Is gastric aspiration needed for newborn management in delivery room?

AIM: Gastric aspiration is still applied in many centres during delivery room management of the newborn without any supporting evidence. We aimed to determine whether gastric aspiration affects vital signs, oxygenation, nutrition and short-term prognosis of the newborn. METHOD: A total of 310 eligible healthy term newborns, identified from a total of 1300 live births, were randomly allocated to receive either gastric aspiration or standard care. During the first 20min, SpO(2), heart rate, cyanosis and retraction scores were recorded once in a min; and blood pressure, respiration rate and neur

Non-contact determination of vital sign alterations in hypovolaemic states induced by massive haemorrhage: an experimental attempt to monitor the condition of injured persons behind barriers or under disaster rubble.

To assess a non-contact method to determine the physical alteration of human subjects confined behind a barrier or under disaster rubble, an experimental, non-contact monitoring system was tested on rabbits in a hypovolaemic state. New Zealand male rabbits behind a barrier were subjected to hypovolaemic shock induced by the withdrawal of arterial blood (2ml per 100g body weight). The hypovolaemic state was determined by linear discriminant analysis using non-contact-derived variables: heart rate X1 and respiratory rate X2. Sixteen rabbits were equally divided between the hypovolaemic and contr

Effect of graded exercise on nitric oxide in expired air in humans.

This study was performed to determine the influence of graded dynamic exercise and of voluntary hyperventilation on both the concentration ([NO]) and the amount of nitric oxide per unit time (VNO) in exhaled air. Young human subjects (n = 8) of varying fitness levels having peak O2 consumption (VO2) values ranging between 25.7 and 50.9 ml/min/kg were studied during graded levels of treadmill exercise. Expired [NO] determined by chemiluminescence was 26.3 +/- 6.7 SE parts per billion (ppb) at rest ranging between 11 and 66 ppb. Although variable, [NO] was maintained as work rate increased. VNO

[Development and disease in the newborn infant from the viewpoint of the thermodynamics of irreversible processes].

The newborn baby was considered thermodynamically as an open system in stationary state and its rate of entropy production determined. Three groups of newborns were studied: (1) healthy, premature newborns (34 weeks gestational age) at the age of 1, 7, 14, 21 and 28 days, (2) healthy, mature newborns (39 wks GA) and mature newborns with a hypoplastic-left-heart syndrome (39 wks GA), both in their first week of life. The entropy production rate was determined by using a diagnostic procedure, based on temperature measurements and a thermodynamic model. The entropy production rate (EPR) indicates

Effects of methotrexate on acute rejection and cardiac allograft vasculopathy in heart transplant recipients.

BACKGROUND: In heart transplant recipients methotrexate has been shown to reverse recurrent/persistent acute rejection refractory to intensified conventional immunosuppression. This study sought to determine whether methotrexate produces a sustained decline of heart allograft rejection rates and renders rejection rates of patients with a history of recurrent/persistent rejection similar to those of heart transplant recipients without such history. METHODS: Rejection, infection, and cardiac allograft vasculopathy were compared in 35 patients treated with methotrexate (12 +/- 9 mg/week for 34 +

Effect of Ivabradine on Endothelial Function in Patients with Stable Angina Pectoris: Assessment with the Endo-PAT 2000 Device.

INTRODUCTION: Ivabradine has opened up new possibilities for treating stable angina and chronic heart failure by lowering heart rate. Ivabradine lowers heart rate by selectively inhibiting the I f current in the sinoatrial node. This study aimed to determine whether the decrease in heart rate achieved with ivabradine was accompanied by hemodynamic changes that might lead to an enhancement of endothelial function. METHODS: Thirty patients with stable angina pectoris were included in the study. Ivabradine (5 mg bid) was added to the recommended standard treatment. Endothelial function was asses

Sacubitril/valsartan: from a large clinical trial to clinical practice.

: The Prospective Comparison of Angiotensin Receptor Antagonist and Neprilysin Inhibitor with Angiotensin-Converting Enzyme Inhibitor to Determine Impact on Global Mortality and Morbidity in Heart Failure Trial (PARADIGM-HF) has shown a reduction in the risk of death and heart failure hospitalizations with sacubitril/valsartan, compared with enalapril, in patients with heart failure and reduced ejection fraction. Guidelines now recommend the substitution of angiotensin-converting enzyme inhibitors or AT1 blockers with sacubitril/valsartan in patients with heart failure and reduced ejection fra

Congenital cardiovascular malformations (CCVM) and structural chromosome abnormalities: a report of 9 cases and literature review.

Nine cases of congenital cardiovascular malformations (CCVM) with associated unbalanced structural chromosomal abnormalities were ascertained in a population-based study of heart defects, constituting 0.4% of the 2,103 cases of CCVM in the Baltimore-Washington Infant Study (BWIS). This represents a four-fold increase over the general population rate. In an effort to determine possible phenotype/karyotype correlations, the literature was searched for cases with similar karyotypic abnormalities. This comparison of 223 literature cases of karyotypic abnormalities with nine similar cases ascertain

Predictive value of early diastolic mitral annular velocity and body mass index as markers of left ventricular relaxation: validation against invasive time constant of relaxation.

AIMS: Impaired left ventricular (LV) relaxation, often assessed noninvasively by reduced early diastolic mitral annular velocity (e'); however, it is age dependent. We re-investigated the relationship between e' and the invasively measured LV relaxation time constant (τ), while accounting for LV geometry, body mass index (BMI), blood pressure, heart rate, and age, and established HF risk markers. METHODS AND RESULTS: We analysed 419 patients with suspected coronary artery disease undergoing left heart catheterisation. The τ was measured using micromanometer recordings, LV volumes by ventricul

Perceptual distortions during simultaneous continuous monitoring of fetal and maternal heart rate in laboring patients.

OBJECTIVE: Our previous work examined the effect on assessment of fetal well-being when presenting twin fetal heart rate tracings (FHRT) simultaneously on screen. To further examine the potential impact of subconscious perceptual influences on fetal well-being, the current study presented singleton FHRT with and without simultaneous presentation of maternal heart rate in a within-subjects design. Tracings were taken from archival cases with either normal or abnormal Apgar scores to identify differential impact depending on final outcome. All tracings were evaluated for variability, acceleratio

Laboratory assessment of anti-thrombotic therapy in heart failure, atrial fibrillation and coronary artery disease: insights using thrombelastography and a micro-titre plate assay of thrombogenesis and fibrinolysis.

As heart failure, coronary artery disease and atrial fibrillation all bring a risk of thrombosis, anti-thrombotic therapy is recommended. Despite such treatment, major cardiovascular events such as myocardial infarction and stroke still occur, implying inadequate suppression of thrombus formation. Accordingly, identification of patients whose haemostasis remains unimpaired by treatment is valuable. We compared indices for assessing thrombogenesis and fibrinolysis by two different techniques in patients on different anti-thrombotic agents, i.e. aspirin or warfarin. We determined fibrin clot for

Diastolic mechanisms of impaired exercise tolerance in aortic valve disease.

In order to determine the significance of abnormalities of diastolic function in patients with left ventricular hypertrophy, exercise echocardiography to heart rates of 140 to 150 beats/min was performed in 18 normal subjects and 14 patients after aortic valve replacement. Simultaneous echo-, phono-, and electrocardiograms were recorded. Left ventricular cavity size was determined at end-diastole and end-systole. The timing of mitral valve opening and closure was measured, and hence left ventricular filling time derived, expressed either as ms/beat, or s/min when multiplied by heart rate. Isov

Chlorpromazine: cardiac arrhythmogenicity in the cat.

To determine the effect of chlorpromazine on ouabain-induced arrhythmia and death, dial-urethane anesthetized cats were pretreated with chlorpromazine (5, 10, 20, 30, 40, or 60 mg/kg, i.v.) and then administered ouabain (2 microgram/kg/min, i.v.). Blood pressure, heart rate and lead II electrocardiogram (ECG) were monitored. The dosages of ouabain necessary to induce premature ventricular contractions, ventricular tachycardia and death were determined. No significant correlation between the dose of chlorpromazine given and the dose of ouabain required to produce arrhythmia or death was found.
