[Dislocation of the extensor tendons of the hand at the metacarpo-phalangeal level (zone V of Verdan)].

MATERIAL AND METHODS: Traumatic dislocation of the extensor tendon over the metacarpophalangeal (MCP) joint is a rare lesion that is caused by the dysfunction or rupture of the sagittal band, which is an important stabilizing structure of the extensor tendon. This mechanical alteration presents itself as instability that affects function and may cause pain or snapping during finger motion. The diagnosis is made when ulnar dislocation of the extensor tendon is observed over the MCP joint. Nonsurgical treatment is successful and should be attempted when injuries are diagnosed within the first th

[Treatment of Dupuytren's disease with collagenase injections in Germany: efficacy and adverse effects in 110 treated joints].

Background Since 2011, a new minimally-invasive treatment for Dupuytren's disease has been available in the form of injectable collagenase (Xiapex®, Pfizer). Purpose Our aim is to report about the results and adverse effects of injectable collagenase for the treatment of Dupuytren's disease in Germany. Patients and Methods We treated 78 patients (61 male, 17 female) with 87 injections in 87 digits (little finger n = 40, ring finger n = 37, middle finger n = 8 and index n = 2). We documented the range of motion before treatment, after 6 weeks and after 6 months. We were able to include 39 patie

Intramedullary Percutaneous Fixation of Extra-Articular Proximal and Middle Phalanx Fractures.

Multiple methods have been described for treating unstable proximal and middle phalangeal fractures. Irrespective of using an open or closed technique of fixation, stiffness and extensor lag at the proximal/distal interphalangeal joint almost always occur. This issue can be avoided by allowing the patients to mobilize the fingers out of plaster or splint as early as possible from the day of surgery. We describe a technique of intramedullary percutaneous fixation of extra-articular proximal and middle phalanx fractures allowing immediate mobilization of fingers, concurrent stabilization with pr

Transient Vasospastic Response Following the Injection of Corticosteroid into the Hand.

A sixty-year-old right hand dominant woman with longstanding left basal joint arthritis and right small trigger finger presented for corticosteroid injections to both areas. She had previously received injections with no adverse effects and good relief of symptoms. Following this most recent injection of corticosteroid, she experienced transient ischemia of the left hand and the right long and ring fingers. Corticosteroid injections can rarely cause local vasospasm, even when not inadvertently injected into the vascular system. This condition is self-limited following supportive treatment.

Thenar flap rehabilitated: a review of 20 cases.

The thenar flap is a satisfactory reconstruction technique for the treatment of extensive loss of the distal phalanx of the index and middle fingers. As we observed in this series of 20 patients, this flap is reliable and provides tissue that closely matches the missing finger pulp. Flap sensibility is good (average Weber of 6.5 mm). Transferring a distally based flap prevents proximal interphalangeal joint contracture and allows the surgeon to better reconstruct the outlines of the distal phalanx, thereby reducing the risk of a clawed nail. Finally, we did not observe major postoperative comp

The Design and Validation of an Open-Palm Data Glove for Precision Finger and Wrist Tracking.

Wearable motion capture gloves enable the precise analysis of hand and finger movements for a variety of uses, including robotic surgery, rehabilitation, and most commonly, virtual augmentation. However, many motion capture gloves restrict natural hand movement with a closed-palm design, including fabric over the palm and fingers. In order to alleviate slippage, improve comfort, reduce sizing issues, and eliminate movement restrictions, this paper presents a new low-cost data glove with an innovative open-palm and finger-free design. The new design improves usability and overall functionality

In Vivo Metacarpophalangeal Joint Kinematics After Silicone Implant Arthroplasty in Patients With Rheumatoid Arthritis.

PURPOSE: The aim of this study was to determine the potential mechanism of implant fracture using 3-dimensional motion analysis of patients with rheumatoid arthritis. METHODS: Active flexion motion in 9 hands (34 fingers) of 6 female patients with rheumatoid arthritis who previously underwent hinged silicone metacarpophalangeal joint arthroplasty was examined using 4-dimensional computed tomography. Positions of the proximal phalanges relative to the metacarpals were quantified using a surface registration method. The deformation of the silicone implant was classified in the sagittal plane in

Comparing brain activation associated with isolated upper and lower limb movement across corresponding joints.

It was shown recently that functional activation across brain motor areas during locomotion and foot movements are similar but differ substantially from activation related to upper extremity movement (Miyai [2001]: Neuroimage 14:1186-1192). The activation pattern may be a function of the behavioral context of the movement rather than of its mechanical properties. We compare motor system activation patterns associated with isolated single-joint movement of corresponding joints in arm and leg carried out in equal frequency and range. Eleven healthy volunteers underwent BOLD-weighted fMRI while p

Vibration syndrome.

Stewart, Alice M., and Goda, D. F. (1970).Brit. J. industr. Med.,27, 19-27. Vibration syndrome. Raynaud's phenomenon, or the finger blanching of men who work with vibrating tools, is undoubtedly due to vasospasm. Nevertheless the abnormal element in the situation is not a series of traumatized nerve endings but a deposition of callus under the palmar surfaces of fingers and thumbs. This deposition is a late consequence of the most distinctive, but not necessarily the most painful, of the numerous effects incurred as a result of the tool speed being completely out of the control of the operator

[Pure sensory stroke due to a pontine lesion: a study of clinical symptomatology and somatosensory evoked potential findings].

We studied clinical symptomatology and somatosensory evoked potentials(SEP) of 7 pure sensory stroke patients due to pontine lesions. Clinical symptoms were examined by modality(pinprick, touch, vibration and joint position sense), intensity and distribution of the sensory disturbance. SEP were recorded by the median nerve stimulation in the upper extremities, and the posterior tibial nerve stimulation in the lower extremities. Patients 1-4 were characterized by decreased contra-lesional fingers or fingers, toes joint position sense, normal pinprick and vibration sense. We think that these pat

An unusual presentation of metacarpophalangeal joint instability of the thumb.

Thumb injuries are not as common as those occurring in the fingers. The authors present the case of a patient who had an isolated avulsion of the extensor pollicis brevis that resulted in metacarpophalangeal joint instability, with intact radial and ulnar collateral ligament.

Novel Minimally Invasive Approach for Arthrodesis in Distal Interphalangeal Joint Using Microscope.

Arthrodesis is a definitive treatment for osteoarthritis of the distal interphalangeal (DIP) joint. The surgical approach is generally performed from the dorsal side of the DIP joint using a Y- or H-shaped incision. However, this approach can lead to relatively noticeable scarring and dorsal tissue complications. We introduced a microsurgery technique to address patients' desire for cosmetic improvement in addition to pain relief. In our modified approach, a small 1.5-cm skin incision was made along the mid-lateral line of the finger. This approach allows for a safer and faster synovectomy, os

Kirschner-wire fixation for postburn flexion contracture deformity and consequences on articular surface.

Kirschner-wire (K-wire) fixation for 3-6 weeks is an approved method for stabilization of the fingers after the release of flexion contracture deformity. On the other hand, articular surface damage in small joints due to pin fixation is still a topic of debate. Reports claiming permanent joint destruction due to this procedure exist in the literature. To clarify this doubt, a prospective study was carried out in 72 patients with flexion contracture of the hand fingers. After the surgical release of the deformity, immobilization of the interphalangeal (IP) and metacarpophalangeal (MCP) joints w

Dermatoglyphic and radiographic findings in a mother and daughter with pseudohypoparathyroidism.

The dermatoglyphic features of a mother and daughter with pseudohypoparathyroidism were compared with those of 19 other reported PHP cases and with findings typical of 45,X Turner syndrome. Our observations included large patterns with a predominance of whorls, unusual accidental patterns on the third fingers, elevated total and absolute finger ridge counts, extralimital digital triradii, intermediately placed axial triradii, and a single complete transverse palmar crease. With barium-coated hand radiographs, the positions of the palmar digital triradii were compared with those of the underlyi

Pathomechanics of lumbrical and flexor digitorum profundus muscle tears in climbers: A cadaveric model.

Lumbrical muscles originate on the flexor digitorum profundus (FDP) tendons and, during fist making, they move in the same direction when FDP muscle produces maximal proximal tendon gliding. Injuries of the bipennate lumbricals have been described when a shear force acts between the origins on adjacent tendons of the FDP, as they glide in opposite directions in asymmetric hand postures. Other structures of the deep flexors complex can be affected during this injury mechanism, due to the so-called quadriga effect, which can commonly occur during sport climbing practise. Biomechanical studies ar

[Case reports of pediatric electrical finger burn injuries' management and late- onset complications].

Pediatric electrical injuries are rare; they only constitute 2-10% of all burn causes. Determination of their actual severity may be challenging due to their small entry and exit wounds. Deep necrosis develops during electrical burns in most cases. These injuries can damage the skin, soft and bone tissues, and in children, the growth plate, which may cause secondary deformities. The objective of these case reports was the presentation of paediatric electrical finger injuries' management and late-onset complications. A 15-year-old boy touched an electric wire while changing a lightbulb, which c

Radiological changes and signs of osteoarthritis in the fingers of male performance sport climbers.

AIM: The aim of this study was to investigate the radiological changes and signs of osteoarthritis in the fingers of performance sport climbers that felt to be from acute mechanical stress placed on the fingers and intense training regime over years. METHODS: A total of 31 male sport climbers who were either strong rock climbers or former members of the Swiss climbing team, and 67 non-climbers participated in the study. Fisher's exact test was used to compare the proportions of signs of osteoarthritis between climbers and non-climbers. For the evaluation of radiological changes, antero-poster
