Simultaneous Osteochondral Fractures of the Medial Patella and Lateral Femoral Condyle Following Lateral Patellar Dislocation: A Case Report.

Patellar dislocation most commonly occurs laterally and is associated with underlying anatomical abnormalities such as trochlear dysplasia and patella alta. While osteochondral injuries typically involve the lateral femoral condyle, simultaneous osteochondral fractures of the medial patella and lateral femoral condyle are extremely rare and often under-diagnosed due to subtle imaging findings and spontaneous patellar reduction. The purpose of this report is to highlight a rare injury pattern following lateral patellar dislocation and to emphasize the importance of early diagnosis using advance

Acute patellar dislocation: how skeletal maturity affects patterns of injury.

OBJECTIVE: The main objective of this study was to understand the role of skeletal maturity in the different patterns of osteochondral and ligamentous injuries after an acute lateral patellar dislocation. MATERIALS AND METHODS: Two radiologists independently reviewed MRIs of 212 knees performed after an acute lateral patellar dislocation to evaluate the presence of high-grade patellar osteochondral injury, femoral osteochondral injury, and medial patellofemoral ligament injury. The association of skeletal maturity (indicated by a closed distal femoral physis), age, sex, and first-time versus

A Computed Tomography Study of the Association Between Increased Patellar Tilt Angle and Femoral Anteversion in 30 Patients with Recurrent Patellar Dislocation.

BACKGROUND Increased femoral anteversion (FA) has been demonstrated in patients with recurrent patellar dislocation (RPD). However, the effect of FA on the patellar tilt angle (PTA) in patients with RPD is unclear. The aim of this study was to compare the FA and PTA between patients with RPD and healthy controls and to investigate the relationship between FA and PTA. MATERIAL AND METHODS A total of 30 knees with RPD and 30 knees from healthy volunteers were evaluated with computed tomography (CT). The FA and PTA were measured and compared between the RPD and control groups. Correlations betwee

Analysis of causes for revision in unicompartmental knee arthroplasty.

The study by Zhao et al identifies the factors leading to the failure of unicompartmental knee arthroplasty (UKA) in their patients. These factors include substandard suturing of the wound, the presence of osteophytes and intra-articular loose bodies causing impingement, premature loosening of the tibial component, choosing unsuitable patients for the procedure, dislocation of the movable insert, and damage to the anterior cruciate ligament and medial collateral ligament. The findings suggest that employing the correct surgical techniques and indications is essential for successful outcomes in

A multidisciplinary approach to a complex case of lower limb trauma.

INTRODUCTION: Blunt trauma of the popliteal fossa is an uncommon cause of lower limb ischemia and it is associated with a high risk for limb loss. REPORT: The case of a young man with anterior dislocation of the right knee joint causing post-traumatic popliteal artery occlusion and severe lower limb ischemia is reported. DISCUSSION: To our knowledge we first report a multidisciplinary approach involving the vascular and plastic surgical teams to treat a wide soft tissue damage after a blunt injury to the popliteal artery.

[Complete anterolateral dislocation of the talus].

A 22-year-old woman suffered an anterolateral dislocation of the talus in a horse-riding accident. Immediately after admission, closed reduction was performed, and then a low knee cast was used for 12 weeks of immobilisation with partial weight bearing (10 kg). Nine months later, the patient had achieved nearly free range of motion of the ankle without pain (dorsal-plantar 10-0-30 degrees ). There were no radiological signs of osteoarthritis or talus bone necrosis. Immediate closed reduction led to an excellent outcome.

Arthroscopic management of the patellar clunk syndrome following posterior stabilized total knee arthroplasty.

The patellofemoral articulation in total knee arthroplasty can give rise to postoperative complications, such as patellar fracture, subluxation, or dislocation. The accumulation of hypertrophic fibrous tissue at the superior margin of the patellar button can give rise to catching or "clunking" of the extensor mechanism. Open surgical excision of this mass is successful in alleviating symptoms but runs the risk of infection and wound complications and delays postoperative mobilization. Arthroscopic resection with motorized instrumentation is highly successful, offers less risk of infection, and

[Proximal tibiofibular joint dislocation diagnosis and treatment].

INTRODUCTION: Proximal tibiofibular joint dislocations (PTFJD) are uncommon and underdiagnosed injuries. Urgent reduction is mandatory to avoid chronic disfunction. The scarcely available literature does not present a unified management guideline. An acute PTFJD case report with spontaneous reduction and a review of the literature is presented, aiming to assist the diagnosis and management of this pathology. CASE REPORT: A 22-years old male presented to the emergency department with high intensity right knee pain after falling in a four-wheel motorcycle. The physical exam revealed a prominent

Injuries about the knee and tibial/fibular shafts.

Injuries to the knee and tibial/fibular shafts are extremely common, with knee injuries alone accounting for over 1.3 million emergency department visits yearly in the United States. Many of these injuries will present with straightforward radiographic findings, but others will have a subtle or complex appearance. This article reviews injuries of the knee and proximal tibial/fibular shaft with emphasis on normal anatomic features that, when disrupted, indicate the presence of subtle but important bone or soft tissue trauma. Although the emphasis is on plain radiography, magnetic resonance (MR)

Chronic fixed-permanent lateral patellar dislocation can be successfully corrected in skeletally mature patients, with satisfactory midterm outcomes.

BACKGROUND: Chronic permenant lateral patellar dislocation is a rare, complex condition that affects an adult's entire lower extremity and may cause functional impairment. This study aimed to identify single-stage surgery for functional recovery in patients and to assess the radiological and functional outcomes of our technique, particularly concerning patellofemoral osteoarthrosis, at mid-term follow-up. METHODS: The study retrospectively analyzed eight patients with eleven afflicted knees who had our novel surgical method, termed the all-in-one treatment, from 2011 to 2017. The average foll

Analysis of lower extremity alignment (LEA) in children with recurrent patellar dislocation by EOS system.

OBJECTIVES: Recurrent patellar dislocation (RPD) greatly affects active young individuals, necessitating the identification of risk factors for a better understanding of its cause. Previous research has connected RPD to lower limb alignment (LEA) abnormalities, such as increased femoral anteversion, tibial external rotation, knee valgus, and flexion. This study aims to use EOS technology to detect RPD-related LEA anomalies, enabling three-dimensional assessment under load conditions. METHODS: A total of 100 limbs (50 in the RPD group, 50 in the control group) were retrospectively analyzed. In

Surgical versus non-surgical interventions for treating patellar dislocation.

BACKGROUND: Patellar dislocation occurs when the patella disengages completely from the trochlear (femoral) groove. Following reduction of the dislocation, conservative (non-surgical) rehabilitation with physiotherapy may be used. Since recurrence of dislocation is common, some surgeons have advocated surgical intervention rather than non-surgical interventions. This is an update of a Cochrane review first published in 2011. OBJECTIVES: To assess the effects (benefits and harms) of surgical versus non-surgical interventions for treating people with primary or recurrent patellar dislocation.

[The effect of breach presentation on the development of the hips].

Authors examined, using sonographic methods in 100 children with breech-presentation, which of the intrauterine postures means a major danger in the development of hip dysplasia. It was found that acetabular alterations, requiring therapy or not, develop more frequently in groups in which the hips were in total flexion, the knees in semiflexion and the lower extremities in major external rotation. Based on their results they state that not the simple breech position is the greatest danger in the development of dysplasia and dislocation in babies born with breech presentation.

Preliminary experience with a method of quadricepsplasty in recurrent subluxation of the patella.

Nineteen knees were treated for recurrent subluxation or dislocation of the patella by lateral and distal transfer of the vastus medialis with or without release of the lateral retinaculum. Fifty-eight per cent has good or excellent and 42 per cent had poor results after a mean follow-up of twenty-nine months. The factors that predisposed to failure were genu valgum greater than 15 degrees and, to a lesser extent, palpable synovitis associated with symptomatic chondromalacia patellae.

Use of semi-constrained total knee arthroplasty in gonarthrosis with collateral ligament insufficiency: Clinical and functional outcomes.

INTRODUCTION: Achieving stability in total knee arthroplasty (TKA) is crucial for long-term implant survival. In cases of severe deformity or ligament laxity, constrained implants may be required. Traditionally, increasing constraint involved intramedullary stems. However, there are intermediary alternatives, including employing a constrained polyethylene insert without stems, thereby avoiding complications related to them. The study aims to evaluate our experience with a non-modular constrained (NMC) implant in primary TKA. MATERIAL AND METHODS: We conducted a retrospective review of the cli
