To document the persistence of perioral dermatitis at dermatology clinics at University Hospital, Saskatoon, we reviewed the charts of all patients with the condition seen between January 1983 and March 1985. Patients with rosacea referred to the clinics during the same period were used as a comparison group. During a 2-year period in Toronto, Canada, 21 printing tradesmen with contact dermatitis were evaluated. 67% had allergic contact dermatitis; 29% due to ultraviolet-cured ink components. Irritant contact dermatitis accounted for 37% of the cases. A case of hand contact dermatitis from Loctite RC/601, 242, and 504 is reported in a 60-year-old machine assembler. Thirteen adults with dermatitis herpetiformis (DH) controlled by dapsone or sulphamethoxypyridazine were given indomethacin or placebo in a double-blind cross-over study. In nine of the thirteen patients the rash and pruritus of DH were exacerbated more by indomethacin than by placebo. Dapsone and sulphamethoxypyridazine requirements were increased during the indomethacin period. The involvement of lipoxygenase products in the pathogenesis of the DH skin lesions is postulated.

We report a case of a distinctive dermatitis in a dairy worker exposed to Brucella abortus while manually delivering an infected calf. The rapid onset of pruritus, the strict limitation of lesions to the upper extremities, and the negative results on growth of culture for microorganisms make direct bacterial infection an untenable cause in this case. Allergic hypersensitivity is the probable mechanism, manifesting itself initially as contact urticaria. Coincidentally our patient also had systemic brucellosis, the symptoms of which antedated and were unrelated to his cutaneous disease.

The occurrence of dermatologic problems caused by consumer exposure to dyes on clothing is reviewed. Thirty-one dyes, mainly disperse with anthraquinone or azo structures, have caused allergic contact dermatitis. Phototoxic dye dermatitis is rare.

In a study of almost 1000 patients with past or present atopic dermatitis (AD) it was found that histories of recurrent (greater than 5 episodes/year) cold sores and upper respiratory infections, as well as histories of zoster were significantly more common in AD patients than in non-atopic controls. A patient with severe atopic dermatitis had pre-Sézary and Sézary syndromes. Although elevated serum IgE values in mycosis fungoides and Sézary syndrome are not infrequent, the direct sequential occurrence of atopic dermatitis and Sézary syndrome is uncommon. The clinical and laboratory changes we observed imply that the relationship of these disorders may be conditioned by changes in immunoresponsiveness.

We describe a patient with clinical features of dermatitis herpetiformis (DH) and histopathologic features suggestive of both DH and bullous pemphigoid (BP). Allergic contact dermatitis accounts for up to 20% of all dermatitis in childhood. The major sources of contact allergy in childhood are metals, shoes, preservatives, and plants. There is considerable variability from region to region and country to country in the prevalence and causes of contact dermatitis in childhood. We suggest that patch testing be done more frequently in evaluating dermatitis in children. Topical or systemic glucocorticosteroids and allergen avoidance are the mainstays of therapy in childhood.

Atopic dermatitis is a common skin disease associated with other allergic diatheses. Ocular complications were seen in 85 (42.5%) of 200 patients. Blepharoconjunctivitis, cataracts, corneal disease and ocular herpes simplex were frequent. Cataract surgery was usually successful, but there were complications of such surgery. The rate of success in attempts to wear contact lenses was surprisingly high for both phakic and aphakic patients.

Atopic dermatitis is a common disease of childhood that remits with age. Its pathogenesis is still uncertain; histamine seems important but the role of IgE is uncertain. The condition presents with several clinical features, none of which alone is pathognomonic. A 6 year old girl is reported who suffered from dermatitis herpetiformis, nephrotic syndrome and celiac disease. HLA typing in our patient disclosed the HLA antigens B8 and DW3, which are known to be frequently associated with nephrotic syndrome, celiac disease and dermatitis herpetiformis. In the literature six cases have been reported of the association of glomerulonephritis and dermatitis herpetiformis. An explanation for the development of different immunological diseases in one patient is offered by the HLA type.
