Cochlear Implantation in a Child With Arnold-Chiari Syndrome, Severe Inner Ear Malformations, and Cochlear Nerve Hypoplasia: A Case Report.

BACKGROUND Arnold-Chiari syndrome is a congenital malformation of the posterior cranial fossa associated with neurological and otoneurological abnormalities, including hearing loss of variable severity. Cochlear implantation in patients with complex central nervous system malformations remains challenging due to uncertain auditory outcomes. This study aimed to evaluate the feasibility and early outcomes of cochlear implantation in a child with Arnold-Chiari syndrome and severe bilateral inner ear malformations. CASE REPORT A 1.5-year-old girl with Arnold-Chiari malformation, hydrocephalus, spi

Convolutional neural network metal artifact reduction in ultra-high resolution photon counting detector CT of middle- and inner-ear implants.

BACKGROUND: Metallic inner- and middle-ear implants with submillimeter structures requiring ultra-high resolution (UHR) imaging invariably result in metal artifacts at CT, which can obscure adjacent microstructures and subtle pathologic findings. Current metal artifact reduction techniques can be less effective for small metal objects or metal in heterogeneous background anatomy, and can sometimes introduce new artifacts or noise. PURPOSE: To develop and evaluate an image-domain convolutional neural network to reduce metal artifacts near inner- and middle-ear implants in UHR photon-counting-d

Endolymphatic hydrops impairs inner ear uptake and distribution of intratympanically injected gadolinium mixed with dexamethasone in patients with Meniere's disease.

PURPOSE: To investigate how endolymphatic hydrops (EH) affects the entry of intratympanic gadolinium in the inner ear of patients with Meniere's disease (MD). MATERIALS AND METHODS: This study included 166 MD patients. MRI was performed 24 h after intratympanic injection of dexamethasone and 20-fold diluted Gd-DTPA. The region of interest with the strongest enhancement in the cochlea and vestibule was selected to calculate the signal intensity ratio (SIR), representing transport speed. The enhancement area in the vestibular perilymph, indicating the distribution volume of Gd-DTPA, was measure

A retrospective review of divers treated for inner ear decompression sickness at Fiona Stanley Hospital hyperbaric medicine unit 2014-2020.

INTRODUCTION: Inner ear decompression sickness (IEDCS) is increasingly recognised in recreational diving, with the inner ear particularly vulnerable to decompression sickness in divers with a right-to-left shunt, such as is possible through a persistent (patent) foramen ovale (PFO). A review of patients treated for IEDCS at Fiona Stanley Hospital Hyperbaric Medicine Unit (FSH HMU) in Western Australia was performed to examine the epidemiology, risk factors for developing this condition, the treatment administered and the outcomes of this patient population. METHODS: A retrospective review of

Noise-induced damage in the zebrafish inner ear endorgans: evidence for higher acoustic sensitivity of saccular and lagenar hair cells.

The three otolithic endorgans of the inner ear are known to be involved in sound detection in different teleost fishes, yet their relative roles for auditory-vestibular functions within the same species remain uncertain. In zebrafish (Danio rerio), the saccule and utricle are thought to play key functions in encoding auditory and vestibular information, respectively, but the biological function of the lagena is not clear. We hypothesized that the zebrafish saccule serves as a primary auditory endorgan, making it more vulnerable to noise exposure, and that the lagena might have an auditory func

Refining surgical techniques for efficient posterior semicircular canal gene delivery in the adult mammalian inner ear with minimal hearing loss.

Hearing loss is a common disability affecting the world's population today. While several studies have shown that inner ear gene therapy can be successfully applied to mouse models of hereditary hearing loss to improve hearing, most of these studies rely on inner ear gene delivery in the neonatal age, when mouse inner ear has not fully developed. However, the human inner ear is fully developed at birth. Therefore, in order for inner ear gene therapy to be successfully applied in patients with hearing loss, one must demonstrate that gene delivery can be safely and reliably performed in the matu

Epac1 Signaling Pathway Mediates the Damage and Apoptosis of Inner Ear Hair Cells after Noise Exposure in a Rat Model.

To investigate the role of the exchange protein directly activated by cAMP (Epac) signaling pathway in inner ear hair cell damage and apoptosis after noise exposure, we analyzed the expression level of Epac1 in a rat model of noise-induced hearing loss (NIHL), based on rat exposure to a 4-kHz and 106-dB sound pressure level (SPL) for 8 h. Loss of outer hair cells (OHCs), mitochondrial lesions, and hearing loss were examined after treatment with the Epac agonist, 8-CPT, or the Epac inhibitor, ESI-09. The effects of 8-CPT and ESI-09 on cell proliferation and apoptosis were examined by CCK-8 assa

Salidroside protects inner ear hair cells and spiral ganglion neurons from manganese exposure by regulating ROS levels and inhibiting apoptosis.

Manganese (Mn) is an essential cofactor for many enzymes and thus plays an important role in normal growth and development. However, persistent exposure to high Mn concentrations can result in deleterious effects on not only the central nervous system but also peripheral nerves, including nerves associated with the auditory system. Our initial research on cochlear organotypic cultures in vitro showed that N-acetylcysteine (NAC) clearly decreases Mn-induced losses in hair cells (HCs), auditory nerve fibers (ANFs) and spiral ganglion neurons (SGNs) in a concentration-dependent manner. Salidroside

Middle and inner ear malformations in mutation-proven branchio-oculo-facial (BOF) syndrome: case series and review of the literature.

Hearing impairment is common in individuals with branchio-oculo-facial (BOF) syndrome. The majority of described individuals have conductive hearing impairment due to malformed ossicles and/or external canal stenosis or atresia, although a sensorineural component to the hearing impairment in BOF syndrome is increasingly being reported. Sophisticated computed tomography (CT) of the temporal bone has revealed middle and inner ear malformations in three previous reports. We present middle and inner ear abnormalities in three additional individuals with mutation-proven BOF syndrome. We suggest tha

Spiral ligament fibrocyte-derived MCP-1/CCL2 contributes to inner ear inflammation secondary to nontypeable H. influenzae-induced otitis media.

BACKGROUND: Otitis media (OM), one of the most common pediatric infectious diseases, causes inner ear inflammation resulting in vertigo and sensorineural hearing loss. Previously, we showed that spiral ligament fibrocytes (SLFs) recognize OM pathogens and up-regulate chemokines. Here, we aim to determine a key molecule derived from SLFs, contributing to OM-induced inner ear inflammation. METHODS: Live NTHI was injected into the murine middle ear through the tympanic membrane, and histological analysis was performed after harvesting the temporal bones. Migration assays were conducted using the

Glycerol pure tone audiometry and glycerol vestibular evoked myogenic potential: representing specific status of endolymphatic hydrops in the inner ear.

This prospective study attempts to explore the effect of glycerol on vestibular evoked myogenic potential (VEMP) in patients with definite unilateral Meniere's disease (MD) and to investigate whether the test reflects different pathologic states of endolymphatic hydrops (EH) in the inner ear by comparing the results of glycerol pure tone audiometry (PTA). Twenty-eight patients with definite unilateral MD were studied. As a control group, 11 patients with vestibular neuritis were selected. Sixteen ears of normal volunteers were enrolled to acquire normal range of the test. Each MD patient under

Preventive magnesium supplement protects the inner ear against noise-induced impairment of blood flow and oxygenation in the guinea pig.

We have recently demonstrated in the guinea pig that preventive dietary magnesium supplement can significantly reduce hearing loss caused by acute impulse noise exposure. To elucidate the underlying protective mechanisms of magnesium, the present study examined its effect on noise-induced impairment of cochlear blood flow (CoBF) and perilymphatic oxygen partial pressure (pO2) in two groups of guinea pigs maintained on optimal or suboptimal dietary magnesium status. While laser Doppler flowmetry was used to determine CoBF, perilymphatic pO2 was measured polarographically using micro-coaxial pla
