Mental health interventions for patients with haematological malignancies.

BACKGROUND: Patients with haematological malignancies frequently experience significant psychological distress due to the complexity and chronic nature of their illness. Despite growing recognition of the mental health burden, an integrative synthesis of effective interventions remains limited. AIM: To identify, categorise and synthesise existing empirical studies on mental health interventions for individuals diagnosed with haematological malignancies. METHODS: Following Whittemore and Knafl's integrative review framework, a comprehensive literature search was conducted across databases and

Beyond maximum grade: the role of real-world digital health technologies to capture, predict, and manage treatment toxicity in haematological cancers.

Real-world data are essential for understanding treatment-related toxicities in haematology, but traditional analyses are limited by data access, sharing constraints, and reduced data granularity. Emerging digital health technologies (DHTs), such as electronic patient-reported outcomes and wearable devices, can be used to capture continuous, patient-generated data not reliably captured by current post-marketing toxicity assessments. Artificial intelligence, synthetic data, and digital twins can be used to predict and simulate toxicity at a much larger scale beyond the capacity of traditional m

The Heterogeneous Syndrome of Noninfectious Causes of Persistent Fever in Neutropenic Patients With Hematologic Malignancy: Another Opportunity for Stewardship?

Although occult fungal, viral, and multidrug-resistant bacterial infections can cause persistent fever in neutropenic patients with hematologic cancer, a variety of noninfectious entities should be considered on a case-by-case basis in the context of negative diagnostic workup for infection.

[GATA factor-related hematopoietic neoplasms].

GATA1, GATA2, and GATA3, collectively known as hematopoietic GATA factors, play a central role in the transcription factor network that governs hematopoietic homeostasis. Dysfunction of these factors leads to various hematopoietic disorders. Aberrant function of GATA1 factor, crucial in erythrocyte and megakaryocyte differentiation, not only causes anemia and thrombocytopenia, but also triggers erythroid leukemia and acute megakaryoblastic leukemia. Similarly, GATA2 factor expression is dynamic in the hematopoietic hierarchy, and dysfunction of GATA2 factor contributes not only to dysfunction

Oncogenic drivers dictate immune control of acute myeloid leukemia.

Acute myeloid leukemia (AML) is a genetically heterogeneous, aggressive hematological malignancy induced by distinct oncogenic driver mutations. The effect of specific AML oncogenes on immune activation or suppression is unclear. Here, we examine immune responses in genetically distinct models of AML and demonstrate that specific AML oncogenes dictate immunogenicity, the quality of immune response and immune escape through immunoediting. Specifically, expression of NrasG12D alone is sufficient to drive a potent anti-leukemia response through increased MHC Class II expression that can be overco

The 5th edition of the World Health Organization Classification of Haematolymphoid Tumours: Lymphoid Neoplasms.

We herein present an overview of the upcoming 5th edition of the World Health Organization Classification of Haematolymphoid Tumours focussing on lymphoid neoplasms. Myeloid and histiocytic neoplasms will be presented in a separate accompanying article. Besides listing the entities of the classification, we highlight and explain changes from the revised 4th edition. These include reorganization of entities by a hierarchical system as is adopted throughout the 5th edition of the WHO classification of tumours of all organ systems, modification of nomenclature for some entities, revision of diagn

Therapeutic potentials of melatonin in the treatment of lymphoma: A review of current evidence.

Hematological malignancies including leukemia, multiple myeloma, and lymphoma are known as leading causes of death around the world. Despite all developments in cancer management, current therapeutic methods are still relatively inefficient, leading to the heavy financial burdens for public health systems. Strategic attempts in clinical practice must be based on three serious goals including (1) increasing the efficacy of treatments and decreasing their side-effects; (2) decreasing financial price of treatments and related morbidity and mortality rates; and (3) improving life quality and survi

[Invasive aspergillosis in the patient with oncohematologic disease].

Invasive aspergillosis is the most common invasive fungal infection in patients with acute hematological malignancies or treated with hematopoietic stem cell transplantation due to the marked alteration of the physiological mechanisms of antifungal immunity that takes place in these situations. For this reason, antifungal prophylaxis has a relevant role in these patients. The introduction of new antifungal agents has motivated the updating of recommendations for prophylaxis and treatment in different guidelines. The objectives of this chapter are a brief review of the mechanisms of immunity ag

The 2016 WHO classification and diagnostic criteria for myeloproliferative neoplasms: document summary and in-depth discussion.

The new edition of the 2016 World Health Organization (WHO) classification system for tumors of the hematopoietic and lymphoid tissues was published in September 2017. Under the category of myeloproliferative neoplasms (MPNs), the revised document includes seven subcategories: chronic myeloid leukemia, chronic neutrophilic leukemia, polycythemia vera (PV), primary myelofibrosis (PMF), essential thrombocythemia (ET), chronic eosinophilic leukemia-not otherwise specified and MPN, unclassifiable (MPN-U); of note, mastocytosis is no longer classified under the MPN category. In the current review,

Imaging features of myeloproliferative neoplasms.

Myeloproliferative neoplasms (MPNs) are a heterogeneous group of haematological disorders including polycythaemia vera (PV), essential thrombocythaemia (ET), primary myelofibrosis (PMF), and chronic myeloid leukaemia (CML). These disorders show large overlap in genetic and clinical presentations, and can have many different imaging manifestations. Unusual thromboses, embolic events throughout the systemic or pulmonary vasculature, or osseous findings can often be clues to the underlying disease. There is limited literature about the imaging features of these disorders, and this may result in u

Purine nucleoside analogues for the treatment of hematological malignancies: pharmacology and clinical applications.

The purine nucleoside analogues (PNAs), fludarabine (FA), 2-CdA (2-chlorodeoxyadenosine, 2-CdA) and pentostatin (2'-deoxycoformycin, DCF) represent a group of cytotoxic agents with high activity in lymphoid and myeloid malignancies. PNAs share similar chemical structure and mechanism of action. Several mechanisms could be responsible for their cytotoxicity both in proliferating and quiescent cells, such as inhibition of DNA synthesis, inhibition of DNA repair and accumulation of DNA strand breaks. Induction of apoptosis through the mitochondrial pathway, direct binding to apoptosome or modulat

Acquired alpha-thalassemia in association with myelodysplastic syndrome and other hematologic malignancies.

Abnormalities of hemoglobin synthesis are usually inherited but may also arise as a secondary manifestation of another disease, most commonly hematologic neoplasia. Acquired hemoglobin disorders can be seen in any population and are not restricted to areas of the world with high incidences of inherited hemoglobinopathies. In fact, the acquired hemoglobinopathies may be more readily recognized where inherited hemoglobin abnormalities are rare and less likely to cause diagnostic confusion. Acquired alpha-thalassemia is the best characterized of the acquired red blood cell disorders in patients w

The association of thrombocytopenia with haematological malignancy.

Isolated thrombocytopenia is commonly diagnosed as immune thrombocytopenia (ITP). Recent publications have stimulated discussions regarding the diagnosis of ITP, including the potential for misclassification, the interaction between autoimmunity and clonality, and downstream complications of malignancy. This review focuses on these considerations in the workup and management of patients with thrombocytopenia.

The Role of Radiotherapy in Hematologic Malignancies in Children, Adolescents, and Young Adults.

Hematologic cancers in pediatric, adolescent, and young adult populations include a diverse spectrum of malignancies. The cornerstone of treatment is multiagent chemotherapy. While radiation therapy (RT) is highly effective and played a pivotal role historically, its use has evolved. In classic HL, advancements in systemic therapy have allowed for reduced RT volumes and doses and careful patient selection. Similarly, NLPHL management has shifted toward observation after complete resection, or limited chemotherapy after incomplete resection with RT used only for partially responding disease sit

Hematologic immune-related adverse effects of immune checkpoint inhibitors: a review.

Immune checkpoint inhibitors (ICIs) targeting CTLA-4, PD-1, and PD-L1 have transformed cancer therapy but may trigger rare, sometimes fatal hematologic immune-related adverse events (irAEs). The most frequent hematologic irAEs are anemia, thrombocytopenia, and pancytopenia, which occur more often with CTLA-4 inhibitors and in patients receiving combination therapy. Risk is heightened when ICIs are combined with chemotherapy or radiation. Common symptoms include fatigue, abnormal bleeding, easy bruising, and recurrent infections. Diagnosis relies on laboratory studies, histologic evaluation, an
