The predictive value of external continuous lumbar drainage, with cerebrospinal fluid outflow controlled by medium pressure valve, in normal pressure hydrocephalus.

BACKGROUND: Although sporadic studies have described temporary external cerebrospinal fluid (CSF) lumbar drainage as a highly accurate test for predicting the outcome after ventricular shunting in normal pressure hydrocephalus (NPH) patients, a more recent study reports that the positive predictive value of external lumbar drainage (ELD) is high but the negative predictive value is deceptively low. Therefore, we conducted a prospective study in order to evaluate the predictive value of a continuous ELD, with CSF outflow controlled by medium pressure valve, in NPH patients. METHOD: Twenty-seve

Parkinsonism in Idiopathic Normal Pressure Hydrocephalus or Hydrocephalic Presentation in Parkinson's Disease: A Report of Three Cases.

Idiopathic normal pressure hydrocephalus (iNPH) occasionally presents with parkinsonism, in addition to its classic triad of symptoms. However, it remains unclear whether such parkinsonism reflects an inherent manifestation of iNPH or comorbid Parkinson's disease, and whether it is levodopa-responsive. We report three patients exhibiting both hydrocephalus and parkinsonism. Two improved with both levodopa and shunt surgery; one improved only with levodopa and not with spinal tap. In one dual responder, dopamine transporter uptake improved postoperatively, and motor symptoms remained stable des

Pupillary responses to the glare illusion in normal pressure hydrocephalus: insights into network dysfunction and neurodegenerative comorbidities.

BACKGROUND: The glare illusion, in which a central area appears self-luminous due to surrounding luminance gradients, induces pupillary constriction, suggesting involvement of higher-order visual processing beyond the brainstem-mediated light reflex. However, its neural underpinnings remain unclear. This study investigated pupillary responses to glare and control stimuli in patients with normal pressure hydrocephalus (NPH), characterized by predominant white matter pathology, and healthy controls. We also examined whether pupillary responses might be influenced by potential comorbid neurodegen

Efficacy and Safety of CSF Shunting for Idiopathic Normal Pressure Hydrocephalus: A Systematic Review and Meta-Analysis.

Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological condition characterized by gait disturbance, cognitive impairment, and urinary dysfunction. CSF shunting remains the standard treatment; however, reported efficacy and complication rates vary across studies. This systematic review and meta-analysis aimed to evaluate the effectiveness and safety of CSF shunting in patients with iNPH based exclusively on evidence from randomized controlled trials (RCTs). The review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Ana

Brain and intracranial volumes are both enlarged and serve as potential risk factors in normal pressure hydrocephalus.

Normal pressure hydrocephalus (NPH) is a poorly understood neurodegenerative condition leading to gait impairment and ultimately dementia. Prior work has shown larger intracranial volume (ICV) among those with NPH which has been taken to establish a link to Benign external hydrocephalus of infancy (BEH) as a predisposing factor. These studies have not evaluated brain volume which we hypothesize will also be elevated in NPH and account for the increase in ICV. Automated analysis was performed on CT head examinations from 305 NPH patients and 294 controls. Brain volume was ~ 4.8% larger in femal

Effects of Exercise on Gait and Functional Performance in Individuals With Idiopathic Normal Pressure Hydrocephalus: A Scoping Review.

BACKGROUND AND PURPOSE: Exercise is recommended for individuals with idiopathic normal pressure hydrocephalus (iNPH), but its effects on gait and functional performance remain underexplored. This scoping review aimed to search (1) effects of exercise or physical therapy on gait and functional outcomes in individuals with iNPH, along with its underlying physiological and biomechanical mechanisms; (2) challenges to exercise implementation; and (3) replication potential of interventions. METHODS: Five databases (PubMed, Embase, Latin American and Caribbean Health Sciences Literature, Cumulative

White matter lesions associated with the reemergence of grasp reflexes in patients with idiopathic normal pressure hydrocephalus.

BACKGROUND: The white matter damage inducing the reemergence of grasp reflexes and their potential lateralization remains unstudied. Idiopathic normal pressure hydrocephalus (iNPH), a subcortical dementia, is an ideal model for these investigations. We aim to understand the contributions of white matter to the inhibition of grasp reflexes in patients with iNPH. METHODS: A total of 48 patients (mean age at admission: 77.8 ± 5.2 years; 56% male) with probable iNPH were retrospectively enrolled in this study. The intensity of grasp reflexes was assessed using a four-category classification. A vo

Idiopathic normal pressure hydrocephalus: Validation of the DESH score as a prognostic tool for shunt surgery response.

OBJECTIVE: Several radiological markers have been linked to clinical improvement after shunt surgery for idiopathic normal pressure hydrocephalus (iNPH). However, iNPH has no pathognomonic feature, and patients are still diagnosed as probable, possible, or unlikely cases based on clinical symptoms, imaging findings, and invasive supplementary tests. The predictive value of the disproportionately enlarged subarachnoid space hydrocephalus (DESH) score is not yet conclusively determined, but it might offer a more accurate diagnostic method. The aim of the present retrospective cohort study was to

Predictive value of spinal CSF volume in the preoperative assessment of patients with idiopathic normal-pressure hydrocephalus.

INTRODUCTION: The pathophysiology, diagnosis, and management of idiopathic normal pressure hydrocephalus (iNPH) remain unclear. Although some prognostic tests recommended in iNPH guidelines should have high sensitivity and high predictive value, there is often no positive clinical response to surgical treatment. MATERIALS AND METHODS: In our study, 19 patients with clinical and neuroradiological signs of iNPH were selected for preoperative evaluation and possible further surgical treatment according to the guidelines. MR volumetry of the intracranial and spinal space was performed. Patients w

CWH43 Variants Are Associated With Disease Risk and Clinical Phenotypic Measures in Patients With Normal Pressure Hydrocephalus.

BACKGROUND AND OBJECTIVES: Variants in the CWH43 gene have been associated with normal pressure hydrocephalus (NPH). We aimed to replicate these findings, identify additional CWH43 variants, and further define the clinical phenotype associated with CWH43 variants. METHODS: We determined the prevalence of CWH43 variants by whole-genome sequencing (WGS) in 94 patients with NPH. The odds of having CWH43 variant carriers develop NPH were determined through comparison with 532 Mayo Clinic Biobank volunteers without a history of NPH. For patients with NPH, we documented the head circumference, prev

The diagnostic value of the pulsatility curve to predict shunt responsiveness in patients with idiopathic normal pressure hydrocephalus.

OBJECTIVE: The aim of this study was to investigate the diagnostic accuracy of the pulsatility curve to predict shunt response in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: Lumbar cerebrospinal fluid dynamics were derived from an automatic lumbar infusion test (LIT) protocol. All patients were treated with ventriculoperitoneal shunting and re-examined 6 months after shunting. Patient demographics and outcomes were gathered in a prospective, electronic database that spanned from January 2012 to January 2020. A validated iNPH scale was used to assess patients preope

Clinical comorbidities correlated with a response to the cerebrospinal fluid tap test in idiopathic normal-pressure hydrocephalus.

AIMS: The mainstay of treatment for idiopathic normal-pressure hydrocephalus (iNPH) is spinal fluid shunting. A tap test (TT) is recommended as an indication of shunting. Patients with iNPH are often elderly and have multiple comorbidities affecting the shunting outcome. We investigated the factors affecting TT in patients with iNPH. METHODS: Seventy-five patients with iNPH were admitted to our department for a TT from April 2010 to May 2021. The patients were divided into a responsive group and an unresponsive group according to the clinical outcomes after TT on the Timed Up and Go Test (TUG

The effect of shunt surgery on neuropsychological performance in normal pressure hydrocephalus: a systematic review and meta-analysis.

We conducted a systematic review of the literature and used meta-analytic techniques to evaluate the impact of shunt surgery on neuropsychological performance in patients with normal pressure hydrocephalus (NPH). Twenty-three studies with 1059 patients were identified for review using PubMed, Web of Science, Google scholar and manual searching. Inclusion criteria were prospective, within-subject investigations of cognitive outcome using neuropsychological assessment before and after shunt surgery in patients with NPH. There were statistically significant effects of shunt surgery on cognition (

[The Current Status of Medical Care for Idiopathic Normal-pressure Hydrocephalus in Medical Centers for Dementia in Japan].

We used a questionnaire to evaluate how patients with idiopathic normal-pressure hydrocephalus (iNPH) are examined in Medical Centers for Dementia (MCDs), which are qualified medical organizations for dementia patients in Japan. Ninety-eight of 205 MCDs responded to our questionnaire. Accordingly, more than 590 patients with iNPH underwent an MCD examination during the period of 1 year. In regards to the Guidelines for Management of iNPH, 40 of the 98 MCDs used neither the first nor second edition of the Guidelines, even though up-to-date information is available in the second edition of the G

Pulse wave myelopathy: An update of an hypothesis highlighting the similarities between syringomyelia and normal pressure hydrocephalus.

Most hypotheses trying to explain the pathophysiology of idiopathic syringomyelia involve mechanisms whereby CSF is pumped against a pressure gradient, from the subarachnoid space into the cord parenchyma. On review, these theories have universally failed to explain the disease process. A few papers have suggested that the syrinx fluid may originate from the cord capillary bed itself. However, in these papers, the fluid is said to accumulate due to impaired fluid drainage out of the cord. Again, there is little evidence to substantiate this. This proffered hypothesis looks at the problem from

Predictors of the disappearance of triad symptoms in patients with idiopathic normal pressure hydrocephalus after shunt surgery.

We identified factors that predict the disappearance of the triad of symptoms (gait disturbance, cognitive impairment and urinary incontinence) of idiopathic normal pressure hydrocephalus (iNPH) following shunt surgery in this study. We classified 71 patients with iNPH into those whose objective symptoms disappeared (disappearance group) or remained (residual group), for each of the triad symptoms 12 months after shunt surgery. Logistic regression analyses were used to identify the predictors of the disappearance of symptoms among 10 variables before shunt surgery (e.g., age, sex, severity of
