The treatment of comminuted tibial shaft fractures in canine patients is burdened by significant risk which involves bone healing complications, such as delayed bone union. Complications may result from iatrogenic damage to blood vessels during fracture stabilization. To minimize this risk, treatment methods increasingly often rely on the concept of biological osteosynthesis. One of such methods involves the treatment of fractures with the use of new hybrid fixator consisted of an interlocking nail connected with type I external fixator. Connection of the nail with external fixator has been re

BACKGROUND: The purpose of this study was to determine the clinical benefit of hydroxyapatite (HA)-coated pins compared with standard stainless steel pins in external fixators applied for unstable fractures of the distal radius.
METHODS: A total of 40 patients (160 pins) with unstable wrist fractures were randomised for uniplanar fixator treatment with the use of identically designed, commercially available pins either composed of stainless steel (steel group) (n = 20) or coated by hydroxyapatite (HA group) (n = 20). Each pin site was clinically evaluated concerning erythema and grade of drai…

The technique of two simultaneous pedicled flaps to a single extremity has recently proven useful in the care of war-injured military personnel. We present two cases of combat-injured Marines who underwent upper extremity reconstruction using simultaneous pedicled flaps. These cases illustrate a simple and successful alternative to free tissue transfer in providing coverage to complex soft tissue defects of the hand and forearm. Good outcomes were obtained in circumstances where free tissue transfer was not indicated.

Eighteen patients with stage 3 Kienböck's disease were treated by débridement of the necrotic core of the lunate and implantation of a free vascularized corticocancellous iliac bone. The wrist was stabilized with an external fixator during healing. The efficiency of the procedure for restoring the structure of the fractured lunate, preventing carpal collapse and improving the clinical outcome was assessed at a mean follow-up of 5 years. The graft became incorporated in the lunate in 16 of the 18 patients and no fracture of the reconstructed lunate or carpal collapse occurred in these cases. Th

Between 1987 and 1993, 41 grade 3B open tibial shaft fractures were treated with the unreamed tibial nail (URTN n = 22) or an external fixator (FIX n = 19). The method of treatment was left to the choice of the operating surgeon. Three below the knee amputations were performed, three patients died, and three were lost to follow-up. In all, 32 patients were followed up to union or at least for 1 year. There were no significant statistical differences between the two groups (P < 0.2, chi-squared, t-test) with respect to fracture type, fracture location, age, gender or accompanying injuries. The

PURPOSE: The proximal crescentic osteotomy is an effective procedure for hallux valgus correction, but stable fixation remains challenging. Our previous biomechanical study suggested that the MEF may offer comparable or greater stability than a specific cannulated screw fixation construct in a cadaveric model. This study aims to evaluate the anatomical safety of the MEF by investigating its relationship with critical neurovascular structures and tendons. METHODS: An anatomical dissection study was performed on ten foot cadaveric specimens. The MEF was applied using five mini-Schanz pins insert

PURPOSE: Knee dislocations (KDs) can be limb-threatening injuries that may require a temporising knee-spanning external fixator (KSEF) for stabilisation. Precise indications for this commonly utilised invasive immobilisation technique remain controversial and poorly defined. The purpose of this study was to establish consensus-driven indications for temporising KSEF use in the initial management of KDs.
METHODS: A working group of fellowship-trained orthopaedic surgeons generated clinical scenarios reflecting commonly debated indications for temporising KSEF application. Utilising a modified

BACKGROUND: Proximal phalangeal fractures with significant bone loss are inherently unstable, often resulting in significant malalignment secondary to the high degree of instability. Often significant bone loss is not amenable to surgical fixation alone and bone grafts are required to maintain length and reconstruct the given defect.
OBJECTIVE: This cases series exhibits three cases of fixation of proximal phalanx fractures of the thumb with associated bone loss using an independently constructed external fixation frame in combination with autologous iliac crest bone graft to restore bone sto

Background: Premature consolidation of the regenerated bone is a relevant and underreported complication of limb lengthening in achondroplasia, with a reported incidence of 7-15% in the international literature. The gain-of-function fibroblast growth factor receptor 3 (FGFR3) mutation paradoxically enhances bone-healing capacity, predisposing patients to accelerated callus maturation during distraction osteogenesis and exposing patients to a risk of invasive procedures to treat this issue. This study reports the incidence and clinical features of premature consolidation in a cohort of achondro
