Maladaptive behavior in survivors: dysexecutive survivor syndrome.

This paper attempts to answer the question: why does normal, goal-directed, purposeful, and coordinated behavior fragment in a survival situation? Events accompanying the initial impact phase of a survival incident are characterized by speed, danger, violence, and uncontrollability. The following recoil phase is known to produce behavioral and cognitive impairment that leads to a reduced ability to produce a response that is meaningful and may result in tonic immobility. The author argues that the commonly witnessed responses among survivors comprise a subset of known behaviors, including loss

Hypoxia training: symptom replication in experienced military aircrew.

INTRODUCTION: Military aircrew are trained to recognize the signs and symptoms of hypoxia in a safe environment using a variety of methods to simulate altitude. In order to investigate the effectiveness of hypoxia training, this study compared the recall of hypoxia symptoms in military aircrew between two consecutive hypobaric chamber hypoxia training sessions conducted, on average, 4.5 yr apart. METHODS: Previously trained subjects completed a questionnaire immediately before and after they underwent refresher hypoxia training and recorded the occurrence, order, and severity of symptoms expe

Low-dose chest computed tomography as a screening tool: findings in 536 aircrews.

BACKGROUND: Plain chest radiographs and pulmonary function tests have been used for pulmonary screening for flight duties of aircrews of the Republic of Korea Air Force. However, the screening accuracy of plain chest radiographs is controversial. Chest CT imaging with low-dose protocol (LDCT) improves detection of intra-thoracic abnormalities compared to plain chest radiographs. The aim of this study was to assess the influence of LDCT on flight duties of aircrews and to investigate their radiologic findings. METHODS: From June 2009 to May 2011, the Aerospace Medical Center screened asymptoma

A fighter pilot case of pulmonary sequestration.

BACKGROUND: Pulmonary sequestration is a rare congenital malformation. The diagnosis of pulmonary sequestration is based on the identification of an abnormal arterial supply. Conventional treatment for pulmonary sequestration is surgical removal of the tissue, which may induce serious trauma and requires a long recovery time. Recently, endovascular treatment has become feasible as a safer and less invasive method to treat pulmonary sequestration. CASE REPORT: To the best of our knowledge, pulmonary sequestration has not been reported in pilots. In this study, we report a fighter pilot case of

Ocular vestibular evoked myogenic potentials and motion sickness susceptibility.

BACKGROUND: A functional vestibular system is a prerequisite for motion sickness (MS) to occur. The otolithic organs (saccule and utricle) are currently considered to be important in the pathogenesis of MS. Recent investigations have demonstrated that the ocular vestibular evoked myogenic potentials (oVEMP) test is an objective evaluation of utricular function. To date, there is no functional guide that can serve as a diagnostic tool for individual susceptibility to MS. Therefore, we investigated the link between oVEMP and MS susceptibility. METHOD: The oVEMP test was performed on 31 MS susce

Oculo-vestibular recoupling using galvanic vestibular stimulation to mitigate simulator sickness.

INTRODUCTION: Despite improvement in the computational capabilities of visual displays in flight simulators, intersensory visual-vestibular conflict remains the leading cause of simulator sickness (SS). By using galvanic vestibular stimulation (GVS), the vestibular system can be synchronized with a moving visual field in order to lessen the mismatch of sensory inputs thought to result in SS. METHODS: A multisite electrode array was used to deliver combinations of GVS in 21 normal subjects. Optimal electrode combinations were identified and used to establish GVS dose-response predictions for t

Commercial spaceflight participant G-force tolerance during centrifuge-simulated suborbital flight.

INTRODUCTION: Medical knowledge of the human body in microgravity and hypergravity is based upon studies of healthy individuals well-conditioned for such environments. Little data exist regarding the effects of spaceflight on untrained commercial passengers. We examined the responses of potential spaceflight participants (SFP) to centrifuge G-force exposure. METHODS: There were 77 individuals (65 men, 12 women), 22-88 yr old, who underwent 6 centrifuge runs over 48 h. Day 1 consisted of two +Gz runs (peak = 3.5+Gz, run 2) and two +Gx runs (peak = 6.0+Gx, run 4). Day 2 consisted of two runs ap

Peripheral neuropathy of the extremity after hyperbaric exposure.

BACKGROUND: Neurological deficits occurring after diving or hyperbaric exposure are typically due to central nervous system decompression illness (DCI). However, consideration of alternate diagnoses is sometimes warranted. CASE REPORT: A 47-yr-old female hyperbaric technician developed foot drop 2 d following her last hyperbaric exposure. She had worked in chamber once daily at 45 fsw for 90 min. The breathing gas was air until the last 15 min, during which she breathed oxygen. Her history was complicated by a gastric bypass and weight loss totaling 160 pounds within the preceding 9 mo. She w

Aeromedical evacuation using an aircraft transit isolator of a patient with Lassa fever.

BACKGROUND: Lassa fever is a viral hemorrhagic fever only present in West Africa. The mortality rate is 1% and may reach 15% among hospitalized patients. Transmission between humans is mostly due to direct contact with infected body fluids. Aeromedical evacuation of patients with viral hemorrhagic fevers (such as Lassa fever) demands strict isolation measures. Only a few cases of such evacuations have been reported in the literature during the last 40 yr. The use of an aircraft transit isolator device could be helpful. CASE REPORT: We report the aeromedical evacuation of a confirmed Lassa fev

A rationale for space motion sickness.

Space motion sickness (SMS) still remains a problematic nuisance for a majority of fliers during their first 1 to 3 days in space and is an enigma to all concerned after almost half a century of spaceflight and extensive ground and in-flight studies. There is no known etiology consistent with observed signs and symptoms, and attempts to produce pre- and in-flight countermeasures have largely failed. Some medications for motion sickness (MS) have been partially effective, but must be used with caution due to their side effects. A hypothesis regarding SMS that is consistent with current knowledg

Weight loss in humans in space.

INTRODUCTION: Bodyweight loss during spaceflight has been observed among astronauts since the early space missions. Considerable mission data has been accumulated, including data from female astronauts, on the many Shuttle and International Space Station missions. The purpose of this study was to investigate the association between observed weight loss during spaceflight and potential covariate factors. METHODS: We performed a statistical analysis of the association between bodyweight change and plausible clinical and mission covariates, using data obtained from the NASA Longitudinal Study of

Tolerance to extended galvanic vestibular stimulation: optimal exposure for astronaut training.

BACKGROUND: We have developed an analogue of postflight sensorimotor dysfunction in astronauts using pseudorandom galvanic vestibular stimulation (GVS). To date there has been no study of the effects of extended GVS on human subjects and our aim was to determine optimal exposure for astronaut training based on tolerance to intermittent and continuous galvanic stimulation. METHODS: There were 60 subjects who were exposed to a total of 10.5 min of intermittent GVS at a peak current of 3.5 mA or 5 mA. A subset of 24 subjects who tolerated the intermittent stimulus were subsequently exposed to 20

Various anti-motion sickness drugs and core body temperature changes.

INTRODUCTION: Blood flow changes and inactivity associated with motion sickness appear to exacerbate the rate of core temperature decrease during subsequent body cooling. We investigated the effects of various classes of anti-motion sickness drugs on core temperature changes. METHODS: There were 12 healthy male and female subjects (20-35 yr old) who were given selected classes of anti-motion sickness drugs prior to vestibular Coriolis cross coupling induced by graded yaw rotation and periodic pitch-forward head movements in the sagittal plane. All subjects were then immersed in water at 18 de

Myasthenia gravis and return to flying status.

INTRODUCTION: Myasthenia gravis (MG) is a common primary disorder of neuromuscular transmission. Although MG was once a fatal disease, today treatment with immunomodulating agents and cholinomimetic medications with good supportive care have improved the prognosis and the ability of patients to adapt to their workplaces, including the flight environment. CASES: The diagnosis of MG in two aircrew members illustrates the range of severity for MG from isolated ocular symptoms to relentlessly progressive generalized disease, as well as the unpredictability of the disease and difficulty in treatme
