A numerical simulation study of airway flow: Impact of bronchial stenosis.

Obstructive lung diseases, marked by airway stenosis, are chronic and pose significant mortality risks. This study aims to analyze airflow patterns in obstructed bronchi, comparing them to healthy airways during tidal breathing to improve our comprehension of disease effects on respiratory function. The current studies mostly overlook the specific morphology of the patient's upper airway or the elastic deformation of the airway soft tissues, which results in the existing results not being sufficient to effectively guide surgical treatment. In this paper, a realistic model of bronchial stenosis…

Computational fluid dynamics comparison of the upper airway velocity, pressure, and resistance in cats using an endotracheal tube or a supraglottic airway device.

INTODUCTION: In veterinary medicine, airway management of cats under general anesthesia is performed with an endotracheal tube (ETT) or supraglottic airway device (SGAD). This study aims to describe the use of computational fluid dynamics (CFD) to assess the velocities, pressures, and resistances of cats with ETT or SGAD. METHODS: A geometrical reconstruction model of the device, trachea, and lobar bronchi was carried out from computed tomography (CT) scans that include the head, neck, and thorax. Twenty CT scans of cats under general anesthesia using ETT (n = 10) and SGAD (n = 10) were model…

A novel method of Uniblocker placement: extraluminal technique supported by trachea length measurement: A CONSORT-compliant article.

BACKGROUND: The use of bronchial blockers has been increased for one-lung ventilation; however, the placement of bronchial blockers is time consuming. The objective of this study was to compare the novel extraluminal technique of Uniblocker placement supported by trachea length measurement on computerized tomography images with conventional intraluminal Uniblocker placement method. METHODS: Seventy adult patients undergoing left side thoracic surgery were included in the study. All the patients were randomly assigned to one of two groups: conventional intraluminal intubation group (CV-IN grou…

Paediatric tracheal carcinoid with carinal involvement and nodal metastasis: challenging airway reconstruction with oncological clearance.

Tracheal carcinoid tumours are exceptionally rare neuroendocrine neoplasms, particularly in the paediatric population. We present the case of a teenage girl with longstanding dyspnoea and intermittent haemoptysis, ultimately diagnosed with a typical carcinoid involving the distal trachea and both main bronchi. Bronchoscopy biopsy confirmed the diagnosis. She underwent tracheal resection and complex carinal reconstruction with cardiopulmonary bypass. Histopathology revealed a grade 1 typical carcinoid with metastatic involvement in two of five subcarinal lymph nodes. Postoperative recovery was …

Early manifestation of ARDS in COVID-19 infection in a 51- year-old man affected by Mounier-Kuhn syndrome.

A 51-year-old man known to be affected by Mounier-Kuhn syndrome (MKS). presented to Accident & Emergecy (A&E) with fever, dyspnea and deterioration of his chronic coughs. Increased diameters of his trachea (39 mm), right (30 mm) and left (26 mm) main bronchi were revealed by chest computerized tomography (CT) scan. CT scan showed also ground-glass opacities (GGO) and bronchiectasis in the mid and lower zones of both lungs. COIVD-19 infection was eventually confirmed by RT-PCR. A severe form of COVID-19 could occur even in the early stages of the disease in presence of underlying co-morbidities…

Pulmonary Tumor Thrombotic Microangiopathy Caused by a Parotid Tumor: Early Antemortem Diagnosis and Long-term Survival.

Pulmonary tumor thrombotic microangiopathy (PTTM) is a high-mortality disease that is difficult to diagnose clinically. Our patient was an 80-year-old woman who came to us due to symptoms of increasing dyspnea. A clinical evaluation showed that she had hypoxemia and pulmonary arterial hypertension without any abnormalities in the major pulmonary arteries, bronchi, or alveoli. A lung perfusion scan showed multiple wedge-shaped perfusion defects. Further examination revealed adenocarcinoma in her right parotid gland with metastasis to the submandibular lymph nodes. We diagnosed her to have PTTM …

Glued lungs: Sticky and tricky.

We report a male patient who underwent bronchoscopic instillation of glue to control moderate hemoptysis which later led to the development of postobstructive pneumonia and extensive foreign body reaction in the bronchial wall and the lung distal to the glue application. He continued to have intermittent hemoptysis and underwent bronchial artery embolization. However, recurrent moderate hemoptysis eventually led to pneumonectomy, which showed severe foreign body reaction in bronchi- and post-obstructive changes in the lung parenchyma and the draining lymph nodes. This case highlights a serious…

Left main bronchus compression due to main pulmonary artery dilatation in pulmonary hypertension: two case reports.

Pulmonary arterial dilatation associated with pulmonary hypertension may result in significant compression of local structures. Left main coronary artery and left recurrent laryngeal nerve compression have been described. Tracheobronchial compression from pulmonary arterial dilatation is rare in adults, and there are no reports in the literature of its occurrence in idiopathic pulmonary arterial hypertension. Compression in infants with congenital heart disease has been well described. We report 2 cases of tracheobronchial compression: first, an adult patient with idiopathic pulmonary arterial…

Update on multidetector computed tomography imaging of the airways.

Recent advances in multidetector computed tomography (MDCT) technology have transformed the imaging evaluation of the trachea and bronchi. Multiplanar 2-dimensional and 3-dimensional volume reconstruction techniques, including external rendering and virtual bronchoscopy, can be generated in mere minutes, thereby complementing conventional axial CT imaging in the depiction of various central airway disease processes including airway stenoses, central airway neoplasms, and congenital airway disorders. Paired inspiratory and dynamic expiratory MDCT imaging, along with newer cine CT imaging method…

Relaxant effect of trapidil on isolated dog bronchus.

Relaxant effects of trapidil on dog bronchi were pharmacologically analyzed in vitro. Trapidil concentration-dependently relaxed bronchial strips contracted with methacholine, histamine or potassium. In the strips under methacholine-contracture, theophylline, papaverine, isoproterenol and atropine produced concentration-dependent relaxations. Propranolol significantly decreased isoproterenol-induced relaxations of bronchial strips under methacholine-contracture, but did not trapidil-induced relaxations. Prostaglandin (PG) E1 and PGE2 in high doses slightly relaxed bronchial strips under methac…

Traumatic tracheo-bronchial ruptures.

A series of 18 ruptures of trachea or bronchi is reported. Gaseous semiology is predominant (pneumomediastinum and subcutaneous emphysema 72%, pneumothorax 67%). Haemoptysis is present in 28%. Assessment of the diagnosis and localization of the rupture was obtained through rigid bronchoscopy, early in the series, and then via fiberoptic endoscopy. Ignored cases in the early phase manifested themselves lately by atelectatic phenomena. Tracheal ruptures (5) were treated by direct repair (1/5) or tracheostomy (3/5) ; 1 died before surgery. Bronchial ruptures (13) were submitted to bronchoplastic …

A Case of Mucus Plug Removal via Bronchoscopy Using a Larger-Sized Endotracheal Tube in a Child With Plastic Bronchitis.

Plastic bronchitis is a severe condition characterised by the formation of dendritic mucus plugs within the trachea and bronchi, leading to respiratory failure due to acute airway obstruction. Bronchoscopic mucus plug removal is considered effective; however, in paediatric patients, it is often difficult to perform due to limitations in tracheal tube size. We report a case of plastic bronchitis associated with influenza A infection during a relapse of nephrotic syndrome. Bronchoscopic mucus plug removal was performed using a cuffed endotracheal tube one size larger than standard, thereby avoid…

A 52-Year-Old Man With an 11-Month History of Fever, Cough, Chest Pain, Pleural Effusion, and Left Lung Atelectasis.

A 52-year-old man was referred to our hospital for cough, fever, chest pain, and progressive dyspnea. He has worked as a full-time security staff at a community center and was in a normal state of health until 11 months prior to referral when he began experiencing cough, expectoration, a high-grade fever (up to 39.7°C), chills, and left chest pain. He visited the local hospital several times with suspected lung cancer. Bronchoscopy showed chronic inflammatory changes in his bronchi. He was given a course of antibiotics, but his fever had not subsided. The patient had visited a bamboo rat farm …

Plastic bronchitis: An unusual complication of acute chest syndrome in adult.

Plastic bronchitis is used to designate endobronchial plugs of rubber-like consistency that form into bronchial trees. It has been described in several diseases like asthma, cystic fibrosis, pulmonary infection, cyanotic congenital heart disease and in few young children with homozygous sickle cell disease. We report the first sickle cell adult case of plastic bronchitis during acute chest syndrome. He developed severe acute respiratory distress syndrome. This unusual presentation related to obstruction by voluminous casts may alert physicians to focus more on the bronchi in sickle cell patien…

Evaluation of HBO2 therapy in pneumatosis cystoides intestinalis.

Pneumatosis cystoides intestinalis (PCI) is a disease characterized by retention of gas in the intestinal wall. Retention of gas can be caused by three mechanisms; gas entry through the intestinal mucosa, gas dissection from the pulmonary alveoli and bronchi, and gas generation in the mucous membrane. Since gas in cysts is composed almost entirely of nitrogen, hyperbaric oxygen therapy (HBO2) is effective for treating PCI due to the oxygen windows effect. However, PCI, caused by a mechanism involving pulmonary alveoli or branches, can become aggravated by HBO2. Therefore, we propose modifying …

[Membranous laryngotracheobronchitis].

Four cases of membranous laryngotracheobronchitis (MLTB) are presented. This entity is defined as an inflammation of larynx, trachea and bronchi, with mucopurulent membranes adhered to subglottic space wall. Radiological study shows irregularity of the proximal tracheal mucosa as well as adhered membranes resembling foreign bodies. Two out of the four children suffered from measles and one had staphylococcal pleuropneumonia. Tracheal aspirate obtained by laryngoscopy yielded S. aureus in two cases, and beta-lactamase positive H. influenzae in another. All patients required nasotracheal intubat…

Bronchial Fissuring: A Severe Complication of Arterial Aneurysm.

Arterial aneurysms are rare and may occur in the context of Behçet's disease. The natural progression of these aneurysms can lead to an increase in their size and eventual rupture into the bronchi, causing life-threatening hemoptysis. We report a case of a subclavian artery aneurysm in a 30-year-old female patient with Behçet's disease who presented with moderate hemoptysis caused by a fistulized left subclavian artery aneurysm into the left main bronchus. The patient was treated with a bolus of corticosteroids followed by oral therapy, and six boluses of cyclophosphamide were scheduled. Unfor…
