This retrospective study included 148 patients who were evaluated for postoperative long-term results after osteosynthesis of isolated zygoma fractures. The influence of open reposition of the zygomaticoalveolar crest to middle face symmetry was evaluated quantitatively with a zygometer. In dependence on operative management patients were divided into two groups: In group I zygomaticofrontal suture (1-point-fixation) and infra-orbital rim were exposed (2-point-fixation), followed by osteosynthesis. In group II additional exposure of zygomaticoalveolar crest was performed and if required osteos

Fractures of the distal end of the radius are common injuries and are the commonest bony injury around the wrist. Management of these fractures has remained controversial as far as modality of treatment is concerned. In this study 90 adult cases of acute displaced intra-articular fractures of the lower end of the radius were classified according to Frykman's and AO classifications after obtaining radiographs in antero-posterior and lateral planes. These were randomly treated by one of three methods: (1) closed reduction and plaster immobilisation, (2) external fixation and (3) open reduction a

In a 12-year period, 104 patients with intraarticular fractures of the distal humerus received surgical treatment. Based on the Arbeitsgemeinschaft für Osteosynthesefragen/Association for the Study of Internal Fixation (AO/ASIF) classification, there were 40 monocondylar fractures, 46 bicondylar fractures, and 18 fractures of the ulnar epicondyle with the fragment dislocated in the elbow joint. In children almost all fixations were done with Kirschner wires. The adult fractures were stabilized according to the recommendations of the AO/ASIF, i.e., monocondylar fractures by screws and/or single

The primary goal in operative fixation of forearm fractures is to restore length, rotational stability, and maintenance of the radial bow. Plate osteosynthesis is well regarded as the gold standard of treatment though often necessitates soft tissue injury, periosteal stripping, and risk of refracture after hardware removal. While intramedullary nails have been utilized for forearm fixation since the early 1900s, technological advancements including locked intramedullary nails have lead to improved outcomes in intramedullary nail forearm fixation. In select patients, intramedullary nail fixatio

CASE: Patellar fractures after anterior cruciate ligament (ACL) reconstruction with a bone-patellar-tendon-bone (BPTB) autograft are a rare complication with a reported incidence of 0.2% to 2.3%. Treatment has previously been nonoperative splinting, lag screws, or a tension-band construct. We present the case of a 14-year-old adolescent girl who suffered a comminuted patella fracture 4 weeks after an ACL reconstruction using a BPTB autograft who subsequently underwent successful operative fixation through a novel technique with the use of a maxillofacial plate and screw system.
CONCLUSION: A

Volar fixed angle fixation may be considered as the beginning of a new era in restoring wrist function to patients with dorsally displaced distal radius fractures even in the face of comminuted or osteopenic bone. A thorough understanding of the anatomy of the wrist is a prerequisite when volarly approaching dorsally displaced distal radius fractures. The demonstration of the device theoretical and practical advantages requires an appreciation of the basics of working length, principles of plate stability, and the effect of cantilever bending. Volar fixed angle fixation successfully improves w

Management of the severely comminuted subtrochanteric fracture of the femur has been extremely difficult due to problems in stabilization, failure of fixation, and/or failure of the device, as well as associated high morbidity and mortality. Twenty-three such fractures treated with the Association for the Study of Internal Fixation (ASIF) blade plate fixation are reviewed. There were 17 primary unions (0 to 6 months), two delayed unions (6 to 12 months), and two nonunions with device failure, requiring reoperation and bone grafting before healing. There were two deaths, but only one was due to

Elderly patients tend to fracture the patella because of a simple fall. In this group of patients restoration of primary functional level is crucial. Sixty-eight patients more than 65 years of age with patellar fractures, who were treated in our institution between January 1990 and December 1999 were evaluated. There were 43 females and 25 males. The ages ranged from 65 to 88 years (17 patients>80). We focus on the mechanism, which led to the fracture, the pre-morbid and post-morbid state of the patients and the various aspects of treatment in this group of patients. The majority (82%) had a s

CASE: An 86-year-old man underwent closed reduction and internal fixation of a comminuted intertrochanteric hip fracture with a cephalomedullary nail on a Hana table. Postoperatively, a displaced fracture subluxation of L1 was detected, resulting in complete loss of motor and sensory function below L1. Urgent posterior decompression and fusion from T10-L4 was performed. Diffuse idiopathic skeletal hyperostosis (DISH) was only diagnosed after the incident.
CONCLUSION: A diagnosis of DISH, with increased risk of occult fracture and/or iatrogenic spine injury during hip fixation, may be unknown

Gunshot injuries of the mandible can result in high rates of complications, especially in cases of bone loss. A fractured mandible accompanying a gunshot wound almost always has an external wound; the fracture is always compound and comminuted. Sometimes management of these injuries may require multiple surgical interventions. In this clinical report, treatment of a patient, who had a mandibular fracture due to a gunshot wound, is presented.A 52-year-old man with a mandibular fracture in the right mandibular body accompanying a gunshot wound was operated on. Owing to a nonunion, a second surge

Recent advances in external fixation hardware, frame application, and pin-site care have resulted in the evolution of external fixation as a safe and versatile technique. It provides skeletal stability, access to the site of injury, and allows early mobilization. It avoids the disadvantages of additional soft-tissue stripping and the foreign body introduction associated with internal fixation of open fractures. The utility of external fixation principles in open hand and wrist fractures is well established. Indications for external fixation in open fractures of the forearm, elbow, and arm are

Comminuted fractures involving the metacarpophalangeal joint are a real challenge for the hand surgeon due to the complexity of their management and the high frequency of functional sequelae caused by the incongruence of the joint surfaces and the injury of soft tissues requiring repair. Metacarpophalangeal joint arthroscopy is an emerging and useful tool for the upper extremity surgeon allowing assisted reduction and percutaneous fixation of these fractures since it permits a complete evaluation of the joint fragments and enables precision in the reduction with the advantage of being a minima

The optimal protocol for postoperative immobilization following operative treatment of scaphoid fractures remains controversial. Reports of successful management with brief postoperative immobilization suggest that earlier restoration of function may be achieved by limiting the duration of immobilization. However, the risk of nonunion and its associated complications suggest that a more conservative approach with extended immobilization could optimize fracture healing. This paper presents a thorough review of the relevant literature and summarizes the myriad postoperative immobilization protoc

Radial head fractures are often secondary to a direct axial force, such as that involved in motor vehicle accidents and falls on an outstretched hand. The Hotchkiss-modified Mason classification is an excellent assessment tool in that it provides commonly accepted direction regarding treatment. For more unstable, comminuted displaced radial head fractures that cannot be reconstructed, replacement of the radial head is warranted. The surgeon should attempt open reduction and internal fixation with restoration of the radial head in anatomical alignment for most type II and some type III fracture

We studied the arterial anatomy and the effect of four-part fractures on the vascularity of the humeral head, using barium sulphate perfusion of 16 cadaver shoulders. The main arterial supply to the humeral head was via the ascending branch of the anterior humeral circumflex artery and its intraosseous continuation, the arcuate artery. There were significant intraosseous anastomoses between the arcuate artery and: 1) the posterior humeral circumflex artery through vessels entering the posteromedial aspect of the proximal humerus; 2) metaphyseal vessels; and 3) the vessels of the greater and le

Since 1980 our treatment of displaced intra-articular calcaneal fractures consists of open reduction with distractor, bone grafting, and internal fixation. Thereby no splints are applied and early postoperative movement is possible. For precise preoperative planning a CT scan is required. Proper timing and careful preoperative planning are essential to prevent soft-tissue complications. In a total of 16 fractures good clinical and radiologic results were achieved in 50% of the cases; results were satisfactory in 25% and poor in 25%. The patients returned to work after an average of 5 months. O

BACKGROUND: Lateral locked plating remains an important treatment strategy for distal femoral fractures but has been associated with nonunion rates ranging from 6% to 20%. The objective of this study was to identify factors associated with nonunion following lateral locked plating of distal femoral fractures with use of a Bayesian analysis.
METHODS: All consecutive patients ≥18 years of age who were treated with lateral locked plating for a distal femoral fracture at 2 Level-I trauma centers between 2006 and 2024 and who had ≥3 months of follow-up were included. Multivariable Bayesian logisti

OBJECTIVE: Fracture classification evolves dynamically with new and enhanced imaging modalities. This paper aims to introduce a novel hypothesis of a sophisticated fracture classification system for the proximal femur trochanteric region (AO/OTA-31A) based on 3D-CT images and accommodate the clinical requirement of the worldwide outbreak of geriatric hip fractures with large amounts of surgical operations.
METHODS: In the current practice of widely preoperative 3D-CT application and cephalomedullary nailing, we attempt to propose a new comprehensive classification system to describe the fract

Bridge plate fixation has traditionally been described for the treatment of high-energy distal radius fractures with extensive comminution, associated instability, and polytrauma with the need for immediate upper extremity assisted weight bearing. Certain patient populations who may similarly benefit from such effective and expedient stabilization include patients with multiple comorbidities who may have lower-energy fractures, poor bone quality, and a baseline reliance on ambulatory assist devices. This article reviews treatment considerations for distal radius fractures in low-demand patient
