Targeted therapy with polymeric nanoparticles in PBRM1-mutant biliary tract cancers: Harnessing DNA damage repair mechanisms.

Biliary tract cancers (BTCs) are aggressive malignancies with a dismal prognosis that require intensive targeted therapy. Approximately 10 % of BTCs have PBRM1 mutations, which impede DNA damage repair pathways and make cancer cells more susceptible to DNA-damaging chemicals. This review focus on development of poly(lactic-co-glycolic acid) (PLGA)-based nanoparticles targeting delivery system to selectively deliver chemotherapy into PBRM1-deficient BTC cells. These nanoparticles improve therapy efficacy by increasing medication targeting and retention at tumour locations. In preclinical studie…

Different biliary tract cancers, same operation: Importance of cancer origin in patients with hilar-invading tumors.

BACKGROUND AND AIMS: For patients with biliary tract cancer involving the hepatic hilum, major hepatic resection with extrahepatic bile duct resection may be required. In addition to perihilar cholangiocarcinoma (PHCC), the same extent of surgery is used in advanced gallbladder cancer (GBC) and intrahepatic cholangiocarcinoma (IHCC) with hilar involvement. Few studies compare prognostic factors and long-term outcomes across tumor types. This study compared risk characteristics and outcomes after surgery in all subtypes of biliary tract cancer with hilar involvement. METHODS: Patients with bil…

Complement in local biliary tract defense: dissociation between bile complement and acute phase reactants in cholecystitis.

Biliary complement concentrations and activity are lower in patients with infected bile than in those with sterile bile in cholecystitis. Plasma complement is increased during the acute phase response to inflammation. To determine whether low biliary complement in infected bile is a specific response to biliary tract infection or part of a general systemic reaction, we analyzed bile complement proteins (C3 and C4) and activity (C4H50) and acute phase reactants fibronectin, C-reactive protein, and alpha 1-antitrypsin concentrations in acute and chronic cholecystitis. Results were correlated wit…

Next-Generation Sequencing of Circulating Tumor DNA for Precision Medicine in Patients with Advanced Biliary Tract Cancers.

PURPOSE: Next-generation sequencing (NGS) tissue testing is recommended in advanced biliary tract cancers (aBTC) due to the increasing approval of targeted therapies. However, tissue NGS is frequently limited by poor access and insufficient tumor material. We exploited circulating tumor DNA (ctDNA) as an alternative for molecular profiling in aBTC. EXPERIMENTAL DESIGN: We retrospectively analyzed 10,937 patients with aBTC to characterize the molecular landscape and assess the clinical validity of ctDNA profiling. In a second retrospective cohort of 126 patients from nine Spanish referral cent…

Robotic Resection of Right Perihilar Intrahepatic Cholangiocarcinoma: Practical Technique of Major Hepatic Resection for Biliary Tract Cancer.

Intrahepatic juxtahilar cholangiocarcinoma frequently necessitates major hepatic resection with radical regional lymphadenectomy to achieve oncologic clearance and accurate staging; however, standardized easy-to-follow robotic techniques for centrally located tumors remain limited. Herein, we present a fully robotic right hepatectomy with formal portal lymphadenectomy in a 76-year-old woman with a 5.5-cm centrally located intrahepatic cholangiocarcinoma with underlying hepatic steatosis. Preoperative evaluation included cross-sectional imaging and volumetric assessment to ensure adequate futur

Broadening the gates: Analysis of potentially modifiable study entry criteria in pancreatic and biliary tract cancer trials.

BACKGROUND: Eligibility criteria for clinical trials are crucial for maintaining safety and study integrity. However, overly restrictive criteria can result in unrepresentative trial populations, leading to gaps in understanding real-world treatment efficacy. Addressing potentially modifiable exclusions (PMEs) could enhance trial accessibility and participation without compromising safety. METHODS: The authors retrospectively analyzed screen-failed patients with biliary tract cancer or pancreatic cancer between August 2019 and November 2024 at The University of Texas MD Anderson Cancer Center…
