Osteology of the skull as it affects halo pin placement.

Twenty-seven cadaver skulls and 20 CT scans of skulls were measured above the orbital rim and ear but below the greatest diameter to determine optimal placement of halo pin sites. At the antero- and posterolateral portions of the calvaria, the outer table averaged 2 mm, the diploe 3 mm, and the inner table 2 mm. At the temporal fossa, the outer cortex averaged 1.7 mm, the diploe 2.0 mm, and the inner table 1.6 mm. Average distance between the anterior edge of the temporal fossa and the frontal sinus approximated 3 cm. These data confirm previously recommended halo pin insertion sites, anterola

Localized primary intracranial Ewing's sarcoma of the orbital roof. Case report.

A rare case of a primary Ewing's sarcoma arising from the intracranial portion of the right orbital roof is presented. Clinical presentation was that of a rapidly growing tender exostosis, associated with headaches. History indicated that the tumor may have been present for more than 1 year. Skull x-rays showed marked diastasis of the coronal suture and a "mottled" appearance on the right frontal bone suggestive of a sarcoma. Bone scan further confirmed the presence of a malignant bone tumor. Computerized tomography scan showed the intracranial extension of the tumor and a follow-up scan allow…

Arachnoid cyst of the frontal part of the temporal lobe producing exopthtalmos CT and MRI evaluation.

Twenty-three patients with an arachnoid cyst in the frontal part of the temporal bone, which produced exopthtalmos, were evaluated with CT and MRI. The majority of the patients had the peak of the bowing of the bony orbit wall in the posterior third (20/23) and only three in the middle. Arachnoid cyst are classified as small (13/23), medium (5/23) or large (5/20) size. Exopthtalmos are also classified as mild, moderate or large. CT and MRI clearly revealed the exopthtalmos, the bone remodelling process and the relationship of the orbit muscles, optic nerve and posterior bony wall of the orbit.

Acute (haematogenous) osteomyelitis of the skull in childhood.

A 13-year old boy presented with a 2-week history of right frontotemporal headache, fever and progressive painful swelling in that region. There was no preceding trauma. Clinical and radiological evaluation confirmed osteomyelitis of the right frontal bone. Staphylococcus aureus was found to be the causative organism. The affected bone was debrided and appropriate antibiotics administered. A subdural abscess developed requiring drainage. Hospital stay was 8 weeks. Haematogenous (pyogenic) osteomyelitis of the skull is rare in childhood and diagnosis may be missed or delayed. A high index of su…

Aneurysmal bone cyst of the orbit: a case report and review of literature.

Aneurysmal bone cyst is a benign fibroosseous lesion which rarely occurs in the orbit. We report on a 7-year-old girl with aneurysmal bone cyst of the orbit who presented with painless proptosis and diplopia. Optic nerve compression resulted in field loss and delayed visual evoked potentials. Radiological and histological features are discussed. The lesion was excised via a frontal craniotomy and the orbital roof reconstructed with a prefabricated titanium plate. Post-operatively a rapid resolution of the proptosis and diplopia followed. Previous reported cases of this rare entity in the orbit

[Development and growth of the vault of the skull].

The vault of the skull is a region of the neurocranium formed by a process of membranous ossification. It consists of several bones: frontal bone, parietal bone, squamous part of the temporal bone, lamina ascendens of the sphenoid, and interparietal bone. The embryological origin of the bones of the skull vault is still the subject of controversy. This can be explained by the different animal models used for these purposes, but also by the various techniques applied to this problem. At all events, it seems that the cells of the neural crest generate some of the bones of the vault and that the …

Lower-limb internal loading and potential consequences for fracture healing.

Introduction: Mechanical loading is known to determine the course of bone fracture healing. We hypothesise that lower limb long bone loading differs with knee flexion angle during walking and frontal knee alignment, which affects fracture healing success. Materials and methods: Using our musculoskeletal in silico modelling constrained against in vivo data from patients with instrumented knee implants allowed us to assess internal loads in femur and tibia. These internal forces were associated with the clinical outcome of fracture healing in a relevant cohort of 178 extra-articular femur and ti

Metatarsal Shape and Foot Type: A Geometric Morphometric Analysis.

Planus and cavus foot types have been associated with an increased risk of pain and disability. Improving our understanding of the geometric differences between bones in different foot types may provide insights into injury risk profiles and have implications for the design of musculoskeletal and finite-element models. In this study, we performed a geometric morphometric analysis on the geometry of metatarsal bones from 65 feet, segmented from computed tomography (CT) scans. These were categorized into four foot types: pes cavus, neutrally aligned, asymptomatic pes planus, and symptomatic pes

Evaluation of craniofacial surgery in the treatment of facial deformities.

Surgical access to the cranial, orbital, and facial areas, as developed by Tessier, has produced not only definitive repair of previously uncorrectable congenital deformities such as orbital hypertelorism and facial stenosis (e.g., Crouzon's, Apert's syndromes) but also has improved markedly the treatment of traumatic and neoplastic defects. The surgical approach allows complete dissection of facial soft tisses including the orbits from the underlying bones followed by corrective osteotomies and fixation. Mobilization of the frontal lobes through a frontal bone flap exposure may be required. T…

The subcranial approach to trauma of the anterior cranial base: preliminary report.

The subcranial approach to the cranio-orbito-frontal junction allows direct access to the central anterior cranial base for repair of fractures, dural tears, and cerebrospinal fluid fistulae. It provides good visualization without brain retraction and is suitable in primary or delayed traumatic cases. For extended visualization, a portion of the frontal sinus may be removed and repositioned at the end of the procedure. Because the brain is not retracted, morbidity is low. Potential and active cerebrospinal fluid fistulae can be successfully managed with the use of free fascial grafts and often

Feminization Surgery of the Upper Face as the Crucial Factor in Gender Confirmation-Pearls and Pitfalls.

Background: Upper-face feminization is a frequently executed procedure in sexual reassignment surgery, owing to its ability to influence gender identity through adjustments to the hairline, forehead, and peri-orbital area. The procedure includes reducing the hairline, lifting the brows, shaving the orbital region, and applying specific techniques to reduce the frontal bone. This research aims to assess the outcomes, results, and potential complications associated with this surgery among transgender patients. Material and Method: Retrospective review of medical records of 20 patients who attend…

Possible complications of frontal sinus fractures after conservative or surgical treatment: retrospective study.

BACKGROUND: Frontal sinus fractures are rare. Although they can manifest as isolated fractures, they mostly occur as a part of larger panfacial injuries of the facial bones. OBJECTIVE: Considering the relative rarity of this type of fracture, we decided to perform a retrospective analysis of patients with this type of injury at our institution, focusing on post-injury complications. MATERIAL AND METHODS: We retrospectively evaluated 26 patients with frontal sinus fracture (isolated or as a part of a complex injury to the facial skeleton) treated and subsequently followed up at our institutio

Small cell neuroendocrine carcinoma masquerading as cellulitis and causing blindness via bilateral orbital involvement.

CASE REPORT: We report the first case of small cell neuroendocrine carcinoma involving both orbits and causing complete blindness. The patient was initially prescribed antibiotics for right periorbital cellulitis, but magnetic resonance imaging demonstrated abnormal enhancing tissue in the superomedial and superior right orbit with destruction of the anterior lamina papyracea, extension into the anterior ethmoid air cells, further extension into the right frontal bone and pockets of fluid in the right frontal lobe. Histopathology from the orbital tissue and right frontal subdural empyema demon…

[Surgical anatomy of the so-called dangerous frontal sinus (Boenninghaus)].

Surgically dangerous type of frontal sinus was first reported by Boenninghaus in 1923. The present study concerns with the types and incidence of the dangerous frontal sinus as investigated by axial scans of HRCT. A total of 286 adult and 33 infant cases was included. The anterior end of the olfactory groove was first classified into three types, i.e., normal type, obliterated type (obliterated by cancellous bone) and dangerous type (with Recessus cristae galli and Torus olfactorius). In the adult, the incidences of these types are, 67.2, 18.5 and 14.3%. In the infant, incidences are 42.4, 51.

Posttraumatic frontal mucocele.

Mucoceles of the frontal sinus are expansive cysts filled of the mucus secreted by goblet cells interspersed in the ciliated mucosa. The aim of this article was to present a case of a frontal mucocele that was developed in a 31-year-old man 19 years after having had a frontal sinus fracture. This is a rare entity. Frontal sinus fractures must be treated by a multidisciplinary team to avoid all possible sequelae. An adequate primary management of frontal sinus fractures is essential to prevent complications. This may include conservative attitude, reduction and fixation with miniplates, obliter
