Compared to moderate-intensity continuous training, short-term high-intensity interval training demonstrates enhanced effects on metabolic flexibility in adult males with obesity.

BACKGROUND: The aim of this study was to assess the effects of short-term high-intensity interval training (HIIT) and moderate-intensity continuous aerobic training (MICT) with matched energy expenditure on metabolic flexibility and other metabolic parameters in adult male individuals with obesity. METHODS: Twenty male individuals with obesity (age: 21.4 ± 1.5 years; body mass index: 31.0 ± 3.6 kg/m2) were enrolled in this crossover design study. Participants were randomly allocated to an intervention sequence of 1) 3 consecutive days of HIIT (30 min, 6 × 2.5 min bouts at 90 % Peak Oxygen Upt

Role of Cardio-Oncology Rehabilitation in Hematopoietic Stem Cell Transplantation and Chimeric Antigen Receptor T-Cell (CAR-T) Therapy.

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) and chimeric antigen receptor T-cell (CAR-T) therapy often lead to severe sarcopenia and cachexia during treatment, making it difficult to maintain exercise tolerance. Consequently, "cancer rehabilitation" programs have been implemented to sustain and improve physical activity and motor function. Hematologic malignancies often involve the use of cardiotoxic drugs. Moreover, graft-vs.-host disease associated with allo-HSCT and the cytokine release syndrome in CAR-T therapy elevate the risk of cardiovascular complications. Thus, esta…

Motor Pathophysiology Related to Dyspnea in COPD Evaluated by Cardiopulmonary Exercise Testing.

In chronic obstructive pulmonary disease (COPD), exertional dyspnea, which increases with the disease's progression, reduces exercise tolerance and limits physical activity, leading to a worsening prognosis. It is necessary to understand the diverse mechanisms of dyspnea and take appropriate measures to reduce exertional dyspnea, as COPD is a systemic disease with various comorbidities. A treatment focusing on the motor pathophysiology related to dyspnea may lead to improvements such as reducing dynamic lung hyperinflation, respiratory and metabolic acidosis, and eventually exertional dyspnea.…

ST elevation in recovery post exercise with normal coronary arteries.

We present the case of a 45-year-old healthy man who successfully completed three stages of the Bruce protocol but developed inferolateral S … e primary event, inducing sufficient myocardial ischaemia to cause a marked drop in blood pressure. Exercise tolerance testing is often a reliable test to rule out reversible myocardial ischaemia. While the physician is focused on ischaemic changes or rhythm abnormalities developing during the exercise phase, the recovery period is just as important and requires as …

Follow-up of surgically treated patients with massive pulmonary embolism--with reference to 12 operated patients.

Since 1978, 17 patients have undergone surgery for massive pulmonary embolism in our department. Twelve patients survived and have been followed up for between 2 and 31 months postoperatively (mean 16 months). Reassessment of these patients included exercise tolerance test, pulmonary function test, perfusion scan, right heart catheterization and coagulation screening. Two survivors present major sequelae, namely vascular pruning and definite signs of pulmonary hypertension. The other 10 patients have minimal or no residual vascular occlusion, but show a high incidence of minor abnormalities: s…

Achieving normal pulmonary function following tracheoplasty in infancy.

Infant long-segment congenital tracheal stenosis (LTS) is rare and presents a challenging clinical scenario. We describe the management of a child who required extracorporeal membrane oxygenation following a respiratory arrest and underwent slide tracheoplasty in infancy for severe LTS and required repeated bronchoscopic reinterventions for recurrent tracheal granulations. At 9 years of age, the child has normal pulmonary function testing and a normal exercise tolerance.

Cor triatriatum with persistent levoatrial cardinal vein late presented as severe mitral stenosis.

Cor triatriatum is a rare congenital cardiac anomaly that usually becomes symptomatic in the first years of life. We present a 28-year-old pregnant female patient presented with shortness of breath, palpitations and decreased exercise tolerance. Transthoracic (TTE) and transesophageal (TEE) echocardiography showed cor triatriatum, features of severe mitral stenosis and atrial septal defect (ASD). The patient underwent successful surgical correction with an uneventful postoperative course.

Cardiovascular Benefits of Exercise Training in Postmenopausal Hypertension.

Exercise training is often considered the cornerstone of nonpharmacological therapy for postmenopausal hypertension while aerobic exercise is the mainstay of life style modification for antihypertension. Moderate-intensity aerobic exercise is well tolerated on most days of the week by most people with postmenopausal hypertension and is not suspected to detract from exercise adherence. That being said, moderate aerobic exercise may be superior for eliciting cardiovascular benefits in hypertensive postmenopausal women and resistance exercise may offer desirable benefits. The beneficial outcomes

Acute impairments in glucose tolerance following one night of partial sleep restriction are not rescued by moderate-intensity walking in young men.

BACKGROUND: High-intensity interval exercise ameliorates the impairment of postprandial metabolic health (e.g., glucose control) that is observed after a night of inadequate sleep. It is unknown whether moderate-intensity walking can elicit similar effects. METHODS: Eleven healthy active males (age: 26 ± 2 yr; BMI: 22.8 ± 2.6 kg/m2) took part in a randomized and repeated-measures cross-over study with: (1) normal sleep (NS; 8 h sleep opportunity); (2) sleep restriction (SR; 3 h sleep opportunity); and (3) sleep restriction + exercise (SRE; 3 h sleep opportunity followed by 30 min brisk walkin

The effects of green exercise on the mental and physical health of people with chronic conditions: a systematic review.

Green exercise, defined as physical activity in natural settings, shows promise for enhancing exercise participation and improving health. This systematic review aimed to assess the effectiveness of green exercise in people with chronic conditions. Seven electronic databases were searched and of the 7801 screened articles, 14 trials met the inclusion criteria. Green exercise was a safe and well-tolerated intervention, with low drop-out levels. It was found to positively affect participants' quality of life in three studies and mental health in four studies. Compared to non-exercise groups, gre

Topical application of the cold-mimetic l-menthol decreases wheel running without affecting the beneficial effects of voluntary exercise in mice.

Topical application of l-menthol, a pharmacological cold-mimetic and agonist of the cold-sensing receptor TRPM8 (transient receptor potential cation channel subfamily M member 8), has been shown to stimulate brown adipose tissue (BAT) thermogenesis and reduce weight gain in both obese and lean male mice, without affecting energy intake. While these findings suggest that l-menthol could offer a novel approach to prevent weight gain, its potential to enhance the benefits of exercise on whole-body metabolic health remains unexplored. In this study, we investigated whether daily topical l-menthol

Silent ischaemia in post-myocardial infarction patients submitted to physical training.

Recently, more attention has been focused on the detection and treatment of silent myocardial ischaemia. Electrocardiographic signs of exercise-induced asymptomatic myocardial ischaemia are very common findings among survivors of acute myocardial infarction. From data of our population we found that silent exercise-induced ischaemia is present in 15-20% of all patients, and that about half of the patients with exercise-induced ST-segment depression were free of symptoms. Ergometric data at the ischaemic threshold are similar between asymptomatic and symptomatic patients while the presence of s
