Etiologic classification of degenerative mitral valve disease: Barlow's disease and fibroelastic deficiency.

Barlow's disease and fibroelastic deficiency are the two dominant forms of degenerative mitral valve disease and have unique differentiating characteristics on clinical and echocardiographic assessment. Preoperative differentiation of patients by both cardiologists and surgeons is important because the techniques, surgical skill, and expertise required to achieve a repair vary among these etiological subsets. Barlow's patients often have multiple complex lesions, thus high rates of repair are only likely to be achieved by a reference mitral valve repair surgeon. In contrast, many forms of fibr…

Myocardial calcification caused by secondary hyperparathyroidism due to dietary deficiency of calcium and vitamin D.

A 6-year-old girl presented with respiratory distress. Chest radiographs exhibited calcifications in the mediastinum. Further imaging reveal … tly reduced and substantially reduced cardiac calcifications were seen on echocardiogram. Pediatric cardiologists must be aware of this potentially fatal but treatable disease in children with cardiac calcifications unexplained by other causes.

Whipple's disease: the elusive cardiac invader in the shadows of systemic illness case report and literature review of the cardiac implications of Whipple's disease.

Tropheryma whipplei, the causative agent of Whipple's disease, is a rarely diagnosed pathogen that typically presents as a chronic, multisystemic illness. While its role in blood culture-negative endocarditis (BCNE) is increasingly recognised-accounting for approximately 4% of such cases-its other cardiac manifestations remain underappreciated. We report a case that underscores the broader cardiac implications of Whipple's disease, emphasising the diagnostic challenges faced when typical findings are absent. This report aims to raise awareness among cardiologists about the spectrum of cardiac …

Toward Providing Equitable Care for Adults with Congenital Heart Disease: Where We Are and Where We Need to Be.

PURPOSE OF THE REVIEW: To understand the growth of the adults with congenital heart disease (ACHD) population and ACHD work force needs in the United States, and to consider solutions to provide access for this population. RECENT FINDINGS: The ACHD population is growing rapidly and is outpacing current efforts to improve access and delivery of ACHD care.  Achieving adequate access for the ACHD population will require a multipronged approach, including training more ACHD cardiologists and advanced practice providers, expanding use of telehealth, increasing collaboration between larger tertiary…

Managing cardiovascular disease in pregnant people: Defining the pregnancy heart team.

Cardiovascular disease is a leading cause of morbidity and mortality for pregnant patients. A significant portion of cardiac morbidity and mortality is preventable and related to poor or delayed recognition of clinical warning signs and oversights in management. The establishment of pregnancy heart teams facilitates multidisciplinary planning to improve management of people with cardiovascular disease. Core members of the pregnancy heart team include obstetricians, midwives, cardiologists, anesthesiologists, nurses, and patient care coordinators. Additional specialists and subspecialists are i…

National Institute for Health and Care Excellence guidelines on myocardial revascularisation.

Cardiologists in the UK predominantly use the National Institute for Health and Care Excellence (NICE) and European Society of Cardiology guidelines to help guide decision-making. This article will appraise the current recommendations from NICE regarding myocardial revascularisation and compare them with other major international guidelines. While there are many similarities, subtle differences exist. These differences arise in part due to the evidence base at time of publication, as well as from the different healthcare systems that they are designed for, and from the cost-effectiveness model…

An Update on Patent Ductus Arteriosus and What is Coming Next.

Patent ductus arteriosus is the most common cardiovascular condition in preterm infants. There is a significant uncertainty about when and how to close ductus arteriosus in preterm infants due to a high spontaneous closure rate even in very immature preterm infants. Diagnosis and management of patent ductus arteriosus remain a challenge for both neonatologists and pediatric cardiologists. Researchers have tried to define a balance between an expectant approach and active treatment in selected infants. This review aimed to focus on the pathophysiology and management of patent ductus arteriosus …

Large intracoronary thrombus and its management during primary PCI.

Large intracoronary thrombus has been reported in significant number of patients with STEMI. Primary PCI is the current standard of care in patients of STEMI. Despite the availability of dual antiplatelets, GP IIb/IIIa inhibitor and effective anticoagulation regimens, large intracoronary thrombus remains one of the biggest challenge to interventional cardiologists during primary PCI. Large intracoronary thrombus may lead to distal embolization, no/slow reflow or embolization into a non-culprit vessel and is associated with adverse cardiovascular outcome. There is no ideal management strategy. …

A care pathway for the cardiovascular complications of COVID-19: Insights from an institutional response.

The infection caused by severe acute respiratory syndrome coronavirus-2, or COVID-19, can result in myocardial injury, heart failure, and arrhythmias. In addition to the viral infection itself, investigational therapies for the infection can interact with the cardiovascular system. As cardiologists and cardiovascular service lines will be heavily involved in the care of patients with COVID-19, our division organized an approach to manage these complications, attempting to balance resource utilization and risk to personnel with optimal cardiovascular care. The model presented can provide a fram…

[A simple but very complicated angioplasty: management of coronary artery perforation and dual antiplatelet therapy. A case report and literature review].

Coronary artery perforation is a rare but potentially fatal complication. Therefore, it is crucial for interventional cardiologists to have knowledge of this condition and what management strategies are available, particularly in the case of procedures with a high level of complexity. Notwithstanding this, even simple procedures are not immune to serious complications, as described in this case report where coronary angioplasty was complicated by left anterior descending artery perforation with cardiac tamponade, giving rise to multiple complications of difficult management. The case presented…

Time does not heal every wound: coronary stent thrombosis of a bare-metal stent more than one decade after its implantation.

In the present report, we describe a unique case of very late stent thrombosis with a bare-metal stent that occurred more than a decade after stent implantation. Potential explanations for the late stent thrombosis are non-compliance to aspirin, late acquired malapposition of the stent, progression of atherosclerosis or in-stent restenosis. In our patient, none of these explanations seems to have played a role. Although the occurrence of (very) late stent thrombosis is not uncommon with drug-eluting stent (DES), it is rather unusual with bare-metal stent (BMS). Nevertheless, cardiologists shou…

Early Amplatzer occluder closure of a postinfarct ventricular septal defect as a bridge to surgical procedure.

The management of postinfarction ventricular septal defects represents a challenge to both cardiologists and surgeons due to the high morbidity and mortality rate. We report the case of a 79-year-old patient who developed an apical rupture of the ventricular septum, nine days after an anterior myocardial infarction. As the patient was in cardiogenic shock and developed acute pulmonary edema we chose to perform a percutaneous closure of the septal defect using an Amplatzer occluder (AO). Despite the incomplete closure, the placement of the device greatly improved the patient's clinical conditio…

[Cardiopulmonary exercise testing: the exercise stress test of the future?].

Scientific evidence is mounting that cardiopulmonary exercise testing (CPET) offers different potential clinical applications in cardiology. In comparison with traditional ECG stress testing, CPET provides more accurate diagnostic information and prognostic insights as well, in particular in patients with chronic heart failure and primary pulmonary hypertension. The barriers that actually limit the diffusion of CPET among cardiologists are primarily "cultural", because modern ergospirometric tools are easy to use, and the time required for a CPET is not dissimilar from that of a standard ECG s…

Patients with palpitations referred for 24-hour ECG recording.

Palpitations are one of the most common reasons for referral to cardiologists; most are assessed as being medically benign. Eighty consecutive subjects referred for 24-hour ECG recording completed a questionnaire at the time of assessment and a follow-up self-report questionnaire at 18 months. Many of those with and without abnormal hearts reported distress, concern about their symptoms, and limitation of everyday activities. There were few differences between these groups at assessment or at follow-up. There was little change in mental state or in disability in either group over 18 months. Pr…

Percutaneous transcatheter embolization of coronary arteriovenous fistulas.

Coronary artery fistulas are infrequently encountered vascular communications that are either congenital or due to cardiac trauma. Most patients with these anomalies are asymptomatic, but late complications can occur and include congestive heart failure, myocardial ischemia, arrhythmias, and endocarditis. Therefore, many investigators have recommended surgical repair, even for asymptomatic patients. Although coronary arteriovenous fistulas pose many challenges to interventional cardiologists, early experiences suggest that these abnormal vessels can be successfully obliterated percutaneously; …
