Distal humeral epiphyseal separation in a young athlete: a case report.

Elbow pain in the young throwing athlete can have numerous etiologies. Skeletally immature athletes commonly sustain epiphyseal injuries secondary to trauma and/or repetitive activity. The case of a 7-year-old Little League pitcher with the diagnosis of distal humeral epiphyseal separation is presented along with a review of the current literature.

Correction of elbow flexion contracture in late obstetric brachial plexus palsy through arthrodiatasis of the elbow (Ioannina method).

Inadequate elbow extension is a recognized sequela after obstetric brachial plexus injury because of muscle imbalance and adversely affects the ability to perform sufficiently most daily living activities. The various methods that have been used to correct this deformity do not offer a satisfactory outcome in the long term and show a tendency for gradual recurrence. We present a new technique of a closed gradual arthrodiatasis using a unilateral hinged elbow external fixator. The technique was applied in 10 patients with elbow flexion contracture as a sequela of brachial plexus birth palsy. Lo

Dynamic electromyography to assess elbow spasticity.

Control of elbow motion was evaluated in 45 extremities of adults with spasticity resulting from traumatic brain injury with use of dynamic electromyography. Simultaneous recording of elbow motion was obtained using a double parallelogram goniometer. Thirty-four male and 9 female patients were studied. Mean elbow flexion was 85 degrees and mean extension was 20 degrees. The average time of elbow flexion was 1.8 seconds. Extension time was prolonged to a mean of 4.0 seconds. Dynamic electromyography revealed a consistent pattern of muscle activity. Severe spasticity was noted in the brachioradi

Chronic lateral elbow instability.

Posterolateral rotatory instability of the elbow is the most common pattern of chronic lateral elbow instability. The primary lesion in posterolateral rotatory instability is injury or attenuation of the lateral ulnar collateral ligament. Posterolateral rotatory instability is diagnosed on the basis of careful history taking and specific physical examination techniques. Reconstruction of the lateral ulnar collateral ligament with repair of the surrounding soft tissue structures is recommended in patients who have symptoms of recurrent lateral instability. Open and arthroscopic reconstruction t

Elbow Flexion Excursion: Intra- and Inter-Throwing Arm Kinetic Evaluations in High School and Professional Baseball Pitchers.

BACKGROUND: Elbow flexion at stagnant time points of the pitch has been associated with increased elbow varus torque (EVT) and elbow injury history. PURPOSE: To determine how elbow flexion angle throughout the pitch (excursion) between individual pitchers and a cohort of pitchers influences throwing arm kinetics in high school (HS) and professional (PRO) pitchers. STUDY DESIGN: Descriptive laboratory study. METHODS: HS (n = 59) and PRO (n = 288) pitchers were instructed to throw 8 to 12 fastball pitches evaluated with 3-dimensional motion capture system (480 Hz). Linear regression models we

Coronoid Fractures.

UNLABELLED: Fractures of the coronoid process of the ulna are usually part of a more complex injury. The type of coronoid fracture corresponds with a specific pattern of injury with known pitfalls and recommended treatments. Tip fractures usually correspond with dislocation of the elbow and fracture of the radial head (the so-called terrible triad of the elbow because it is prone to redislocation, subluxation, and arthrosis) and are best repaired with a suture passed through drill holes along with repair or replacement of the fractured radial head and reattachment of the lateral collateral lig

The pathoanatomy of lateral ligamentous disruption in complex elbow instability.

We sought to determine the lateral soft-tissue injury pattern in a consecutive series of patients with elbow dislocation (10 cases) or fracture-dislocation (52 cases) that required open operative repair. Patients who were seen more than 3 months after injury or those in whom previous operative intervention had obscured the anatomy were excluded. There were 42 men and 19 women (mean age, 43 years; range, 13-82 years). One patient had bilateral injuries. The mean time to surgery was 15 days after injury, with a range from 1 to 76 days. There were associated fractures in 52 elbows: coronoid (39),

Intermittent pneumatic compression effect on eccentric exercise-induced swelling, stiffness, and strength loss.

OBJECTIVE: The purpose was to determine if intermittent pneumatic compression (IPC) affects muscle swelling, stiffness, and strength loss resulting from eccentric exercise-induced injury of the elbow flexors. We hypothesized that the compression would decrease swelling and stiffness. DESIGN: Repeated measures design with a before-after trial comparison within each day. SETTING: Conducted at a university Somatic Dysfunction Laboratory. SUBJECTS: Twenty-two college women students were studied. They had not been lifting weights or otherwise participating in regular arm exercise for the 6 month

Isokinetic profile of elbow and shoulder muscle strength of seated throwers.

Throwing events such as discus, shot put, and javelin involve repetitive and explosive overhead activities in a short period, which increases the risk of upper extremity injuries. Furthermore, the available data on seated throwers are inadequately documented. Thus, this study aims to (1) describe the muscle strength profiles of shoulder external and internal rotators and the elbow flexors and extensors in seated throwers, and (2) identify the differences in muscle strength between seated throwers competing at international levels and those at national levels. Twenty-one seated throwers partici

Dislocation of the elbow with ipsilateral forearm fracture. Six particular cases.

Elbow dislocation associated with ipsilateral radial shaft fracture is an infrequent injury (nine cases reported in the literature). We present six new cases observed between 2006 and 2012, with an average age of 31 years and a mean follow-up of 18 months. The forearm fracture and ipsilateral dislocation of the elbow were probably caused by forearm hypersupination with extension of the elbow. The dislocation was reduced by manipulation before open reduction and osteosynthesis of the forearm fracture. Four elbows were stable after reduction; two markedly unstable elbows necessitated temporary h

Investigation on 2331 cases of pulled elbow over the last 10 years.

Pulled elbow is a common upper extremity injury in children. We present a retrospective study of 2331 pulled elbow cases examined in our hospital over the last ten years. All pediatric patients with a diagnosis of pulled elbow from January 2002 to December 2011 were retrospectively reviewed according to sex, age, affected arm, recurrence rate, mechanism of injury and treatment outcomes. There is no significant sex difference. The frequency of injury peaked for both boys and girls at 6 months and 2 years of age. The left arm was more affected than the right. The recurrence rate was 14%. In abou

Dislocation of the elbow with intra-articular entrapment of the medial epicondyle in adults. Report of two cases.

Intra-articular entrapment of the medial epicondyle with dislocation of the elbow is a common injury pattern in childhood. The occurrence of this injury is extremely rare in adults. We present two cases of dislocation of the elbow with intra-articular inclusion of the medial epicondyle in adult patients. One case was treated by closed reduction and the other required open reduction. A brief review of the scanty literature on the subject is also presented. The case reports highlight the need for early diagnosis and management of these atypical cases in adults.

Obesity may be a risk factor for recurrent heterotopic ossification in post-traumatic stiff elbow among children and teenagers.

BACKGROUND: Post-traumatic elbow stiffness and heterotopic ossification (HO) affects long-term life quality, as commonly in children and teenagers as in grownups. Childhood obesity considerably influences public health because it causes stroke, hypertension and diabetes mellitus. Previous research discussed its clinical complications in orthopedic diseases. However, no clinical research reveals the interaction between childhood obesity and HO after elbow injuries. HYPOTHESIS: Obesity might be a risk factor of recurrent HO after elbow arthrolysis in children and teenagers, correlated with the …
