Discovery of Novel Pyrazole-4-Carboxamide and Aromatic Carboxamide Derivatives Containing a Trans-2-(Trifluoromethyl)cyclopropyl Moiety as Potential SDHIs.

A series of racemic pyrazole-4-carboxamide and aromatic carboxamide derivatives bearing a trans-2-(trifluoromethyl)cyclopropyl moiety were designed and synthesized as potential succinate dehydrogenase inhibitors (SDHIs). The compounds were evaluated for their in vitro and in vivo antifungal activities. Compound 10h showed potent activity against Rhizoctonia solani and Pyricularia oryzae, with EC50 values of 0.04 and 1.59 mg/L, respectively. At 5 mg/L, 10h provided 77.8% control of rice sheath blight and 88.9% control of soybean rust, outperforming fluxapyroxad and boscalid. It inhibited succin

Risk of Major Adverse Cardiovascular Events With Dolutegravir Versus Efavirenz-Based Antiretroviral Therapy: Emulated Target Trials Using Routine, De-identified Data From South Africa.

BACKGROUND: Integrase inhibitors, including dolutegravir, may increase risk of major adverse cardiovascular events (MACEs). However, limited data exists from low- and middle-income countries, where tenofovir disoproxil fumarate, lamivudine, and dolutegravir (TLD) has largely replaced tenofovir disoproxil fumarate, emtricitabine, and efavirenz (TEE). METHODS: We used de-identified data from a South African managed healthcare organization from people with HIV (PWH) without cardiovascular disease, who either initiated TEE or TLD between April 2020 and Dec 2023 (initiation cohort) or were receivi

Efavirenz 400 vs 600 mg combined with lamivudine and tenofovir in treatment-naïve HIV-infected patients in China: A randomized, multi-centered, controlled trial.

BACKGROUND: Optimizing efavirenz (EFV) dosing to minimize adverse events (AEs) remains a critical issue. Studies indicate that reducing the EFV dose from 600 to 400 mg daily lowers side effect intensity while maintaining virological suppression. We previously showed that 600 mg daily EFV may lead to unnecessarily high exposure in Chinese patients, but the long-term efficacy and safety of dose reduction beyond 48 weeks in this population remain unknown. METHODS: We conducted a multicenter, randomized controlled trial comparing EFV 400 vs 600 mg, each combined with lamivudine (3TC) and tenofovi

Compare antiretroviral drug concentrations in hair and plasma across EFV-based regimens in China.

Effective antiretroviral therapy (ART) depends on adequate drug exposure. Plasma ART concentrations provide a short-term assessment of drug exposure, and hair promises to be an alternative matrix for measuring long-term exposure. We aimed to determine the association between plasma and hair ART concentrations and explore the therapeutic concentrations in hair. A cohort study in which HIV-infected adults receiving tenofovir disoproxil fumarate (TDF) + lamivudine (3TC) + efavirenz (EFV) regimen for at 6 months were recruited and paired hair and plasma samples collected at about 6±1 months of ART

Anti-Leukotriene Receptor Blockers Improve Tendon-Bone Interface Healing in a Rat Model of Acute Rotator Cuff Tear.

BACKGROUND: Excessive expression of proinflammatory cytokines after rotator cuff (RC) surgery impairs the quality of tendon-bone interface (TBI) healing. There is evidence that the asthma drug montelukast (MS) inhibits the expression of proinflammatory cytokines. This study was conducted to verify the effect of MS administration on TBI healing after RC repair. MATERIALS AND METHODS: Thirteen rats in the MS group were intraperitoneally administered 10 mg/kg of the drug daily for 2 weeks after RC surgery, and 13 rats in the control group were administered only 0.9% saline. The healing effect of

Electrospun polycaprolactone fibers encapsulating omega-3 and montelukast sodium to prevent capsular contracture in breast implant surgery.

Capsular contracture (CC) is a common complication associated with breast implant surgery and is characterized by excessive fibrotic tissue formation around the implant. However, there is no established gold-standard treatment to prevent CC. This study aimed to prepare fish oil/montelukast sodium (MTKS)-loaded polycaprolactone (PCL) fibers and evaluate their effectiveness in preventing CC. PCL, a biocompatible and biodegradable material, was used to fabricate electrospun fibers incorporating fish oil, a source of omega-3 (ω3) polyunsaturated fatty acids (EPA and DHA), and MTKS, a leukotriene r

Ciprofol attenuates sepsis-induced cardiomyopathy via α7 nicotinic acetylcholine receptor-dependent modulation of myocardial inflammation and NF-κB/STAT3 signaling.

Sepsis-Induced Cardiomyopathy (SIC) is a life-threatening complication with limited therapies, significantly contributing to septic mortality. This study investigated the cardioprotective effects of ciprofol, a novel anesthetic, in SIC and elucidated the role of the α7 nicotinic acetylcholine receptor (α7nAChR) pathway. Using a cecal ligation and puncture (CLP)-induced murine sepsis model, we assessed ciprofol pretreatment's impact on myocardial injury, cardiac function, survival, and inflammation. The α7nAChR antagonist methyllycaconitine (MLA) was used to investigate pathway involvement. Our

Vanzacaftor-tezacaftor-deutivacaftor for children aged 6-11 years with cystic fibrosis (RIDGELINE Trial VX21-121-105): an analysis from a single-arm, phase 3 trial.

BACKGROUND: In phase 2 trials in people with cystic fibrosis aged 18 years and older, vanzacaftor-tezacaftor-deutivacaftor has been shown to be a safe and effective, once-daily cystic fibrosis transmembrane conductance regulator (CFTR) modulator. Restoring normal CFTR function early in life has the potential to prevent manifestations of cystic fibrosis. We aimed to evaluate the safety, tolerability, efficacy, and pharmacokinetics of vanzacaftor-tezacaftor-deutivacaftor in children with cystic fibrosis aged 6-11 years. METHODS: In this multicentre, single-arm, phase 3 trial (RIDGELINE Trial VX

Pharmacokinetics of rifampicin and isoniazid in patients with HIV-tuberculosis coinfection receiving efavirenz-based antiretroviral treatment: an ANRS12292-RIFAVIRENZ sub-study.

BACKGROUND: Increasing rifampicin dosing is considered a potent strategy for shortening TB treatment duration. Although previous data among patients with HIV-TB coinfection has shown that doubling rifampicin dosing had a small effect on EFV concentrations, its effect on the pharmacokinetics (PK) of antituberculosis drugs remains lacking in this population. OBJECTIVES: To compare the PK of rifampicin and isoniazid with and without EFV co-administration in patients with HIV-TB coinfection using two rifampicin dosing regimens (10 and 20 mg/kg/day) and EFV dosing (600 and 800 mg q24h). Rationale design and synthesis of new roflumilast analogues as preferential selective and potent PDE-4B inhibitors.

In this study, we designed and synthesized novel analogues of roflumilast that exhibit selective inhibition of PDE-4B. To accomplish this target; synthesis of novel series (4a-u, 5a-i, and 6) was done, aiming at obtaining new PDE-4B inhibitors hits based on the proposed pharmacophore, 1-(cyclopropylmethoxy)-2-(difluoromethoxy) benzene moiety. Enzyme assay was used to measure the IC50 values for the PDE-4B inhibition of all the synthesized compounds along with roflumilast as a reference drug. The results demonstrated that most of the examined candidates exhibited considerable inhibitory activit

A Drug Repurposing Approach to Identify Therapeutics by Screening Pathogen Box Exploiting SARS-CoV-2 Main Protease.

Coronavirus disease-19 (COVID-19) is caused by severe acute respiratory syndrome coronavirus -2 (SARS-CoV-2) and is responsible for a higher degree of morbidity and mortality worldwide. There is a smaller number of approved therapeutics available to target the SARS-CoV-2 virus, and the virus is evolving at a fast pace. So, there is a continuous need for new therapeutics to combat COVID-19. The main protease (Mpro ) enzyme of SARS-CoV-2 is essential for replication and transcription of the viral genome, thus could be a potent target for the treatment of COVID-19. In the present study, we perfor
