Impulse control disorders in patients with pituitary adenoma managed with or without dopamine agonists: a cross-sectional study from a UK centre.

PURPOSE: Studies from various countries raise concerns on the association between dopamine agonist (DA) treatment and impulse control disorders (ICDs) in patients with pituitary adenomas. We investigated the prevalence of ICDs in patients with pituitary adenomas in a UK centre using two instruments applied in clinical practice for assessing ICDs. METHODS: Cross-sectional study of adults with prolactinoma, acromegaly or non-functioning pituitary adenoma (NFPA) treated or not with DAs in a pituitary centre. Screening tools for ICD were the Minnesota Impulse Disorders Interview (MIDI) and Questi

[Stealing as an Impulse Control Disorder Associated with Pramipexole - A Case Report from Forensic Psychiatric Practice].

We report the case of a 48-year-old female whom we examined for legal culpability. The proband was taking the dopamine agonist pramipexole as a treatment for Parkinson's disease. In temporal association, she had committed numerous shoplifting offences. While pathological gambling, excessive shopping and hypersexuality have often been described as adverse effects of dopamine agonists, there are only few reports about impulsive stealing in this context. We discuss the synergy of the pharmacological effects with familial, biographic and social factors, suggesting a bio-psycho-social etiology.

[Dialectical-behavioral outpatient therapy for adolescents with impulsive and self-harming behavior].

A slightly modified version of the Dialectical-Behavioral Therapy for Adolescents (DBT-A) for impulsive and self-injurious adolescents has been implemented in the Child and Adolescent Psychiatric Service in Zurich, Switzerland, since 2005. This DBT-A comprises individual therapy, skills training, and a single parent meeting over a 6-month period. This article reports on the translation of this approach into clinical practice and presents an evaluation based on the clinical quality of control assessments. Participants of the treatment program were 43 female adolescents aged 14 to 19 living in t

[Hypersexuality in Parkinson's disease. Advantage of the presence of the entourage for medical assessment].

OBJECTIVE: To improve the management of hypersexuality caused by antiparkinsonian treatment and its psychopathological implications in patients with Parkinson's disease (PD). If hypersexuality is a classic form of impulse control disorder (ICD) observed in PD, its rate is certainly underestimated. METHODS: We have proposed to meet patients with Parkinson's disease, referred by the neurology department of Lille University Hospital, for detection or suspicion of hypersexuality, in the presence of their spouse. The session consisted of an interview conducted by our psychiatry team. This evaluati

Improving impulse control: using an evidence-based practice approach.

School social workers in a suburban school district implemented a two-year collaboration to identify empirically effective interventions and apply them to their own practice. During the first year of the project, the workers and the consultant discussed the use of evidence-based practice and developed strategies and tools for monitoring and evaluating practice. In the second year, each worker monitored the progress of one student using a standardized intervention. Five social workers completed the project; collected data on outcomes showed improved self-control for all participants. Limits on

Biological contributions to addictions in adolescents and adults: prevention, treatment, and policy implications.

PURPOSE: Despite significant advances in our understanding of the biological bases of addictions, these disorders continue to represent a huge public health burden that is associated with substantial personal suffering. Efforts to target addictions require consideration of how the improved biological understanding of addictions may lead to improved prevention, treatment, and policy initiatives. METHOD: In this article, we provide a narrative review of current biological models for addictions with a goal of placing existing data and theories within a translational and developmental framework t

The role of impulse control disorders in Tourette syndrome: an exploratory study.

INTRODUCTION: Tourette syndrome (TS) is a neurodevelopmental condition characterised by the presence of multiple motor tics and one or more phonic tics, often associated with co-morbid behavioural problems. Impulse control disorders (ICDs) are a set of disorders where patients have significant difficulties in controlling their urges to perform rewarding behaviours. ICDs are expected to be common in patients with TS, as many problems reported in TS are related to difficulties in controlling impulsivity. AIMS: This exploratory study aimed to determine the clinical characteristics of ICDs in adu

Impulse-control disorders in college students.

We investigated impulse-control disorders (ICDs), alcohol use and outcome expectancies in students (n=571, 63.9% female, age: M=21.7) using questionnaires. Twenty (3.5%) were positively screened for lifetime ICDs, mostly males (n=16, 80%). Disorder-specific rates ranged between 0 and 1.2%. Differences in alcohol use and expectancies between ICDs and controls did not reach significance.

Factors related to correctional facility incarceration among active injection drug users in Baltimore, MD.

AIM: We investigated the moderating effect of impulse control on the association between drug use and incarceration among active injection drug users (IDU). METHODS: The study sample consisted of 282 IDUs aged 15-50 years from the Baltimore metropolitan region who reported injection drug use within the past 6 months and indicated that heroin or speedball was their drug of choice. Impulse control was measured using commission error standardized scores from the Test of Variables of Attention (TOVA). Incarceration was obtained using self-reported lifetime history of incarceration in correctional

Psychological treatment dropout among pathological gamblers.

Premature dropout from treatments for pathological gambling is potentially of significant importance, if it occurs before substantial progress has been made in addressing the problem. A systematic review of current research on dropout from psychological treatments for pathological gambling identified 12 studies from five countries. Dropout ranged from 14% to 50%, with a median of dropout 26%. Overall, 31% of the participants dropped out of treatment. Few studies distinguish between dropouts at different stages of participation. The evidence on specific variables that predict dropout is limited

GPs and problem gambling: can they help with identification and early intervention?

General Practitioners (GPs) are well placed to identify problem gamblers and provide early intervention. To date there is no evidence to suggest that GP's are routinely screening patients for potential gambling problems. This paper discusses the prevalence of problem gambling, the links with other health problems and ways that GPs can assist. Results from a pilot project that provided educational resources to GPS are also discussed. Suitable screening tools are available that could easily be used by GPs to assess the possibility of gambling problems in patients who may be at increased risk but

Measuring state changes in human delay discounting: an experiential discounting task.

A new experiential discounting task (EDT) is presented. Unlike existing question-based measures of delay discounting, which rely on imagined consequences during task completion, this EDT requires that participants experience choice consequences (i.e. delays and pseudo consumatory responses) during the measurement period. As a preliminary examination of this task's sensitivity to variability in discounting, 12 participants (six females) completed a timing test (production and reproduction), a question-based measure of delay discounting, and the EDT during non-sleep-deprived (awake 7 h) and slee

Citalopram treatment of compulsive shopping: an open-label study.

BACKGROUND: Compulsive shopping, a DSM-IV impulse-control disorder not otherwise specified, is characterized by preoccupation with shopping and inability to resist buying unneeded items, with resulting marked distress, social or occupational impairment, and financial and/or familial problems. Because an open-label trial suggested that fluovaxamine, a selective serotonin reuptake inhibitor (SSRI), is effective for this disorder, we tested the effectiveness of the SSRI citalopram. METHOD: We enrolled adults meeting formal diagnostic criteria (as defined by McElroy and colleagues) in a 12-week o

A historical appraisal of America's experience with "pyromania"--a diagnosis in search of a disorder.

When "pyromania" from 1840-1890 is reviewed, it stands out as a concept that at first found favor in an era of moral insanity and moral treatment. During this period pyromania was variously labeled as a form of monomania, moral insanity, impulsive mania, or instinctive mania. As early as 1850, and clearly after the Civil War, however, controversial arguments regarding the locus of personal responsibilty emerged. Arguments were put forward that there could be diseased brains but not diseased minds. Without the proof of an organic lesion in the brain, fire-setting became an act whose locus reste

Evidence-based practice in group work with incarcerated youth.

As a result of the Youth Criminal Justice Act's increased focus on restorative justice, treatment, rehabilitation, and reintegration of youth, many more juvenile offenders require mental health services while resident in youth detention facilities [Youth Criminal Justice Act (2002, c.1). Ottawa: Department of Justice Canada. Retrieved September 19, 2008 from http://laws.justice.gc.ca/en/Y-1.5]. Several common characteristics such as violence, aggression, and other antisocial behaviors, associated with criminal behavior, have been identified among male and female offenders. Externalizing spectrum or spectra? Underlying dimensions of the externalizing spectrum.

BACKGROUND: Major psychiatric disorders have been conceptualized to comprise of two dimensions namely, the internalizing and externalizing spectrum. Externalizing spectrum disorders consist of childhood disruptive behaviour disorders (DBD), antisocial personality disorder (ASPD) and substance use disorders. They are supposed to share common underlying personality traits. However, there is a need to explore the underlying dimensionality of externalizing spectrum of both disorders and traits in a mixed clinical and nonclinical sample from a non-western cultural-ethnic backdrop. Psychiatric comorbidity among first-time and repeat DUI offenders.

Driving under the influence of alcohol or other substances is a serious public health concern. Previous research has shown that psychiatric comorbidity is more prevalent for repeat offenders than the general population, and that first-time offenders exhibit elevated rates of psychiatric comorbidity, but few studies have directly compared first-time and repeat DUI offenders. The current study compares psychiatric comorbidity among repeat and first-time DUI offenders. First-time and repeat DUI offenders completed the screener module of the Computerized Assessment and Referral System (CARS), adap

Assessing inattention and impulsivity in children during the Go/NoGo task.

Behavioural performance in the Go/NoGo task was compared with caregiver and teacher reports of inattention and hyperactivity-impulsivity in 1,151 children (N = 557 boys; N = 594 girls) age 9- 10-years-old. Errors of commission (NoGo errors) were significantly correlated with symptom counts of hyperactivity-impulsivity, while errors of omission (Go errors) were significantly correlated with symptom counts for inattention in both caregiver and teacher reports. Cross-correlations were also evident, however, such that errors of commission were related to inattention symptoms, and errors of omissio
