Reducing the rare event: lessons from the implementation of a ventilator bundle.

The ventilator-associated event (VAE) is a potentially avoidable complication of mechanical ventilation (MV) associated with poor outcomes. Although rare, VAEs and other nosocomial events are frequently targeted for quality improvement efforts consistent with the creed to 'do no harm'. In October 2016, VA Greater Los Angeles (GLA) was in the lowest-performing decile of VA medical centres on a composite measure of quality, owing to GLA's relatively high VAE rate. To decrease VAEs, we sought to reduce average MV duration of patients with acute respiratory failure to less than 3 days by 1 July 20

Pandemic flu and the sudden demand for ECMO resources: a mature trauma program can provide surge capacity in acute critical care crises.

BACKGROUND: Patients with severe H1N1 pneumonia created a sudden demand for extracorporeal membrane oxygenation (ECMO) capacity. In a single referral center, the established procedures, protocols, and staff of the Level I trauma service were adapted to help manage this nontrauma critical care crisis. METHODS: When airway pressure release ventilation and high-frequency oscillator ventilation failed, we used standard ECMO circuits and the VDR-4 critical care ventilator. We cannulated patients percutaneously in the intensive care unit and transported them on ECMO. Trauma service resources includ

Pediatric cardiac patients with pulmonary hemorrhage supported on ECMO: An ELSO registry study.

BACKGROUND: Pulmonary Hemorrhage (PH) is a rare but potentially devastating condition and pediatric cardiac patients are at increased risk for. ECMO may be used to safely support these patients, but data is limited. METHODS: Observational retrospective cohort study from the ELSO registry database in pediatric cardiac patients from birth to 18 years old with PH supported on ECMO from January 2011 through December 2020. The objectives of the study were to characterize pediatric cardiac patients with PH before ECMO and to describe factors associated with improved survival. RESULTS: A total of 1

Critical Care Dilemma: Complex Congenital Pulmonary Airway Malformation With Pulmonary Hypertension in a Neonate.

Congenital pulmonary airway malformations (CPAMs) are the most prevalent congenital lung lesions, yet their postnatal trajectory remains heterogeneous. Although many antenatally detected macrocystic lesions remain clinically silent, rapid postnatal enlargement can precipitate tension physiology, air-leak syndromes, and secondary pulmonary hypertension - events that mandate urgent departure from elective surgical timetables. We report a term female infant with a right-sided type 1 CPAM diagnosed by 20-week ultrasound. Despite an initial asymptomatic course, the lesion expanded abruptly at four

Prognostic implications of cluster-defined phenotypes in AECOPD patients with bronchiectasis: A multicenter study.

BACKGROUND: The clinical impact of bronchiectasis (BE) in acute exacerbations of COPD (AECOPD) remains controversial, with unclear phenotypic heterogeneity. RESEARCH QUESTION: Does BE independently influence clinical outcomes and phenotypic heterogeneity in AECOPD patients? STUDY DESIGN AND METHODS: This prospective multicenter cohort study analysed 11 759 hospitalised AECOPD patients from 10 Chinese medical centres. Propensity score matching (1:3) balanced baseline characteristics, and unsupervised cluster analysis identified phenotypic subgroups. Primary endpoints included mortality and ex

Cardiorespiratory action of opioid/tachykinin agonist peptide hybrid in anaesthetized rats: Transduction pathways.

AWL3106 composed of opioid (dermorphin) and tachykinin (substance P7-11) pharmacophores is a new compound with high analgesic potency and markedly reduced ability to induce tolerance and dependence. The present study aimed to determine the respiratory and cardiovascular responses evoked by this peptide in urethane-chloralose anaesthetized, spontaneously breathing rats in the presence or absence of vagal connection. Intravenous injection of AWL3106 at a dose of 0.3μmol/kg in intact rats resulted in apnoea lasting 5.1 ± 0.7s. Breathing that followed was of diminished frequency (F) and augmented

Neonatal management and outcome after thoracoamniotic shunt placement for fetal hydrothorax.

AIM: To evaluate the short-term neonatal outcome after fetal thoracoamniotic shunt placement for isolated hydrothorax. METHODS: Retrospecti … leural drainage. Twenty-one (53%) children required mechanical ventilation, of whom 13 (33%) needed high-frequency ventilation as rescue therapy. Overall 30/40 (75%) infants survived the neonatal period. Neonatal survival rate was significantly higher when infants were born ≥32 weeks' gestation as compared with <32 weeks: 93% (26/28) versus 33% (4/12), p<0.01. CONCLUSION: …

Contemporary treatment options for meconium aspiration syndrome.

The presence of meconium in the respiratory tract causes atelectasis, hypoxemia, hypercapnia, persistent pulmonary hypertension, inflammatory changes, and surfactant inactivation. Prevention is the most important factor in the management of meconium aspiration syndrome and it includes both prenatal and postnatal care. Prenatal procedures include constant fetal heart rate monitoring during labor, examination of acid-base equilibrium in the capillary blood from the fetal scalp, and eventually amnioinfusion. Amnioinfusion is not a widely accepted care and further studies to confirm its benefits a

Shifting from laparotomy to thoracoscopic repair of congenital diaphragmatic hernia in neonates: early experience.

BACKGROUND: Thoracoscopic repair of congenital diaphragmatic hernia (CDH) is now widely performed by pediatric surgeons. We compare our experience with this operation versus the results for a group of CDH patients treated by laparotomy at the same center over the same period. METHODS: From January 2008 to December 2011, we reviewed the medical records of 50 neonates who underwent surgery for posterolateral CDH (34 by laparotomy, 16 by thoracoscopy). Two thoracoscopic operations (12.5 %) were converted to thoracotomy. Eight patients treated with extracorporeal membrane oxygenation (ECMO) under

Neonatal procedural pain exposure and pain management in ventilated preterm infants during the first 14 days of life.

OBJECTIVES: Ventilated preterm infants are at high risk for procedural pain exposure. In Switzerland there is a lack of knowledge about the pain management in this highly vulnerable patient population. The aims of this study were to describe the type and frequency of procedures and to determine the amount of analgesia given to this patient group in two Swiss neonatal intensive care units. METHOD: A retrospective cohort study was performed examining procedural exposure and pain management of a convenience sample of 120 ventilated preterm infants (mean age = 29.7 weeks of gestation) during the

Hypercapnia as a Key Mechanism in the Development of Pulmonary Hypertension in Malignant Pertussis: A Case Report.

Malignant pertussis (MP) is a highly lethal complication of pertussis in young infants, typically associated with hyperleukocytosis, pulmonary hypertension (PHT), and multiorgan failure. Some authors suggest that structural pulmonary vascular damage explains the irreversibility and poor prognosis. In contrast, we present a well-documented case where sustained control of hypercapnia improved PHT. This case prompted a literature review. A 44-day-old male infant was admitted to our paediatric intensive care unit with confirmed pertussis and severe hypoxaemia. He required immediate intubation. The

Case Report: Respiratory outcome in a preterm infant following previable rupture of membranes and persistent oligohydramnios.

Mid-trimester preterm premature rupture of membranes is a rare complication of pregnancy associated with significant maternal and fetal risks. The ensuing prolonged oligohydramnios can lead to fetal pulmonary hypoplasia. In addition, there is an increased risk of miscarriage, preterm birth, and chorioamnionitis, contributing to septic morbidity in the mother-baby dyad. This case report describes the management and outcomes of an infant born at 32 weeks following the rupture of membranes at 16 weeks of gestation, resulting in severe oligohydramnios. Soon after birth, the infant had respiratory

Pulmonary artery size as an indication for thoracoscopic repair of congenital diaphragmatic hernia in neonates.

PURPOSE: We reviewed 24 consecutive cases of prenatally or immediately postnatally diagnosed left-sided congenital diaphragmatic hernia (CDH) to evaluate pulmonary artery (PA) size as an indication for thoracoscopic repair (TR). METHODS: CDH repair is planned once echocardiography confirms improvement in pulmonary hypertension. TR is chosen if cardiopulmonary status is stable more than 10 min in the decubitus position in the neonatal intensive care unit (NICU) under conventional mechanical or high frequency oscillatory ventilation (HFOV) with/without nitric oxide (NO) and the patient appears

Minimally Invasive Transverse Aortic Constriction in Mice.

Minimally invasive transverse aortic constriction (MTAC) is a more desirable method for the constriction of the transverse aorta in mice than standard open-chest transverse aortic constriction (TAC). Although transverse aortic constriction is a highly functional method for the induction of high pressure in the left ventricle, it is a more difficult and lengthy procedure due to its use of artificial ventilation with tracheal intubation. TAC is oftentimes also less survivable, as the newer method, MTAC, neither requires the cutting of the ribs and intercostal muscles nor tracheal intubation with

Treatment of smoke inhalation in five horses.

Five horses were admitted for treatment of smoke-inhalation injuries sustained in a barn fire. Three of the horses were mildly affected, with high respiratory rates (24 to 36 breaths/min) and normal to low arterial oxygen tensions (77.0 to 94.1 mm of Hg), and responded well to administration of diuretics, bronchodilators, corticosteroids, and antibiotics. The 2 remaining horses were severely affected. Both were in respiratory distress, with markedly low arterial oxygen tensions (50.4 and 57.1 mm of Hg) and cyanosis. These 2 horses required fluid resuscitation in addition to the treatments give

The use of dipyridamole to wean from inhaled nitric oxide in congenital diaphragmatic hernia.

A full-term neonate with a left-sided congenital diaphragmatic hernia (CDH) was ventilated mechanically by high-frequency oscillatory ventilation (HFOV). Despite inhaled nitric oxide (iNO) at a dose of 15 ppm, the neonate had severe respiratory acidosis and was placed on extracorporeal membrane oxygenation (ECMO) for 2 days. On day 7 of life, surgical repair of the CDH was performed. After the intervention, iNO (20 ppm) had to be restarted because of severe pulmonary hypertension (PHT). Ventilatory support and iNO then were weaned progressively. However, each daily attempt to discontinue iNO (
