Cerebral abscess with multiple rims of MRI.

We present a patient with multiple brain abscesses caused by Nocardia asteroides. On T2-weighted MRI, multiple concentric rims were seen in the abscess, which could be a finding specific for infection. The rims may be due to organization of the necrotic debris and phagocytoses by macrophages in the capsule.

Metastatic community acquired Klebsiella pneumonia infection, secondary to skin and soft tissue infection: A case report.

Klebsiella pneumonia is known to cause hospital-acquired infections, primarily in immunocompromised patients. Recently, a distinct syndrome of community-acquired invasive Klebsiella pneumonia infection has been observed, mainly in the Southeast Asian population. This syndrome is associated with disseminated infection and the formation of multiple organ abscesses. Affected organs include the liver, the meninges, the brain, the eyes, and rarely the skin and soft tissue. Most of the affected patients suffer from diabetes mellitus. We present a case of invasive community-acquired Klebsiella pneumo

Rapidly Progressing Intramedullary Spinal Cord Abscess: A Case Report.

Intramedullary spinal cord abscess is a rare and severe infectious disease characterized by devastating neurological deficits. We report a c … audate nucleus. The patient died 19 days after admission. Our report is the first case of extensive brain abscess development over a short period. Based on our experience, prompt administration of antibiotics and emergency abscess drainage of the cervical cord (and ventricular drainage, if necessary) are recommended in cases of neurological deterioration in pat …

Cerebral mucormycosis (Phycomycosis) in a heroin addict.

Cerebral mucormycosis was found at autopsy in a 27-year-old male narcotic addict with rapidly progressive focal neurologic signs. There was no evidence of rhinocerebral involvement of systemic illness, and a nuclide brain scan suggested brain abscess.

Cerebral Pseudallescheria boydii infection: unique occurrence of fungus ball formation in the brain.

Pseudallescheria (Petriellidium) boydii is the most frequent etiologic agent of mycetoma in temperate regions of the world. In addition, it may also occur as a systemic infection in immunocompromised patients. Infection of the central nervous system is rare, and only eleven cases of Pseudallescheria brain abscess have been documented. We report a leukemic patient in whom disseminated pseudallescheriasis was diagnosed only after death. This case is unique in that a white matter cavitation was present containing a fungoma of Pseudallescheria, the first report of a cerebral fungus ball caused by …

REPRINTED FROM THE 2023 HYPERBARIC INDICATIONS MANUAL 15th Edition:Intracranial Abscess.

The term "intracranial abscess" (ICA) includes cerebral abscess, subdural empyema, and epidural empyema, which share many diagnostic and therapeutic similarities and, frequently, very similar etiologies. Infection may occur and spread from a contiguous infection such as sinusitis, otitis, mastoiditis, or dental infection; hematogenous seeding; or cranial trauma. Brain abscess usually results from predisposing factors such as HIV infection, immunosuppressive drug treatment, surgery, adjacent infection (i.e., mastoiditis, sinusitis, dental infection), or systemic infection causing bacteremia. Ap…

A Challenging Picture of Cancer-and Inflammation-Related Changes.

The authors describe a diagnostically challenging case where a patient with previous squamous cell carcinoma of the tonsil developed a putative second primary squamous cell carcinoma in the lung (stage IV with lung and bone metastases). During palliative chemotherapy several episodes of severe infection occurred, eventually resulting in abscess formation in the hip and brain. The dilemma of distinguishing between metastasis and abscess and the therapeutic implications are discussed.

Extensive actinomycosis with intracranial and mediastinal involvement: a therapeutic challenge.

Central nervous system (CNS) involvement by actinomycosis is rare, seen in 2%-3% cases. It mostly spreads to CNS by haematogenous route from a distant primary site such as oral cavity, lung, abdomen or pelvis. Direct CNS extension can also occur. It mostly presents as brain abscess, meningoencephalitis, actinomycetoma, subdural empyema and epidural abscess. We report one case of extensive actinomycosis having intra and extraparenchymal CNS, spinal canal, retropharyngeal and mediastinal involvement. Due to such widespread extension and involvement of vital areas, complete surgical debulking was…

[A Case of Bacterial Meningitis in Which Occurrence of Ataxie Optique Led to Detection of a Small Subdural Abscess and Pialitis].

We report the case of a 69-year-old man with bacterial meningitis who presented with ataxie optique in the peripheral part of the left visual field in both hands. A detailed neurological examination with contrast-enhanced brain MRI in the early stage of the clinical course identified a small subdural abscess and pialitis in the right parietal area. A favorable outcome was obtained with antibiotic therapy alone. In a case with higher brain dysfunction of unknown cause in the clinical course of bacterial meningitis, a detailed neurological examination may be helpful to identify the causative sit

Pediatric Meningococcal Meningitis Complicated by Cerebellar Abscess: A Case Report.

Neisseria meningitidis is a well-recognized cause of bacterial meningitis in infants and adolescents. Its intracranial suppurative complications, such as brain abscess, are exceedingly rare. Only a few published case reports describe this complication, most involving neonates. We report a case of a previously healthy 11-year-old male who presented with a one-day history of acute fever, abdominal pain, vomiting, and irritability. On investigation, cerebrospinal fluid analysis was consistent with bacterial meningitis; polymerase chain reaction detected Neisseria meningitidis, confirming meningoc…

Intracranial infection in cardiac transplant recipients.

Infections have produced most of the deaths in the Stanford cardiac transplant program. Of the first 182 transplant recipients, 27 developed … 30 days after immunosuppressive therapy for treatment of rejection was increased. The prognosis for brain abscess depended on the causative organism. All patients with aspergillus infection died despite treatment with amphotericin B. The toxoplasma abscess responded to a combination of sulfadiazine and pyrimethamine. Meningitis was successfully suppressed or cu …

[Pre-septal orbital cellulitis from odentogenic origin--combined surgical and endodontic approach: a case report].

Orbital abscesses are common infectious diseases. The etiology of orbital abscesses may vary from common sinusitis in most of the cases, to cocaine sniffing. As a result of the proximity to the brain, orbital abscesses may complicate to life threatening situations. The infectious process spreads to the orbit in several ways: hematogenous, via anatomic spaces in the maxillofacial region, direct invasion. The treatment philosophy combines surgical and Antibiotic therapy to resolve the acute phase of the disease, followed by elimination of the source. The purpose of this paper is to report a case
