteries and single ventricle heart disease, but they have been less studied in the growing population of survivors of TOF surgery, particularly in infancy and childhood. Adolescents with TOF, eve

Maternal postpartum depressive symptoms partially mediate the association between preterm birth and mental and behavioral disorders in children.

Preterm birth has been linked with postpartum depressive (PPD) disorders and high symptom levels, but evidence remains conflicting and limited in quality. It remains unclear whether PPD symptoms of mothers with preterm babies were already elevated before childbirth, and whether PPD symptoms mediate/aggravate the effect of preterm birth on child mental disorders. We examined whether preterm birth associated with maternal PPD symptoms, depressive symptoms trajectories from antenatal to postpartum stage, and whether PPD symptoms mediated/aggravated associations between preterm birth and child men

Understanding desisting and persisting forms of delinquency: the unique contributions of disruptive behavior disorders and interpersonal callousness.

BACKGROUND: While associations between conduct disorder (CD), oppositional defiant disorder (ODD), attention deficit hyperactivity disorder (ADHD), and interpersonal callousness (IC) symptoms and delinquency onset are well established, less is known about whether these characteristics differentiate desisting and persisting delinquency. The current study examined whether childhood and adolescent CD, ODD, ADHD, and IC symptoms uniquely distinguished boys who exhibited persisting versus desisting delinquency from adolescence into adulthood. METHODS: Participants were 503 boys (57% African Americ

Sex differences in autonomic correlates of conduct problems and aggression.

OBJECTIVE: To examine sex differences in autonomic nervous system functioning in children and adolescents with conduct problems and to evaluate the role of aggression in predicting autonomic nervous system functioning, over and above the effects of disruptive behavior. Although deficiencies in autonomic responding among boys with oppositional defiant disorder and/or conduct disorder are well documented, it remains unclear whether such findings extend to girls or apply only to children with aggressive forms of conduct problems. METHOD: Electrodermal responding, cardiac pre-ejection period, and

Psychosocial adversities underestimated in hyperkinetic children.

Abnormal psychosocial factors, assessed both clinically and by raters blind to clinical presentation, were examined in 21 hyperkinetic and 26 conduct disordered children. Blind raters found the frequency of psychosocial adversities to be similar in the two disorders. By contrast the clinical rater, who did know the diagnosis of patients, rated adverse psychosocial situations as much lower in hyperkinetic children than in children with conduct disorder. Logistic regression showed, particularly in the category of abnormal intrafamilial relationships (Lack of warmth in parent child relationship,

Animal models of neuropsychiatric disorders.

We survey the utility of animal models of mental illness, based on the identification of possible neurocognitive or neurobehavioral endophenotypes. Three broad clusters of neuropsychiatric disorder are discussed: (a) impulsive-compulsive syndromes, comprising drug addiction, attention deficit/hyperactivity disorder, gambling, obsessive-compulsive disorder, and compulsive eating; (b) disorders at the cognitive-emotional interface, comprising anxiety, depression, and schizophrenia; and (c) disorders purely of cognition, which contribute to the third cluster, cognitive disorders. The emphasis is

[Combination of dialectic and behavioral therapy (DBT) and duloxetin in kleptomania].

OBJECTIVE: Kleptomania is characterized by repetitive stealing of things not needed for personal use. Comorbidity with affective disorders, personality disorders and attention deficit hyperactivity disorder (ADHD) is often described. Pharmacological treatment options are serotonin reuptake inhibitors, naltrexone or mood stabilizers. There are no controlled studies for cognitive-behavioral (CBT) and psychodynamic psychotherapies. However, literature suggests that patients respond to CBT, especially when combined with pharmacological strategies. Dialectical behavioral therapy (DBT) is successful

Antipsychotic Prescribing and Safety Monitoring Practices in Children and Youth: A Population-Based Study in Alberta, Canada.

BACKGROUND AND OBJECTIVE: Antipsychotic medication use has steadily increased in Canada, with an expansion in the profile of users and the diagnoses for which they are used. The use of antipsychotics is associated with a number of adverse effects for which routine monitoring is recommended. The objectives of this study were to determine the most common diagnoses associated with antipsychotic use in children in Alberta, Canada and the proportion who receive recommended laboratory tests for adverse effects. METHODS: Data on dispensed antipsychotics, diagnoses, prescribers, and laboratory testin

Clinical Characteristics of Preschool Children with Oppositional Defiant Disorder and Callous-Unemotional Traits.

There is a need to know whether callous-unemotional (CU) traits identify a more severe group of oppositional defiant children (ODD). The aim of this study is to ascertain cross-sectionally and longitudinally the specific contribution of CU levels and the presence of ODD in the psychological state of preschool children from the general population. A total of 622 children were assessed longitudinally at ages 3 and 5 with a semi-structured diagnostic interview and questionnaires filled out by parents and teachers. In multivariate models simultaneously including ODD diagnosis and CU levels, contro

Substance exposure in utero and developmental consequences in adolescence: a systematic review.

BACKGROUND: The impacts of maternal substance use have been observed in both research and clinical experience. Several studies have shown that preschool children are at heightened risk of developing various cognitive, behavioral, and socioemotional difficulties. Most knowledge has been generated concerning alcohol consumption during pregnancy and the postnatal effects thereof. Less is known about substance use other than alcohol (for instance, opiates, marijuana, and cocaine) during pregnancy and the long-term developmental consequences. OBJECTIVE: The aims of this review are to identify rele

A controlled longitudinal 5-year follow-up study of children at high and low risk for panic disorder and major depression.

BACKGROUND: To evaluate the longitudinal course of psychiatric disorders in children of parents with panic disorder (PD) and major depression (MD) as they transition through the period of risk from childhood into adolescence. METHOD: Over a 5-year follow-up, we compared psychiatric disorders in four groups of children: (1) offspring of parents with PD plus MD (n=136); (2) offspring of parents with PD without MD (n=27); (3) offspring of parents with MD but without PD (n=53); and (4) offspring of non-PD non-MD parents (n=103).
