Global assessment of patients--a bedside study. I: The influence of physical findings on the global assessment.

Three physicians selected ten basic physical findings for a study of diagnostic decision making in the global assessment of patients. The physicians independently examined 201 hospital in-patients for the presence of each of the findings and concluded their assessment with a global evaluation of whether the patient appeared ill. Each of the ten findings were used by at least one of the physicians in his overall assessment of the patient. All the observers considered 'looking older than age', 'trouble with breathing', 'cyanosis', and 'anaemia' important, but disagreed in their use of the remain

Shared decision-making in the care of individuals with diabetes.

People with diabetes often live with other chronic conditions and lead complicated lives. Determining what is the best management decision for a patient requires consideration of each individual's personal, social and biomedical context, what he or she values, the reasons he or she has to value the available options, and the relative contribution of each option in terms of benefits, harms, costs and inconveniences. Empathic conversations between patients and clinicians to diagnose the patient situation that necessitates action and the range of evidence-based actions that best address the situa

Risk stratification in Eisenmenger syndrome.

Pulmonary arterial hypertension complicating congenital heart disease can progress to the life-threatening irreversible form known as Eisenmenger syndrome. When conservative treatments are inadequate, the risk of death as a result of the disease must be weighed against the risk associated with transplantation. Risk stratification has become a fundamental tool for the prediction of outcomes and the guidance of treatment in pulmonary arterial hypertension. However, the current risk scores for pulmonary arterial hypertension are not specific to pulmonary arterial hypertension with congenital hear

Pediatric Surgical Risk Assessment Tools: A Systematic Review.

BACKGROUND: Pediatric surgical risk assessment tools use patient- and procedure-specific variables to predict postoperative complications. These tools assist clinicians in preoperative counseling and surgical decision-making. The objective of this systematic literature review was to compile and compare existing pediatric surgical risk tools that are broadly applicable across pediatric surgical specialties. METHODS: A systematic literature review was performed following the Preferred Reporting Items for Systematic Reviews (PRISMA) guidelines. Relevant publications were identified and screened

Payer decision-making with limited comparative and cost effectiveness data: the case of new pharmacological treatments for gout.

CONTEXT: The need for comparative effectiveness (CE) data continues to grow, fuelled by market demand as well as health reform. There may be an assumption that new drugs result in improved efficacy compared with the standard of care, therefore warranting premium prices. Gout treatment has recently become controversial, as expensive new drugs enter the market with limited CE data. METHODS: The authors reviewed published clinical trials and conducted a cost effectiveness analysis on a new drug (febuxostat) versus the standard (allopurinol) to illustrate the limitations in using these data to in

Assessment of the interobserver reliability in discharge decision-making for patients undergoing thoracic surgery based on chest ultrasound compared to chest X-ray findings.

BACKGROUND: Safe authorization of patient discharge after thoracic surgery must be ensured following chest tube removal. This decision is commonly based on chest X-ray findings. Chest ultrasound (US) has emerged as a potentially valuable alternative. The aim of this study was to assess the interobserver reliability of discharge authorization decisions based on chest X-ray compared with chest US findings. METHODS: We conducted a single-center prospective study including 60 consecutive patients undergoing thoracic surgery. A semi-quantitative evaluation of pneumothorax, pleural effusion, subcut

To What Degree Do Patients' and Clinicians' Ratings of Appropriateness of TKA Align, and Were Expected Outcomes Associated With Those Ratings?

BACKGROUND: TKA is performed only after the patient and the surgeon agree that the potential benefits of surgery outweigh potential risks. However, as many as 20% of patients are dissatisfied after TKA, suggesting that the shared decision-making process does not consistently serve our patients well. Expected outcomes are increasingly used in clinical practice, as an addendum to preoperative clinical and demographic factors, to inform this decision; however, the added value of this information in aligning patients and surgeons regarding the benefit and risk of TKA remains poorly understood. The Influence of Hospital Policies on Clinicians' Decisions to Withhold or Withdraw Life-Sustaining Treatment.

BACKGROUND: There is considerable variation in clinicians' approaches to decisions to withhold or withdraw life-sustaining treatment (LST) across US hospitals. These differences are not explained by patient preferences alone and are likely influenced by other factors (eg, hospital policies, hospital culture, state laws, medical society guidelines). RESEARCH QUESTION: How do hospital policies influence clinician approaches to decisions to withhold or withdraw LST among patients admitted to an ICU? STUDY DESIGN AND METHODS: We conducted semistructured interviews with ICU nurses and physicians

Weighing the benefits and risks of oral immunotherapy in clinical practice.

Food allergies are common and affect 6-8% of children in the United States; they pose a significant burden on the quality of life of children with allergy and their caregivers due to multiple daily restrictions. Despite the recommended dietary avoidance, reactions tend to occur due to unintentional exposure to the allergenic food trigger. Fear of accidental ingestions with potentially severe reactions, including anaphylaxis and death, creates anxiety in individuals with food allergy. Oral immunotherapy has emerged as a form of active and potentially disease-modifying treatment for common food

Decoding pan-cancer treatment outcomes using multimodal real-world data and explainable artificial intelligence.

Despite advances in precision oncology, clinical decision-making still relies on limited variables and expert knowledge. To address this limitation, we combined multimodal real-world data and explainable artificial intelligence (xAI) to introduce AI-derived (AID) markers for clinical decision support. We used xAI to decode the outcome of 15,726 patients across 38 solid cancer entities based on 350 markers, including clinical records, image-derived body compositions, and mutational tumor profiles. xAI determined the prognostic contribution of each clinical marker at the patient level and identi…

Indirect signs of aortic dissection on POC-TTE despite an ADD-RS of 0 and D-dimer < 500 ng/mL.

Aortic dissection (AD) is a "can't miss" diagnosis for emergency physicians. An algorithm combining the Aortic Dissection Detection Risk Score (ADD-RS) with D-dimer has been proposed as a high-sensitivity clinical decision tool for AD that can determine the need for advanced imaging. Here we present a case of a 48-year-old male who presented to the emergency department (ED) with chest pain and dyspnea. He had an ADD-RS score of 0 and negative D-dimer, which placed him in the low-risk category not requiring further advanced imaging. Despite this, he was found to have a pericardial effusion and

Stratification of coronary artery disease patients for revascularization procedure based on estimating adverse effects.

BACKGROUND: Percutaneous coronary intervention (PCI) is the most commonly performed treatment for coronary atherosclerosis. It is associated with a higher incidence of repeat revascularization procedures compared to coronary artery bypass grafting surgery. Recent results indicate that PCI is only cost-effective for a subset of patients. Estimating risks of treatment options would be an effort toward personalized treatment strategy for coronary atherosclerosis. METHODS: In this paper, we propose to model clinical knowledge about the treatment of coronary atherosclerosis to identify patient-sub

Management of Cardiovascular Disease Patients With Confirmed or Suspected COVID-19 in Limited Resource Settings.

In this paper, we provide recommendations on the management of cardiovascular disease (CVD) among patients with confirmed or suspected coronavirus disease (COVID-19) to facilitate the decision making of healthcare professionals in low resource settings. The emergence of novel coronavirus disease, also known as Severe Acute Respiratory Syndrome-Coronavirus-2 (SARS-CoV-2), has presented an unprecedented global challenge for the healthcare community. The ability of SARS-CoV-2 to get transmitted during the asymptomatic phase and its high infectivity have led to the rapid transmission of COVID-19 b

Practical Recommendations for the Diagnosis and Medical Management of Stable Angina: An Expert Panel Consensus.

Stable angina affects a significant number of coronary artery disease patients, impairing their quality of life and worsening their prognosis. It manifests even despite a history of revascularization and is often poorly controlled with drug therapy. Comorbid conditions are frequently encountered in coronary artery disease patients, affecting their prognosis and rendering the diagnosis and management of angina more challenging. In this article, derived by an expert panel meeting, we attempt a practical approach to stable angina, focusing on symptomatic patients subjected to previous coronary re

The dynamics of deceased donor kidney transplant decision making: insights from studying individual clinicians' offer decisions.

Despite the high demand, >7500 recovered kidneys annually go unused, with transplant centers showing significant variation in their offer acceptance practices. However, it remains unclear how much of this variation occurs between individual clinicians within the same center and its impact on allocation efficiency and equity. This study quantified the variability in kidney offer acceptance decisions attributable to clinicians vs centers and examined the role of donor quality in acceptance decisions. We analyzed national transplant registry data (from January 2016 to December 2020) linked to on-

Clinical challenges and individualized approaches to dialysis therapy in older adults.

Global population aging has substantially increased in the number of older adults who undergo kidney replacement therapy (KRT). Age-related physiological changes and functional dependence in this population lead to the multifaceted clinical and ethical challenges associated with KRT. Geriatric syndromes, including functional impairment, frailty, malnutrition, and multimorbidity, can influence the choice of dialysis modality and modify dialysis prescriptions, often resulting in patients requiring assistance with dialysis implementation. Although dialysis remains a key life-sustaining therapy, t

Primary radical surgery or preoperative drainage? A clinical decision framework for pediatric choledochal cysts.

BACKGROUND: For children with complicated choledochal cyst (CDC), the decision between primary radical surgery (PRS) and preoperative drainage (PD) remains subjective. We aimed to develop an objective preoperative risk framework to inform this choice. METHODS: A retrospective, single-center review of 454 children with CDCs was conducted. The primary outcome was disease-related severe complications (Clavien-Dindo ≥ II). A prediction model for severe complications was built using logistic regression. A sensitivity analysis used a composite outcome that also included extreme surgical difficulty
