Traumatic hip dislocations associated with posterior wall fractures of the acetabulum in the pediatric population are in general a consequence of high-energy trauma. After expeditious reduction, instability mandates for further diagnosis and intervention. Plain radiographs or computerized tomography (CT) scans can misjudge the involvement of the posterior wall of the acetabulum due to the partially calcified nature of the pediatric bone. We present two cases of pediatric traumatic hip dislocation associated with posterior wall fractures of the acetabulum. In both cases, obvious postreduction i

The purpose of this paper was to describe a classification of posterior fracture dislocations of the hip joint, which would highlight the important aspects of managing this injury and more closely correlate with the prognosis. During the period from July 1994 to September 1997, the senior author operated on 60 posterior fracture dislocations of the hip joint. The results of the surgery were scored according to our ability to accurately anatomically reconstruct the acetabulum. We found that the degree of comminution was a greater determent of whether we could achieve an anatomical reconstructio…

The case of an 39-year-old man is presented, who sustained a bilateral locked fracture dislocation of the shoulders occurring during an epileptic seizure. Radiographs demonstrated a compression-fracture of the anteromedial aspect of the humeral head bilaterally (reversed Hill-Sachs-lesions). Additionally, fractures of the lesser and greater tuberosity were diagnosed at the right site. Open reduction and internal fixation was performed in both shoulders within 12 hours. 6 months later the patient has no complaints with a free range of motion. Diagnostics, treatment and result are discussed in c…

Hip dislocation after hip pinning has been reported in association with sepsis, but in this paper the authors describe a case of gradual, spontaneous dislocation of the hip over several weeks, without sepsis, in a 36-year-old man who had undergone screw-plate fixation of a displaced basi-cervical fracture. This is only the second reported case of spontaneous dislocation after hip pinning not associated with infection.

Shoulder dislocation associated with ipsilateral humeral shaft fracture is an uncommon combination. Two cases treated, one by open and the other by closed methods, are presented. The mechanism of this complex injury is detailed and the treatment modalities are discussed. The relevant literature is reviewed.

A 22-year-old male motorcyclist fell from a height and sustained an obturator hip dislocation with concomitant fracture of the ipsilateral greater trochanter. The immediate reduction of the dislocation associated with open reduction of the fracture of the greater trochanter showed excellent results at the short-term and long-term follow-up examinations. This extremely rare hip injury seems not to have been previously reported in the literature.

Transverse sacral fractures with cauda equina lesion appear only sporadically in the world literature. In our medical center 667 spine fractures were admitted during a twelve year period and we report only 2 cases of sacral fractures with neurological lesions (0,20%). The neurological abnormalities that occur are a perianal sensory loss with bladder and anal sphincters disturbance. Sphincters electromyographic studies and cystometric examination show a lower motor neuron lesion. In one of our cases, the nervous lesion was more spread with an unilateral sensory loss S1-S2 and a motor loss L4-L5

An analysis of plain X-ray films, tomography and myelography in 30 patients with proven cervical spinal cord injury, whose X-ray films were negative, has been performed. Features considered were the sagittal diameter of the spinal canal, associated cervical spondylosis, soft tissue changes, mobility studies and the value in cases of cervical injury of two recently described radiological signs. The only consistent feature in the majority of patients was a large posterior osteophytic spur in the lower cervical spine. No other radiological feature was found to be of particular value in diagnosis.

CASE: A 65-year-old man presented with an index finger dorsal combined saw injury. Examination showed exposed articular defect of the index finger proximal phalangeal base and extensor lag. The proximal phalangeal base articular defect was reconstructed using distal tibial osteochondral allograft and the tendon injury reconstructed with a turn-down interposition graft. The patient attained a painless functional arc of motion at 17 months postoperatively.
CONCLUSION: Proximal phalangeal base fracture-dislocations with partial-articular bone loss pose reconstructive challenges. We present a rar

PURPOSE: The purpose of this study was to compare the clinical outcomes, radiographs, and complications of radial head arthroplasty in patients with concomitant Monteggia or transolecranon fracture dislocations to patients with isolated radial head fractures.
METHODS: A retrospective cohort comparison was performed, identifying unreconstructible radial head fractures that were treated with radial head arthroplasty. Patients >18 years of age with at least 1 year of follow-up were included. Patients with Monteggia or transolecranon fracture dislocations were matched by age (within 5 years), sex…

BACKGROUND: Trans-scaphoid perilunate fracture-dislocation (TSPLFD) is an uncommon but high-risk injury in athletes, and acute median nerve symptoms can influence surgical timing and approach.
CASE PRESENTATION: We report two 20-year-old collegiate football players who sustained TSPLFD during play. In Case 1, closed reduction in the emergency department restored alignment, but the patient reported mild paresthesias in the median nerve distribution. He underwent open reduction and internal fixation (ORIF) of the scaphoid with lunotriquetral stabilization and carpal tunnel release. In Case 2, c

CASE: A 9-year-old boy presented with a severe median nerve palsy 6 months after a simple elbow dislocation. Neurotmesis was confirmed on surgical exploration, and it was repaired primarily in elbow flexion with the arm casted postoperatively. At 1-year follow-up from surgery, the patient reported near normal sensory improvement with motor deficits that did not affect his full return to activities.
CONCLUSION: The median nerve neurotmesis after elbow dislocation without an associated fracture is rare. This case highlights the importance of careful neurologic examination in children sustaining

OBJECTIVE: Dislocations of the elbow are not common in skeletally immature patients. Herein, we present a case report on a rare pediatric posterior trans-olecranon fracture dislocation of the elbow, a type of dislocation that has never been reported in children. We aim to discuss the injury mechanism and introduce our treatment approach for this specific condition.
PATIENT: A ten-year-old girl presented with pain, swelling and deformity of the left elbow following a fall from a rocking chair.
DIAGNOSES AND INTERVENTIONS: The complete injury history and detailed physical examination were reco…

CASE: A 32-year-old man sustained a rare and complex injury pattern representing an anterior glenohumeral fracture dislocation with critical intra-articular glenoid bone loss, and a double disruption of the superior shoulder suspensory complex (acromion + coracoid fractures), following a fall. To restore joint stability, surgical reconstruction of the glenoid fossa was performed, while fixation of the acromion and coracoid fractures was necessary to re-establish superior shoulder suspensory complex integrity.
CONCLUSION: Achieving structural stability facilitated fracture healing and rehabili…

We report an exceptionally rare and complex case of quadruple bilateral fracture dislocation of the elbow and the wrist in a 39-year-old construction worker, who sustained the injury after falling from a scaffold of 9 m secondary to a 6.9 magnitude earthquake. The patient was treated with a combination of closed and open reduction, internal fixation, and radial head arthroplasty, and had a good functional outcome at 12 months follow-up, with no complications or instability. This case illustrates the challenges and the possibilities of managing such complex injuries, and adds to the scarce lite…
