Gut mucosal barrier: the frontline of bidirectional regulation of the gut-X axis.

While maintaining absorption and physiological exchange, the gut mucosal barrier prevents microorganisms and their products from freely entering tissues and blood vessels. Therefore, its functional state can affect signals transmitted to distal organs. This review defines the gut-X axis as bidirectional communication between the intestine and extraintestinal systems, and points the intestinal barrier as a selective gatekeeper that regulates thresholds and amplifies gut-derived information flow. We summarize barrier organization, the renewal and repair mechanism to maintain the epithelial integ

The Gut Microbiome in Pouchitis: A Narrative Review of Pathogenesis and Therapeutic Modulation.

Pouchitis is the most common long-term complication following restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC), with a cumulative prevalence of up to 50% within the first two years after IPAA surgery. Accumulating evidence implicates gut microbial dysbiosis in the pathogenesis of pouch inflammation, supported by characteristic microbial alterations and the clinical responsiveness of pouchitis to microbiota-directed therapies such as antibiotics and probiotics. This narrative review synthesizes evidence mainly published in the last decade on micro

Beyond the Viroporin: The Envelope Protein as a Therapeutic Target in Viral Immunomodulation.

In their comprehensive review, Shekhar et al. (Rev Med Virol. 2025; 35:e70067) provide a valuable synthesis of the viral envelope (E) protein's role in subverting innate immunity and its potential as a pharmacological target. While we commend this timely work, our letter offers a critical perspective on the translational challenges and complexities that warrant deeper consideration. We argue that the context-dependent functionality of the E protein-wherein its immunomodulatory effects vary with infection stage, cell type, and viral load-poses a significant hurdle for therapeutic intervention,

Deriving consensus sepsis clusters via goal-directed subgroup identification in multi-omics study.

Sepsis, a syndrome of life-threatening organ dysfunction caused by dysregulated host responses to infection, exhibits profound pathobiological heterogeneity, hindering the development of effective therapies. Current subtyping approaches, often reliant on single-omics data or unsupervised clustering, yield poorly reproducible and therapeutically misaligned classifications. Here, we introduce a goal-directed subgroup identification (GD-SI) framework that optimizes patient stratification for differential treatment responses, integrating longitudinal multi-omics data (transcriptomic, proteomic, me

Targeting of canonical WNT signaling ameliorates experimental sclerodermatous chronic graft-versus-host disease.

Chronic graft-versus-host disease (cGVHD) is a major life-threatening complication of allogeneic hematopoietic stem cell transplantation. The molecular mechanisms underlying cGVHD remain poorly understood, and targeted therapies for clinical use are not well established. Here, we examined the role of the canonical WNT pathway in sclerodermatous cGVHD (sclGVHD). WNT signaling was activated in human sclGVHD with increased nuclear accumulation of the transcription factor β-catenin and a WNT-biased gene expression signature in lesional skin. Treatment with the highly selective tankryase inhibitor

Pyrimidine synthesis inhibition enhances cutaneous defenses against antibiotic resistant bacteria through activation of NOD2 signaling.

Multidrug-resistant bacterial strains are a rapidly emerging healthcare threat; therefore it is critical to develop new therapies to combat these organisms. Prior antibacterial strategies directly target pathogen growth or viability. Host-directed strategies to increase antimicrobial defenses may be an effective alternative to antibiotics and reduce development of resistant strains. In this study, we demonstrated the efficacy of a pyrimidine synthesis inhibitor, N-phosphonacetyl-L-aspartate (PALA), to enhance clearance of methicillin-resistant Staphylococcus aureus (MRSA), Pseudomonas aerugino

Impact of Yuehua Decoction on quality of life and safety in multidrug-resistant tuberculosis: a prospective cohort study using multivariable regression analysis.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) remains a major global health challenge, and prolonged chemotherapy often leads to substantial symptom burden and impaired health-related quality of life (HRQoL). Host-directed adjunctive therapies, including Traditional Chinese Medicine, have received growing interest; however, prospective clinical evidence on Yuehua Decoction remains limited. This study evaluated whether adjunctive Yuehua Decoction improves HRQoL and safety among patients receiving standardized MDR-TB treatment. METHODS: We conducted a prospective, non-randomized cohort

Current use of pre-clinical models for evaluation of human adenovirus-based cancer therapeutics.

Human adenoviruses (HAdVs) are enjoying a renaissance in interest for the development of cancer therapies following the approval of Adstiladrin (Ferring Pharmaceuticals) for the treatment of bladder cancer, strong phase 3 data from CG Oncology's cretostimogene grenadenorepvec, and many other products in late-stage clinical development. Most therapeutic oncolytic HAdV (oHAdV)-based vectors are engineered to enhance their natural selectivity for human cancer cells and/or to alter their capsid proteins to bias their tropism toward cancer tissues. Furthermore, modern therapeutic oHAdV-based vector

The emerging role of neutrophil extracellular traps in liver diseases: a narrative review.

Neutrophil extracellular traps (NETs) are web-like chromatin structures released by neutrophils that play dual roles in host defense and tissue injury. In liver diseases, NETs have emerged as key pathogenic mediators. This narrative review synthesizes current evidence on the involvement of NETs in alcohol-associated liver disease (ALD), metabolic dysfunction-associated steatotic liver disease (MASLD), viral hepatitis, hepatic ischemia-reperfusion injury (HIRI), hepatocellular carcinoma (HCC), and intrahepatic cholangiocarcinoma (iCCA). Mechanistically, NETs amplify sterile inflammation, promot

Staphylococcus aureus: A Review of the Pathogenesis and Virulence Mechanisms.

Staphylococcus aureus is a formidable human pathogen responsible for infections ranging from superficial skin lesions to life-threatening systemic diseases. This review synthesizes current knowledge on its pathogenesis, emphasizing colonization dynamics, virulence mechanisms, biofilm formation, and antibiotic resistance. By analyzing studies from PubMed, Scopus, and Web of Science, we highlight the pathogen's adaptability, driven by surface adhesins (e.g., ClfB, SasG), secreted toxins (e.g., PVL, TSST-1), and metabolic flexibility in iron acquisition and amino acid utilization. Nasal, skin, an

Pathogenesis of Perianal Fistulising Crohn's Disease: Current Knowledge, Gaps in Understanding, and Future Research Directions.

Fistulising perianal involvement is a common presentation of Crohn's disease. Perianal Crohn's disease dramatically impairs patients' social and sexual life and its management remains a critical challenge in field of inflammatory bowel disease. No medical therapy is specifically designed for the treatment of perianal Crohn's disease, largely due to the poorly understood pathophysiology. Limited evidence from preclinical and clinical studies suggests that the initiation, progression, and maintenance of Crohn's disease-associated perianal fistulas involves complex interactions between host, micr

Entry Inhibitors of Hepatitis C Virus.

Hepatitis C virus (HCV) infection affects approximately 1% of the world's population and is a major cause of chronic liver diseases. Although antiviral therapy consisting of direct-acting antivirals (DAAs) can cure the majority of HCV patients, it is still limited by viral resistances, drug-drug interactions, and high costs. Moreover, the role of DAAs in the prevention of occurrences of graft reinfection in HCV patients who receive liver transplantations is still under comprehensive clinical investigation, bringing the risk of recipient reinfection. HCV entry is composed of initial non-specifi

Granzyme B Inhibition by Tofacitinib Blocks the Pathology Induced by CD8 T Cells in Cutaneous Leishmaniasis.

In cutaneous leishmaniasis, the immune response is not only protective but also mediates immunopathology. We previously found that cytolytic CD8 T cells promote inflammatory responses that are difficult to treat with conventional therapies that target the parasite. Therefore, we hypothesized that inhibiting CD8 T-cell cytotoxicity would reduce disease severity in patients. IL-15 is a potential target for such a treatment because it is highly expressed in human patients with cutaneous leishmaniasis lesions and promotes granzyme B‒dependent CD8 T-cell cytotoxicity. Here we tested whether tofacit

Early diagnosis of invasive mould infections and disease.

Invasive mould infections (IMIs), such as invasive aspergillosis or mucormycosis, are a major cause of death in patients with haematological cancer and in patients receiving long-term immunosuppressive therapy. Early diagnosis and prompt initiation of antifungal therapy are crucial steps in the management of patients with IMI. The diagnosis of IMI remains a major challenge, with an increased spectrum of fungal pathogens and a diversity of clinical and radiological presentations within the expanding spectrum of immunocompromised hosts. Diagnosis is difficult to establish and is expressed on a s

Autoimmune dermatologic toxicities from immune checkpoint blockade with anti-PD-1 antibody therapy: a report on bullous skin eruptions.

Monoclonal antibodies against the immune checkpoint programmed cell death receptor 1 (PD-1) improve the hosts' antitumor immune response and have showed tremendous promise in the treatment of advanced solid tumors and hematologic malignancies. Reports of serious autoimmune dermatologic toxicities from immune checkpoint blockade therapy, however, are emerging. We report our experience with five patients who presented with pruritic vesicles and blisters on the skin while treated with anti-PD-1 antibody immunotherapy with either nivolumab or pembrolizumab. Four of the patients' skin biopsies reve

Acute viral encephalitis.

Acute viral encephalitis is a rare but devastating complication of common virus infections. Primary viral encephalitis is due to brain invasion by the virus and subsequent replication in the central nervous system, whereas postinfectious encephalitis is thought to be immune-mediated. The most frequent causes of primary encephalitis worldwide are rabies and the arboviruses, which comprise different families of viruses distributed according to geography and seasonality. Herpes simplex virus (HSV) is the commonest cause of encephalitis in Western countries. Herpes simplex encephalitis (HSE) occur

Recombinant interferon-beta therapy and neuromuscular disorders.

Interferon-beta (IFNbeta) is an extra-cellular protein mediator of host defense and homeostasis. IFNbeta has well-established direct antiviral, antiproliferative and immunomodulatory properties. Recombinant IFNbeta is approved for the treatment of relapsing-remitting multiple sclerosis. The immunomodulatory effects of IFNbeta administration failed to demonstrate consistent benefit during treatment of various autoimmune neuromuscular diseases. Existing studies were flawed due to the often uncontrolled and unblinded nature of protocols, small patient numbers per study, the undetermined optimum d
