Apoptosis in polycystic kidney disease.

Apoptosis is the process of programmed cell death. It is a ubiquitous, controlled process consuming cellular energy and designed to avoid cytokine release despite activation of local immune cells, which clear the cell fragments. The process occurs during organ development and in maintenance of homeostasis. Abnormalities in any step of the apoptotic process are associated with autoimmune diseases and malignancies. Polycystic kidney disease (PKD) is the most common inherited kidney disease leading to end-stage renal disease (ESRD). Cyst formation requires multiple mechanisms and apoptosis is con…

Nephrolithiasis in medullary sponge kidney.

Seventy-one patients with documented Medullary Sponge Kidney (MSK) and nephrolithiasis underwent complete metabolic evaluation. These patients constituted 7.3% of our calcium stone-forming population Metabolic anomalies (hypercalciuria, hyperoxaluria, hypocitraturia and hyperuricosuria) were observed in 82% of patients. No patient was hypercalcemic and none had hyperparathyroidism. Thus the patients with medullary sponge kidney and renal stones had the same spectrum of metabolic anomalies as the overall population of idiopathic stone formers. Although these patients may have anatomic anomalies…

Vitamin d, and kidney disease.

Mineral metabolism abnormalities, such as low 1,25-dihydroxyvitamin D (1,25(OH)(2)D) and elevated parathyroid hormone (PTH), are common at even higher glomerular filtration rate than previously described. Levels of 25-hydroxyvitamin D (25(OH)D) show an inverse correlation with those of intact PTH and phosphorus. Studies of the general population found much higher all-cause and cardiovascular (CV) mortality for patients with lower levels of vitamin D; this finding suggests that low 25(OH)D level is a risk factor and predictive of CV events in patients without chronic kidney disease (CKD). 25(OH…

CKD-MBD after kidney transplantation.

Successful kidney transplantation corrects many of the metabolic abnormalities associated with chronic kidney disease (CKD); however, skeletal and cardiovascular morbidity remain prevalent in pediatric kidney transplant recipients and current recommendations from the Kidney Disease Improving Global Outcomes (KDIGO) working group suggest that bone disease-including turnover, mineralization, volume, linear growth, and strength-as well as cardiovascular disease be evaluated in all patients with CKD. Although few studies have examined bone histology after renal transplantation, current data sugges…

Dual kidney transplantation: case report.

Chronic shortage of kidney transplants worldwide has led to the use of organs from so called marginal or borderline donors, now termed "expanded-criteria donors". There has been an emerging practice of dual kidney transplantation (DKT) to compensate for sub optimal nephron mass of such kidneys. We performed DKT in "Merkur" University Hospital in August 2005. The donor was a 72-year old female with a history of long-term hypertension, aneurysm of the posterior cerebral artery, cerebrovascular insult (CVI), and with normal creatinine values and kidney function at the time of explantation. Initia…

Sleep disorders in kidney disease.

Sleep disorders are common in patients with end stage renal disease receiving hemodialysis or peritoneal dialysis. However also a well functioning renal graft does not cure the poor sleep pattern which now emerges as a problem even in early chronic kidney disease (CKD). When patients are made aware for the first time of a disease such as CKD, which may brink to dialysis or at the best to a renal transplant patients begin to experience a disordered sleep. Sleeping disorders include insomnia (I), sleep apnoea (SAS), restless legs syndrome (RLS), periodic limb movement disorder (PLMD), excessive …

Leptospirosis-associated acute kidney injury.

Leptospirosis is the most important zoonosis in the world. Patients are typically young men. Several factors are involved in acute kidney injury (AKI) in leptospirosis, including direct nephrotoxic action of the leptospira, hyperbilirubinemia, rhabdomyolysis and hypovolemia. The major histological findings are acute interstitial nephritis and acute tubular necrosis. Leptospirosis-induced AKI is usually nonoliguric and hypokalemic. Tubular function abnormalities precede a decline in the glomerular filtration rate, which could explain the high frequency of hypokalemia. Antibiotic treatment is ef…

[Perioperative modifications in kidney function].

Impaired microcirculation and tubule injuries explain the changes in kidney function during and after surgery. Surgery is a risk factor for acute renal failure, the incidence of which ranges from 0.8% to 40%, depending on the kind of surgery. Mortality due to perioperative acute renal failure ranges from 17% to 60%, depending on the kind of surgery. For almost 10 to 20% of survivors, acute renal failure becomes chronic. Of the various definitions of acute renal failure, the RIFLE classification(risk of renal dysfunction, injury to the kidney, failure of kidney function, loss of kidney function…

[Thrombocytopenia and acute kidney injury].

A 31-year old man is addressed to the emergency department for fever, abdominal pain and vomiting. Laboratory tests reveal an inflammatory syndrome, thrombocytopenia, acute kidney injury associated with major proteinuria (more than 3 g/24 h). Evaluation will conclude to an acute interstitial nephritis and the serology is positive for Hantavirus. This clinical case illustrates differential diagnosis of acute renal failure and reminds the theory of hemorrhagic fever with renal syndrome : usual clinical presentation is "flu"-like symptoms, thrombocytopenia and acute renal failure. Hantavirus is e…

[Preemptive living donor kidney transplant].

We analyze and describe the epidemiology, characteristics and outcome of patients who underwent preemptive kidney transplants from living donors. The outcome of preemptive transplants from deceased donors and that of living donor transplants in patients who have started dialysis are often compared with the outcome of preemptive transplants from living donors. The causes of the better outcome of preemptive kidney transplants are analyzed here. We have examined data from large registries including the Collaborative Transplant Study, USRDS and ANZDATA registry as well as the results of single-cen…

Hereditary and familial kidney cancer.

PURPOSE OF REVIEW: Changing strategies in the evaluation and treatment of renal neoplasms have been fostered by developments in the understanding of the genetics and molecular biology of renal tumors. Many of these discoveries have come from research into hereditary kidney cancer syndromes and the genetic mutations responsible for tumorigenesis. The purpose of this review is to familiarize the reader with the known heritable forms of kidney cancer, the recent advances in the field and their impact on patient management. RECENT FINDINGS: Genetic and phenotypic characterization of renal cancer …
