Lumbosacral epidural lipomatosis causing rapid onset cauda equina syndrome.

Spinal epidural lipomatosis (SEL) is a rare cause of cauda equina syndrome (CES), which must be diagnosed with MRI in conjunction with a high level of clinical suspicion. Most reported cases are associated with obesity, steroid use or are secondary to endocrinopathies, frequently present subacutely or chronically, and have been managed with both surgical decompression and non-operative measures. We describe an obese 55-year-old man with rapid onset CES secondary to idiopathic lumbosacral SEL which was managed successfully with surgical decompression. Although often thought to be a trivial radi

Ulnar nerve entrapment neuropathy at the elbow: simple decompression.

Ulnar nerve entrapment neuropathy at the elbow, or the cubital tunnel syndrome, is frequently encountered in neurosurgical practice as the second most common peripheral nerve entrapment after carpal tunnel syndrome. Patients typically present with weakness or atrophy of the hand as well as paresthesias in the ulnar nerve distribution. The diagnosis can be confirmed with a careful clinical examination and electrophysiological studies. Patients who have failed conservative therapy are considered for surgery. Although a number of surgical options are available, simple decompression of the ulnar n

Progressive coma after the transsphenoidal decompression of a pituitary adenoma with marked suprasellar extension: report of two cases.

Two cases of neurological deterioration and coma after the transsphenoidal decompression of a pituitary adenoma with marked suprasellar extension and invasion of the 3rd ventricle are presented. Emergency ventricular shunting led to prompt neurological improvement, which, supplemented by radiation therapy, allowed long-term amelioration of symptoms. Three possible explanations for this complication are offered: 1) traction of the attached 3rd ventricle into the decompression site, causing increased obstructive hydrocephalus, 2) vasopressin release by surgical manipulation of the pituitary stal

[Surgical treatment of facial palsy].

The results of surgical treatment of 46 cases of facial nerve paralysis from 1974 to 1987 were reported. Among them, facial nerve decompression comprised 36 cases and facial nerve grafting 10 cases. The causes of facial nerve paralysis were head trauma in 13, surgical injury in 22, otitis media in 4, facial neurofibroma in 2 and Bell's palsy in 5. Thirty-six cases were followed up from 0.5 to 6.5 years. Twenty-five cases (69.5%) had totally recovered, 7 cases (19.4%) partially recovered and 4 cases (11.1%) had no recovered. The result of facial nerve decompression was better than that of facia

Volkmann's ischemia.

The posttraumatic syndrome of Volkmann's ischemia and infarction is the end result of compromise of muscle perfusion within the osteofascial compartments of the forearm. The "closed space" arrangement as well as the particular neurovascular anatomy make the flexor forearm muscles particularly susceptible to the cyclic derangement of blood flow, which ultimately results in muscle ischemia and infarction. Only strong clinical awareness will allow appropriate early diagnosis and treatment by surgical decompression. Fasciotomy and epimysiotomy serve to interrupt the cycle. The successful treatment

Quantitative evaluation of bone remodelling after decompression of maxillofacial cysts using fractal analysis: A prospective cohort study.

INTRODUCTION: Maxillofacial cysts may cause functional and aesthetic impairment if inadequately managed. Decompression is a conservative approach that reduces surgical morbidity while promoting bone regeneration. This study evaluated the effectiveness of decompression in managing intrabony maxillofacial cysts using fractal analysis and clinical outcomes. MATERIAL AND METHODS: A prospective cohort study was conducted at a tertiary care hospital, including 28 patients aged 7-60 years with intrabony maxillofacial cysts requiring decompression. The primary outcome was the change in fractal dimens

Current opinions on the management of prolonged ischemic priapism: does penoscrotal decompression outperform corporoglanular tunneling?

Prolonged ischemic priapism presents a treatment challenge given the difficulty in achieving detumescence and effects on sexual function. To evaluate current practice patterns, an open, web-based multi-institutional survey querying surgeons' experience with and perceived efficacy of tunneling maneuvers (corporoglanular tunneling and penoscrotal decompression), as well as impressions of erectile recovery, was administered to members of societies specializing in male genital surgery. Following distribution, 141 responses were received. Tunneling procedures were the favored first-line surgical in

Spontaneous hydrocele resolution after hyperbaric oxygen treatment: a clinical case report.

Hyperbaric oxygen is considered an adjunctive treatment to medical and surgical care. We present a unique case in which a male patient with decompression illness affecting inner ear and spinal cord presented a worsened unilateral hydrocele synchronously with the neurological pathology. At the Diving and Hyperbaric Medicine Department, the patient was initially recompressed using a modified United States Navy Treatment Table 6A; on the following days he was treated for decompression illness using hyperbaric oxygen. Hyperbaric oxygen treatment has not been used for the treatment of hydrocele, bu

Controversy: endoscopic approaches to metastatic thoracic disease.

Traditional approaches to thoracic metastases and spinal cord compression have been well worked out and validated in the literature. Anterior decompression is clearly superior to laminectomy; vertebrectomy and reconstruction are indicated for sagittal collapse, instability, and pain; and surgical decompression is necessary in cases of bony impingement. The role of endoscopic and minimally invasive techniques in treatment of metastatic disease is evolving. Dr. Lieberman advocates the use of thoracoscopic anterior approaches as the principal application in these patients, whereas Dr. McLain has

Safe Decompression of the Right Ventricle for PAIVS in Neonates With Coronary Fistulae: Including the Selective Use of Fistula Ligation to Avoid Coronary Steal.

BACKGROUND: There are a number of surgical and interventional treatment options for infants with pulmonary atresia with intact ventricular septum (PAIVS). In our practice, we characterize coronary fistulae and interruptions with angiography in the newborn and have developed a strategy to safely decompress the right ventricle in association with ligation of fistulae if necessary. METHODS: All infants operated for PAIVS at age < 60 days from 1999 to 2018 were retrospectively studied. Pre- and postoperative variables were collected, angiograms were reviewed, and a territory score was created to

Bilateral interlaminar fenestration and unroofing for the decompression of nerve roots by using a unilateral approach in lumbar canal stenosis.

BACKGROUND: Conventional laminectomy is a simple procedure that provides adequate decompression in cases of lumbar spinal canal stenosis. However, with this surgical modality, important posterior osteoligamentous elements necessary for spinal stability are lost; and it is often accompanied by damage to the facet joint. Bilateral interlaminar fenestration and unroofing of the intervertebral foramen to achieve decompression of the nerve roots by using a unilateral microsurgical approach in lumbar spinal canal stenosis are less invasive compared with conventional laminectomy. METHODS: We carried

Full-Endoscopic Extraforaminal Decompression for Far-Out Syndrome Due to Lumbosacral Transitional Vertebra: A Technical Note.

CASE: A 44-year-old woman presented with radiculopathy due to pinching of the L5 nerve between enlarged transverse process (TP) and sacral ala in lumbosacral transitional vertebra (LSTV) (far-out syndrome). On failure of conservative treatment, the patient underwent posterolateral full-endoscopic extraforaminal decompression under local anesthesia. Under endoscopic visualization, the enlarged TP was seen stenosing the extraforaminal area. A high-speed diamond burr was used to drill the TP. Following bony decompression, the perineural soft tissue in the extraforaminal region was dissected to de

Extensive spinal epidural abscess evacuated with unilateral interlaminar approach (with operative video): illustrative case.

BACKGROUND: The authors present the case of a patient with an extensive lumbosacral spinal epidural abscess (SEA) that was successfully evacuated in its entirety with a unilateral interlaminar approach. This is an alternative to wide bony decompression at multiple segments. The surgical approach described decompresses the neural elements while maintaining spinal integrity. OBSERVATIONS: Partial removal of the ligamentum flavum can provide an interlaminar corridor that is sufficient to evacuate extensive SEAs in select patients. LESSONS: This case illustrates a surgical approach that successf
