[AAI stimulation and nodal rhythm].

We describe the case of a 77 year-old white woman treated by AAI pacing. Nodal rhythm with ventriculo-atrial conduction and retrograde P wave were observed in ECG Holter monitoring. The retrograde P wave was sensed by the pacemaker, which responded correctly.

Ontogeny of canine intrathoracic cardiac nervous system.

The functional cardiac innervation of 61 puppies from nine different litters (2-8 littermates), ranging in age from 1 day to 7 wk, was investigated. The efferent sympathetic nervous system exerted minimal effects on the heart of 1-day-old puppies, gradually influencing the heart more thereafter such that by 7 wk of life it was functionally mature. In contrast, efferent parasympathetic cardiac innervation was well developed at birth, maturing thereafter such that by 4-7 wk of age its capacity to modulate the heart was similar to that found in adults. The right- and left-sided efferent sympathet

Cardiac arrhythmias, conduction system abnormalities, and autonomic tone in patients with brucellosis.

OBJECTIVE: Arrhythmias can occur because of cardiac involvement in patients with brucellosis. We investigated the effect of Brucella (Brucella melitensis) on cardiac arrhythmias in patients without pre-existing arrhythmogenic heart disease. PATIENTS AND METHODS: 205 patients with sinus rhythm who were diagnosed with brucellosis and 205 healthy controls were enrolled. ECG was performed, and the parameters were recorded for each patient. 24-hour ambulatory ECG (Holter) monitoring was conducted, and the recordings were analyzed. The time-domain heart rate variability (HRV) results were evaluated

Bisphenol A exposure and cardiac electrical conduction in excised rat hearts.

BACKGROUND: Bisphenol A (BPA) is used to produce polycarbonate plastics and epoxy resins that are widely used in everyday products, such as food and beverage containers, toys, and medical devices. Human biomonitoring studies have suggested that a large proportion of the population may be exposed to BPA. Recent epidemiological studies have reported correlations between increased urinary BPA concentrations and cardiovascular disease, yet the direct effects of BPA on the heart are unknown. OBJECTIVES: The goal of our study was to measure the effect of BPA (0.1-100 μM) on cardiac impulse propagat

[Intrauterine treatment of incomplete fetal heart block in a mother with Sjögren syndrome].

BACKGROUND: Isolated fetal heart block is considered as an immunological disorder in the majority of cases. Mothers of affected fetuses often suffer from connective tissue disease (Sjögren syndrome or Lupus erythematodes). All of them test positive for anti-SS-A (anti Ro) and/or anti-SS-B (anti La) antibodies. Once established, third-degree congenital heart block is permanent and often requires a pacemaker. CASE: We report on a pregnancy in a mother with Sjögren syndrome which was complicated by the development of incomplete fetal heart block, diagnosed by pulsed wave Doppler echocardiography

Electrocardiographic imaging of cardiac resynchronization therapy in heart failure: observation of variable electrophysiologic responses.

BACKGROUND: Cardiac resynchronization therapy (CRT) for congestive heart failure patients with delayed left ventricular (LV) conduction is clinically beneficial in approximately 70% of patients. Unresolved issues include patient selection, lead placement, and efficacy of LV pacing alone. Being an electrical approach, detailed electrical information during CRT is critical to resolving these issues. However, electrical data from patients have been limited because of the requirement for invasive mapping. OBJECTIVES: The purpose of this study was to provide observations and insights on the variab

Atrial escape-capture bigeminy in dominant atrial rhythm with 2:1 exit block.

A 27-year-old woman with atrial bigeminy is reported in whom long PP intervals alternate with short PP intervals. All P waves are negative in lead II and all PR intervals measure 0.12 s. In the 12-lead electrocardiogram, however, these P waves were definitely different in configuration from each other, and were divided into two groups. Namely, these negative P waves are divided into those of dominant atrial rhythm J1 with 2:1 exit block, and those of atrial escape J2. Long J1-J2 intervals alternate with short J2-J1 intervals. These electrocardiographic findings show the presence of atrial esca

The effects of cocaine on cardiac electrophysiology in conscious, unsedated dogs.

This study was performed to determine the cardiac electrophysiological effects of cocaine and specifically to determine the time course of these actions. Eighteen chronically instrumented conscious dogs were tested with i.v. cocaine at doses of 1 or 4 mg/kg. The following statistically significant changes were observed 1 min following the 4 mg/kg dose of cocaine: heart rate increased from 135 +/- 8 to 186 +/- 9 beats/min, QRS duration increased from 60 +/- 5 to 74 +/- 5 ms, corrected QT interval increased from 298 +/- 7 to 339 +/- 8 ms, intraatrial conduction time increased from 27 +/- 2 to 31

The conduction bundle at the atrioventricular junction. An anatomical study.

The variability in the topographical anatomy of the conduction bundle at the site of the atrioventricular junction has been studied in four normal human hearts. The junctional area has been removed en bloc and serially sectioned. The conduction bundle and adjacent structures such as the posterior limb of the trabecula septomarginalis and the membranous septum have been reconstructed based on calculations from the histological sections. The study reveals marked variability particularly in the extent of the posterior limb of the trabecula septomarginalis. In one instance, the muscle was almost t

[Mesothelioma of the atrioventricular node and sudden death].

An incomplete atrioventricular block of grade III was detected by electrocardiography in a 3.5-year-old boy one week before his sudden death. On autopsy, cardiomegaly involving a 47% increase in heart weight and a moderate adaptational endocardial fibroelastosis were noted, as was a nodular tumor sized 6 X 3 X 1.5 mm at the atrioventricular node. The tumor was concluded to be a microcystic mesothelioma that had been destructive to the node and had begun infiltrating the cardiac muscle.

Ablation therapy for patients with supraventricular tachycardia.

Over the past decade, a more complete understanding of the mechanisms of supraventricular tachycardia has made possible the development of surgical and catheter techniques designed to control these arrhythmias. Ablative techniques are generally used when the arrhythmia proves refractory to drug and/or antitachycardia pacing therapy or when lifelong drug therapy is believed to be less desirable than a potentially curative procedure. In this essay, we explore the known mechanisms for supraventricular arrhythmias as well as the rationale for the use of ablative techniques.

Time-dependent heart period and contractility responses to successive brief vagal stimuli.

The vagosympathetic trunks of anesthetized propranolized dogs were stimulated with brief electrical bursts, and heart period (P-P interval), atrioventricular conduction (P-R interval), and pressure generated on a balloon anchored in the atrium (AP) were measured. Two vagal stimulus bursts were given separated by an integral number of cardiac cycles but with each placed at the same relative time in the cardiac cycle. The time between stimulus bursts (T) varied from 2 to 60 s. For all cardiac variables over this time span, the peak response to the second stimulus (P2) was less than that of the f

[Spontaneous blocks by intra-atrial conduction disorders of reciprocating atrioventricular rates using a Kent's bundle].

Paroxysmal tachycardia in Type A Wolff-Parkinson-White syndrome was due to a reciprocating rhythm involving a left lateral Kent Bundle in the retrograde direction. Spontaneous interruption of the reentry resulted not from block in the normal or accessory atrioventricular pathways but from an intraatrial conduction defect: delay in conduction between the left and right atria on intraatrial reentry terminated the tachycardia.

De subitaneis mortibus. XV. Hereditary stenosis of the His bundle in Pug dogs.

Syncope and sudden death occurs in certain purebred Pug dogs which have been found to have intermittent sinus pauses and paroxysmal second degree heart block on electrocardiographic (ECG) study. We have established a colony of such dogs to study this problem and here report the results of histological examination of the cardiac conduction system in twenty-one of them. These include a dam which may be considered the proband, three of her offspring (two littermates) and three fetal pups in an unborn litter of one of these; three of the four adult dogs died suddenly and unexpectedly. Automated beat-wise arrhythmia diagnosis using modified U-net on extended electrocardiographic recordings with heterogeneous arrhythmia types.

Abnormality of the cardiac conduction system can induce arrhythmia - abnormal heart rhythm - that can frequently lead to other cardiac diseases and complications, and are sometimes life-threatening. These conduction system perturbations can manifest as morphological changes on the surface electrocardiographic (ECG) signal. Assessment of these morphological changes can be challenging and time-consuming, as ECG signal features are often low in amplitude and subtle. The main aim of this study is to develop an automated computer aided diagnostic (CAD) system that can expedite the process of arrhyt

Atrial premature beat bigeminy with slow pathway conduction causing regular RR intervals.

A 74-year-old female patient presented with atrial premature beat (APB) bigeminy. Sinus beats were conducted via the fast pathway (FP), while APBs were conducted via the slow pathway (SP). Two types of QRS complexes with different natures appeared alternately, yet the RR intervals remained regular, mimicking accelerated junctional rhythm complicated with interfering atrioventricular dissociation. This article explores the pathogenesis of this rare electrocardiographic phenomenon.

[Basics in the EP lab-how and when to pace?].

Electrical stimulation is a central tool in the evaluation and management of arrhythmias in the electrophysiological laboratory. Targeted atrial and ventricular pacing maneuvers allow induction, mechanistic characterization, and termination of arrhythmias. This provides important insights into the involved conduction pathways, anatomical location, and the underlying arrhythmogenic substrate, thereby forming the basis for a tailored ablation strategy. A structured approach even prior to the first stimulation maneuver establishes the foundation for successful diagnosis and therapy (ensuring stab

Electrophysiological Mechanisms of ST Segment and T-Wave Changes: Insights into Ischemia, Conduction Delay, Hypertrophy, and Electrolyte Disturbance.

ST-T abnormalities are commonly encountered in cardiology, cardiac surgical, critical care, and emergency settings. ST segment depression refers to displacement of the ST segment below the PR-segment (baseline), whereas ST segment elevation refers to displacement above this baseline. A downward displacement greater than 0.5 mm below the baseline or an upward displacement exceeding 0.1 mV in the limb leads or 0.2 mV in the precordial leads, measured 40 ms after the J point relative to the baseline, is considered significant. ST-T abnormalities can be primary or secondary. Primary ST-T changes a
