The best treatment for diseases of the hip can begin if one diagnoses the disorder as soon as possible after onset. In many cases, successful management of the disease will prevent permanent damage to the hip joint. If injury occurs to the developing hip joint, major hip surgery may be necessary in adult life.

Diagnostic and operative arthroscopies of the hip joint have been performed from an anterior approach after extension of the joint. The force needed to achieve a sufficient visualization of the hip joint was studied. In an anesthetized patient 300 Newtons (N) to 500 N was required, whereas up to 900 N was needed in an unanesthetized subject to achieve sufficient joint extension. An apparently congenital antero-superior hip dislocation is identified during surgery. With reference to this case, three possible clinical courses for congenital hip dislocations are outlined: As a rule full recovery is most frequent. During the 2-year period 1978-1979, a total of 2109 hip fractures (of the proximal end of the femur) occurred in Oslo. The age- and sex-specific annual incidence was the highest ever reported. A previous hip fracture had occurred in 13 per cent of the women and 6.8 per cent of the men. In 1979, a total of 5920 hip fractures was reported in Norway. Compared with Oslo, all other counties had a lower incidence. The number of fractures in Oslo was five times greater in 1982 compared with 1950. This increase cannot be explained only by the increasing number of elderly persons.

The moulded baby syndrome comprises: head moulding (plagiocephaly); pelvic obliquity with unilateral loss of hip abduction in flexion; and occasionally scoliosis, torticollis and bat ears. The hips, however, are radiologically normal and do not require the treatment used in the management of congenital dislocation or dysplasia. A review of 67 hips confirms this finding.

Subchondral cysts can mimic the radiographic features of intra-osseous ganglia or a variety of neoplastic processes. This case demonstrates the development of a massive subarticular cyst in the supra-acetabular bone. The unique pathogenesis of subchondral cysts is illustrated by the evolution of radiographic features that may be useful in planning treatment.

A patient is reported who developed a fistula between the hip and the caecum 39 years after arthrodesis of her hip. She presented with a painful right hip and radiographs showed that the Smith-Petersen nail used for arthrodesis had moved up through the acetabulum and into the pelvic cavity. The nail was removed but within a week a fistula which discharged alimentary contents had developed between the hip and the caecum. The patient was treated conservatively, and three weeks later the fistula had closed.

Using real-time high-resolution sonography, it is possible to directly visualize the cartilaginous femoral head and define its relation with the acetabulum in normal and pathologic states. Thirty-six examinations were performed on 33 infants, newborn to 1 year of age. Two transverse views were obtained through the femoral head and acetabulum. The indications for and results of operations for arthrodesis of the hip undertaken between 1975 and 1980 have been compared with a similar series published from the same department in 1964. The development and reliability of total replacement of the hip has reduced the need for arthrodesis of the joint, with its attendant problems of long term overload of the back and knee. However arthrodesis is still a valuable salvage procedure after immobilisation of the muscles which move the joint, often as a result of multiple operations, or in the presence of troublesome infection.

Thirty six children with transient synovitis of the hip had a total of 80 recurrences, 69 of them personally observed, and 11 described by the mother. The authors have reviewed 93 children with myelomeningocele and with hip involvement. Some had been operated upon and others had not. The results of surgery were often disappointing when the patient was evaluated in adolescence. A number of stiff hips and recurrences of dislocation were seen after tendon transplants, open reduction or osteotomies. Considering these results and the major non-orthopaedic problems of such patients, it is concluded that bilateral dislocations should not be operated on and that early treatment should be applied in unilateral cases mainly by osteotomy.
