Beyond endovascular limits: open surgical management of bilateral giant petrocavernous internal carotid artery aneurysms in a pediatric patient.

Bilateral giant petrocavernous internal carotid artery (ICA) aneurysms are extremely rare, and its natural history, treatment response, and prognosis are unknown. Herein, we present the second case of a bilateral giant petrocavernous aneurysm treated with an external carotid artery-radial artery-M2 segment of the middle cerebral artery (ECA-RA-M2) bypass and ICA ligation. A 12-year-old female diagnosed with bilateral petrocavernous ICA aneurysms presented with headache, vomiting, and ptosis on the left eye. Endovascular treatment was not feasible due to financial limitations; hence, she underw

Chinese expert consensus on the treatment and diagnosis of moyamoya disease and moyamoya syndrome (2024).

AIM: Moyamoya disease (MMD) is a chronic, occlusive cerebrovascular disorder with no unified management guidelines. This consensus aims to provide updated, evidence-based recommendations for the diagnosis and treatment of MMD. METHODS: We integrated recent findings from epidemiology, genetics and imaging studies-particularly those published since 2017-along with expert input from over 20 clinical centres. Advances in diagnostic criteria, treatment strategies and perioperative management were reviewed and incorporated to update the 2017 consensus. RESULTS: Refinements in diagnosis include hig

Cerebral bypass for symptomatic vertebrobasilar dolichoectasia.

The standard therapeutic strategies for vertebrobasilar dolichoectasia (VBD) are controversial due to their complexity, high risks and great complications. The current study is conducted to describe the use of cerebral bypass to treat symptomatic VBD. This retrospective study from 2019 to 2024 involved 20 patients who underwent cerebral bypass to treat symptomatic VBD. Cerebral ischemia and compression were the main presentations of patients with VBD. Dissecting, partially thrombosed and incorporate important perforating arteries were the morphological characteristics of VBD. Among the entire

Spontaneous occlusion of ruptured microaneurysm formed on postoperative transosseous anastomosis after indirect revascularization in a patient with moyamoya disease.

A 40-year-old female with a history of ischemic moyamoya disease treated with indirect revascularization at ages 12 and 25 years presented with a sudden severe headache. Imaging studies revealed focal parenchymal hemorrhage and acute subdural hematoma, confirming a microaneurysm formed on the postoperative transosseous vascular network as the source of bleeding. Conservative management was performed, and no hemorrhage recurred during the 6-month follow-up period. Interestingly, follow-up imaging revealed spontaneous occlusion of the microaneurysm. However, due to the rarity of this presentatio

Validation of the Berlin Grading System for moyamoya angiopathy with the use of [15O]H2O PET.

The Berlin Grading System assesses clinical severity of moyamoya angiopathy (MMA) by combining MRI, DSA, and cerebrovascular reserve capacity (CVRC). Our aim was to validate this grading system using [15O]H2O PET for CVRC. We retrospectively identified bilateral MMA patients who underwent [15O]H2O PET examination and were treated surgically at our department. Each hemisphere was classified using the Suzuki and Berlin Grading System. Preoperative symptoms and perioperative ischemias were collected, and a logistic regression analysis was performed. A total of 100 hemispheres in 50 MMA patients (

The feasibility of performing multiple burr hole surgery in pediatric moyamoya patients as a response to failed mEDAS.

OBJECTIVE: To investigate the feasibility and clinical effectiveness of performing multiple burr hole surgery in pediatric moyamoya patients as a response to failed modified encephaloduroarteriosynangiosis (mEDAS). METHODS: From January 2014 to May 2018, multiple burr hole surgery (MBS) was conducted on 16 hemispheres in 12 patients as a secondary treatment following mEDAS. The male-to-female ratio was 1:2 and the average age at the time of mEDAS was 6 years old. The average patient age was 9 ± 3 years olds (range 7-17) at the time of MBS which occurred an average of 46 months after mEDAS. An

The First 24 h Hemodynamic Management in NICU after Revascularization Surgery in Moyamoya Disease.

OBJECTIVE: To evaluate whether hemodynamic factors are risk factors for prognosis in moyamoya disease (MMD). MATERIALS AND METHODS: The retrospective study reviewed a single-center MMD cohort in Huashan Hospital from August 2017 to January 2020. Stroke events in 30 days and follow-up modified Rankin Scale (mRS) grade were recorded. Systematic assessments with perioperative mean arterial pressure (MAP), red blood cell (RBC) parameters, and fluid management were also conducted. Logistic regressions were applied to evaluate the predictors of worse outcomes. Data was analyzed using SPSS 24.0. RE

Subclavian Artery to Internal Carotid Artery Interpositional Bypass for Carotid Artery Occlusion: Technical Case Report.

BACKGROUND AND IMPORTANCE: Conventional microsurgical treatment for symptomatic internal carotid artery (ICA) occlusion is revascularization with superficial temporal artery (STA) to middle cerebral artery bypass. However, in rare cases where the common carotid artery, external carotid artery (ECA), or both are also occluded, other microsurgical treatment options must be considered. CLINICAL PRESENTATION: We present the case of a 52-yr-old woman with common carotid artery occlusion and weak ICA flow from collateral connections between the vertebral artery, occipital artery, and ECA. She had i

Unexpected immediate postoperative resolution of long-standing neurological deficits following extracranial-intracranial bypass: a report of three cases.

While intracranial ischemic insults often result in neuronal death and permanent neurological deficits, some patients may develop potentially reversible neurological dysfunction from persistent hypoperfusion, as surviving neurons remain in an "idling" state. We report a unique series of three patients with long-standing neurological deficits who underwent EC-IC bypass for repeated TIAs and demonstrated unexpected, rapid resolution of long-standing pre-existing neurological deficits. We suggest that these individuals harbored regions of underperfused, idling neurons that responded rapidly to re

Exposure of the External Carotid Artery Through the Posterior Triangle of the Neck: A Novel Approach to Facilitate Bypass Procedures to the Posterior Cerebral Circulation.

BACKGROUND: The external carotid artery (ECA) is the main high-flow donor for extracranial-intracranial revascularization procedures. However, anatomic restraints limit the availability of ECA in posterior exposures of the craniocervical junction aimed for bypass to distal vertebral artery segments. OBJECTIVE: To examine the feasibility and safety of exposure of the ECA through the posterior triangle of the neck. METHODS: A preliminary feasibility study on the posterior neck exposure of the ECA was performed in 1 cadaveric head (2 sides) followed by a morphometric study on 9 cadaveric heads

Hyperperfusion Syndrome After Stenting for Intracranial Artery Stenosis.

Hyperperfusion syndrome (HPS) is a rare but potentially devastating postoperative complication developing after endarterectomy and carotid stenting. Limited information is available about this complication. The aim of this study was to assess the incidence of HPS and risk factors leading to its development. We retrospectively reviewed 178 consecutive cases of patients who underwent stenting of intracranial artery revascularization. We analyzed the association between HPS and patient's age, collateral vascular supply of the lesion, the interval between operation and the last occurrence of ische

Subclavian carotid bypass of occluded common and internal carotid arteries facilitated by an anomalous branch of the internal carotid artery.

Typically, internal carotid artery (ICA) occlusion precludes carotid endarterectomy. Extracranial branches of the ICA are uncommon. If this anomaly occurs in the setting of total proximal ICA occlusion then revascularization is feasible. We describe a unique example of such a case.

Facial and neck hematoma after carotid artery stenting: an uncommon misadventure in endovascular carotid revascularization.

The complication rates of carotid artery stenting (CAS) vary from 3.0% to 4.4%, and most commonly include ischemic stroke, intracranial hemorrhage, or groin complications. We present the rare complication of a patient who underwent CAS for a symptomatic 90% left internal carotid artery stenosis and developed an expanding cervical hematoma after the procedure with imminent respiratory compromise. After intubation, an arteriogram revealed perforation of the external carotid artery trunk, proximal to the origin of the internal maxillary artery. The artery was subsequently embolized and the hemato

Moyamoya disease and surgical intervention.

Stroke is an increasingly recognized cause of morbidity in the pediatric population. The incidence of ischemic stroke is 1.2 per 100,000 children. There are many known etiologies of childhood cerebral ischemia but moyamoya is one of the only ischemic conditions of childhood that can be effectively treated with surgery. Moyamoya disease is by definition idiopathic, whereas moyamoya syndrome refers to a similar disease course in conjunction with a known predisposing condition. The clinical manifestations and disease progression are similar. Furthermore, surgical treatment has been shown to be ef

'Moya' than meets the eye: neurofibromatosis type 1 associated with Moyamoya syndrome.

Moyamoya syndrome (MMS) is an uncommon association of neurofibromatosis type 1 (NF1). We describe a seven-year-old chinese girl with NF1 and unilateral MMS with multiple hyperintensities on T2-weighted magnetic resonance (MR) images. The ischaemic lesions in the ipsilateral white matter were hypointense on fluid attenuated inversion recovery (FLAIR) MR images, in contrast to the hyperintense "unidentified bright objects" (UBOs) of NF1. Neuroradiologists should be aware of associated MMS in NF1 patients, and distinguish the effects of ischaemia from UBOs, especially on FLAIR MR imaging.

Recanalization of a symptomatic extracranial internal carotid artery near occlusion with proximal and distal protection: technical case report.

OBJECTIVE: To describe a novel approach to recanalizing symptomatic extracranial internal carotid artery near occlusion using proximal and distal emboli protection devices. METHODS: Patients presenting with symptomatic extracranial internal carotid artery near occlusion who underwent endovascular recanalization between October 2004 and July 2005 were included in this study. During these procedures, a 9-French Concentric Balloon Guide Catheter (Concentric Medical, Mountain View, CA) was advanced into the common carotid artery proximal to the site of occlusion. Computer simulation of cerebral blood flow in moyamoya and the results of surgical therapies.

Moyamoya is the disease which involves the terminal portions of the internal carotid or origins of the middle or anterior cerebral arteries. The posterior communicating arteries are also involved, but not the vertebrals or the basilar artery. The disease occurs more commonly in females than males and it has two age peaks at less than 10 and 40 years. Over the years many treatment options or procedures have been advocated for this disease either with direct bypasses or indirect revascularization procedures or both in combination. Whether one procedure is better than another is a matter of quest
