Comparison of an Implantable Middle Ear Microphone and Conventional External Microphone for Cochlear Implants: A Clinical Feasibility Study.

OBJECTIVES: All commercially available cochlear implant (CI) systems use an external microphone and sound processor; however, external equipment carries lifestyle limitations. Although totally implantable devices using subcutaneous microphones have been developed, these are compromised by problems with soft tissue sound attenuation, feedback, and intrusive body noise. This in vivo pilot study evaluates a middle ear microphone (MEM) that aims to overcome these issues and compares hearing performance with that of an external CI microphone. DESIGN: Six adult participants with an existing CI were

Side-of-Implantation Effect on Functional Asymmetry in the Auditory Cortex of Single-Sided Deaf Cochlear-Implant Users.

Cochlear implants (CIs) allow to restore the hearing function in profoundly deaf individuals. Due to the degradation of the stimulus by CI signal processing, implanted individuals with single-sided deafness (SSD) have the specific challenge that the input highly differs between their ears. The present study compared normal-hearing (NH) listeners (N = 10) and left- and right-ear implanted SSD CI users (N = 10 left, N = 9 right), to evaluate cortical speech processing between CI- and NH-ears and to explore for side-of-implantation effects. The participants performed a two-deviant oddball task, s

Pre-linguistic social communication skills and post implant language outcomes in deaf children with cochlear implants.

PURPOSE: This study investigates the relationship between pre-linguistic social communication skills and age of cochlear implant for future … ficant pre-implant correlates of language outcomes. CONCLUSIONS: Long term language outcomes after cochlear implantation are the product of a set of communicative, cognitive and environmental factors. Early pre-implant social communication skills are an important consideration for clinicians who guide parents as to likely long-term outcomes post cochlear implantation. …

Benefits of Binaural Integration in Cochlear Implant Patients with Single-Sided Deafness and Residual Hearing in the Implanted Ear.

The purpose of the study is to gauge the benefits of binaural integration effects (redundancy and squelch) due to preserved low-frequency re … zation error showed significance only for squelch (r = -0.67; p = 0.02). Hearing preservation after cochlear implantation has considerable benefits because the preserved low-frequency hearing in the implanted ear contributes to binaural integration, presumably through the preserved temporal fine structure.

A Fully-Implantable Cochlear Implant SoC with Piezoelectric Middle-Ear Sensor and Arbitrary Waveform Neural Stimulation.

A system-on-chip for an invisible, fully-implantable cochlear implant is presented. Implantable acoustic sensing is achieved by interfacing the SoC to a piezoelectric sensor that detects the sound-induced motion of the middle ear. Measurements from human cadaveric ears demonstrate that the sensor can detect sounds between 40 and 90 dB SPL over the speech bandwidth. A highly-reconfigurable digital sound processor enables system power scalability by reconfiguring the number of channels, and provides programmable features to enable a patient-specific fit. A mixed-signal arbitrary waveform neural

A safety evaluation of dexamethasone-releasing cochlear implants: comparative study on the risk of otogenic meningitis after implantation.

CONCLUSION: Dexamethasone released from a cochlear implant seems not to enhance the risk for postoperative infections. OBJECTIVE: Dexamethasone has a positive impact on hearing preservation for electric acoustic stimulation (EAS). Due to their antiproliferative and immunosuppressive properties, steroids may enhance the risk of postoperative infections. A comparative study was performed to evaluate the risk of pneumococcal meningitis after implantation of dexamethasone-eluting cochlear implants. METHODS: Thirty guinea pigs were implanted with non-eluting (n = 15) or dexamethasone-eluting (n =

Multi-component and multi-frequency tympanometry: a comparison between cochlear implanted ear and non-implanted ear.

OBJECTIVES: Cochlear implant (CI) surgery may alter middle ear structure and function, potentially affecting the measurement of the electrically evoked stapedius reflex threshold (eSRT). This study aimed to investigate such alterations using multi-component and multi-frequency tympanometry. Specific objectives were to compare tympanometric findings, multi-component tympanogram patterns, and frequency-dependent changes in admittance (ΔY), susceptance (ΔB), conductance (ΔG), and phase angle (Δθ) between implanted and contralateral non-implanted ears. METHODS: Twenty-two paediatric participants

Molded Round Window Niche Implant as a Dexamethasone Delivery System in a Cochlear Implant-Trauma Animal Model.

BACKGROUND: Preserving residual hearing after cochlear implant (CI) surgery remains a crucial challenge. The application of dexamethasone (DEX) has been proven to positively affect residual hearing. To deliver DEX in a localized and controlled way, a round window niche implant (RNI), allowing drug diffusion via the round window membrane into the cochlea, may be used. To prove this concept, an RNI for guinea pigs as a CI-trauma model was manufactured by molding and tested for its drug release in vitro and biological effects in vivo. METHODS: The RNIs were molded using silicone containing 10% D

Successful implantation of a cochlear implant in a four-yr-old boy after kidney transplantation: a case report.

Sensorineural hearing loss is common in children with chronic renal insufficiency. The implantation of a CI is performed routinely in children with profound sensorineural hearing loss. A feared complication is a local infection with subsequent meningitis. Because of this risk, a successful implantation of a CI in children under immunosuppression after kidney transplantation has yet to be described. A four-yr-old boy with congenital renal dysplasia and posterior urethral valves, who was successfully transplanted with a kidney from his father at the age of two and a half yr, is presented. The bo

Explantation of a nucleus multichannel cochlear implant and re-implantation into the contralateral ear. A case report of a new strategy.

We present a unique case in which a multichannel cochlear implant device was explanted and the same device was re-implanted into the contralateral ear. A patient with bilateral total deafness secondary to head injury received an implant in his left ear but developed severe facial nerve stimulation. Because this stimulation could not be eliminated effectively with change of programming, it was decided to implant the contralateral ear. Since the device itself was functioning well, it was explanted from the left ear and re-implanted successfully into the right ear. Facial nerve stimulation was mi

Multichannel optical cochlear implants enable spectrally distinct auditory activity.

When hearing fails, cochlear implants (CIs) restore auditory perception. Yet, coding of spectral information remains a bottleneck because each electrode broadly activates the auditory nerve. As light can be more conveniently confined in space, optical (o)CIs present an alternative. Here, we combined expression of the potent channelrhodopsin ChReef in spiral ganglion neurons (SGNs) with oCIs comprising 5-10 green LEDs in gerbils. This combination enabled systematic comparison of encoding intensity and spectral information by individual oCI channels to acoustic and electrical stimulation within

Electrophysiological Assessment of Semantic Processing of Cochlear Implant Users Using an Audiobook.

Cochlear implants (CI) afford listeners with limited spectral resolution, thereby constraining semantic processing. Prior research has predominantly used isolated words or sentences without context, a method that lacks ecological validity and may place greater demands on working memory (WM) due to the absence of contextual linguistic information. This study examined the semantic processing of experienced CI users (N = 21) and age-matched normal-hearing controls (NH, N = 21) using an audiobook. We measured neuroelectric brain activity while participants listened to a story in which we inserted
