Hypothenar hammer syndrome (HHS) is a rare cause of digital ischemia and pain caused from repetitive trauma to the palm. Often related to occupational practices, thrombosis and embolization can occur. Treatment is often surgical and involves excision with or without reconstruction. RATIONALE: The treatment of secondary aortoenteric fistula (SAEF) involves maintaining hemodynamic stability, infection control, revascularization, and surgical repair. Conventional open repair is associated with high mortality, whereas endovascular stent-graft repair is associated with recurrent infection or bleeding.
PATIENT CONCERNS: We report the case of an 85-year-old man with SAEF who presented with gastrointestinal bleeding and retroperitoneal abscess.

Iatrogenic lacrimal drainage trauma during surgery is not common. It has been reported during orbital surgeries, medial maxillectomies, post-trauma repairs, excision of adherent lesions in the vicinity, and facial reconstructive procedures. The present case reports a focal loss of the lacrimal sac wall during the excision of an intricate dermoid cyst. The defect was sealed by a nasal mucosal graft harvested from the floor of the ipsilateral nasal cavity. Postoperatively, the epiphora was resolved, and a CT-dacryocystography confirmed the anatomical integrity of the reconstructed lacrimal sac.

The gold standard for bulbar urethroplasty has been excision and primary anastomosis. Application of this approach is generally limited to strictures that are 2 cm or less in the bulbar urethra due to penile shortening. Strictures greater than 2 cm are successfully treated with augmentation urethroplasty wherein the narrowed segment is not excised but widened with the use of a skin flap or a tissue graft. Buccal mucosa is the most prevalent tissue for bulbar urethral stricture repair for strictures greater than 2 cm. Outcomes for the different graft locations are similar, approximately 90%.

Two boys, 10 years of age, with ventricular septal defect and severe aortic insufficiency were treated using a new method. An autograft of anterior pulmonary artery wall and the adjacent leaflet and subpulmonary muscle was excised. The excised pulmonary artery segment was used to replace the prolapsed right coronary cusp, and the autograft muscle was used to close the ventricular septal defect. The right ventricular outflow tract and pulmonary valve were repaired using an autologous pericardial patch with an autologous single cusp. The results in both cases were satisfactory.

The roles of DNA crosslink and its repair in the induction of sister-chromatid exchanges (SCEs) were studied in normal, xeroderma pigmentosum (XP) complementation group A, and Fanconi's anemia (FA) fibroblasts after treatment with mitomycin C (MC) or decarbamoyl mitomycin C (DMC) for 1 h. A 40-year-old G3P3 woman with grade 3 cervical intraepithelial neoplasia underwent loop electrosurgical excision procedure complicated by a large vesicovaginal fistula. She was initially managed with an indwelling Foley catheter to allow for fistula maturation. During planned surgical repair it was discovered that the fistula had closed spontaneously.

Four cases of acquired urethral diverticula in male subjects are reviewed. All patients presented with complications such as urinary tract infection, urethral fistulas and stone or sequestrum formation in the diverticulum. The recommended treatment is excision of the diverticulum and repair of the urethra. The relevant literature has been reviewed.

Giant-cell tumors and aneurysmal bone cysts involving the lower end of fibula pose special management problems. Excision of the lower end of the fibula in such cases is undertaken reluctantly because it may result in lateral instability of the ankle. Two cases, one of giant-cell tumor and one of aneurysmal bone cyst of the lower end of the fibula, were excised and the lateral ligaments of the ankle and the periarticular tissue were carefully repaired. In both patients the ankle joints were stable two years after the operation.
