Adjustment disorder and psychiatric practice: cultural and historical aspects.

A universal concern of healers and health practitioners of all social groups is that of caring for individuals reacting to social and interpersonal hardships. One could view individuals experiencing such hardships as prototypical patients of a society's health profession, however this may be defined. In contemporary psychiatry, such individuals are diagnosed as showing an "adjustment disorder." In view of the time-honored legitimacy of caring for individuals meeting criteria for an adjustment disorder, it is ironic that the status of this "disorder" in the discipline's contemporary nosology is

Psychosocial stresses and psychiatric disorders in an outpatient population in Saudi Arabia.

A retrospective study of 270 psychiatric outpatients at King Faisal Specialist Hospital was conducted to determine psychosocial stresses that contribute to a patient's illness in an Arab culture. These stresses were identified subjectively by the patient as causes of his or her illness; they were then categorized as mental, family, work, economic, grief, physical illness, or other. The categories were correlated with other factors such as age, occupation, sex, and diagnosis. Of 270 patients, 142 (52.6%) reported that stressful life events contributed to their illness, a ratio comparable to tha

Stressful sequelae of disabling illness.

Fifty former rehabilitation patients completed the recent life changes questionnaire (RLCQ), an inventory of potentially stressful life events. Subjects indicated those that had occurred to them during the 6 months prior to onset of illness/injury and during the 6 months following hospital discharge. Certain RLCQ items have associated point values ("life change units" or LCU) indicative of the amount of stress induced by that event. The average number of LCU reported for the post-discharge period exceeded by 3 1/2 times the average for the pre-injury epoch. Ranges for both sets of values were

Gender differences in rating stressful events, depression, and depressive cognition.

Assessed gender differences in the ratings of stressful events, depression, and cognitive distortion. Responses of 70 males and 70 females (N = 140) to the Life Stress Questionnaire, the Beck Depression Inventory, and the Automatic Thought Questionnaire were studied. Statistical analyses revealed significant differences between sexes on all dependent measures. Men reported experiencing more stressful life change. However, women rated the impact of stressors more severely. Women had higher depression ratings, and men exhibited greater distortions in cognitive content. It is proposed that cognit

Psychological aspects of essential blepharospasm.

Evaluation of 20 essential blepharospasm patients by using structured interview and psychological testing (MMPI, Rorschach) revealed characteristic pathological personality patterns in all cases. Recent losses or life change, marital conflict, and specific precipitating events were common. Test findings indicated unexpressed hostility, denial, and repression in a majority of women. Three of five men showed evidence of depression. This initial systematic investigation of psychological factors in blepharospasm supports the hypothesis that neurological and psychogenic factors may interact etiolog

Hospital management of severe poisoning.

Prompt diagnosis and appropriate early treatment will prevent most poisoning episodes from becoming serious. The hazards of any treatment must be weighed against its benefits, and specific indications must be observed in the use of pharmacologic antagonists and chelating agents. Supportive therapy is of predominant importance.

People who deliberately poison or injure themselves: their problems and their contacts with helping agencies.

A representative sample of 130 people was interviewed shortly following self-poisoning or self-injury. The interview method is described. Events in the week prior to the attempt, and the incidence of various kinds of chronic problems are reported. Events involving key relationships were much more common than other kinds. The most important event was a quarrel, particularly in the 48 hours prior to the attempts and more common with female than with male attempters. The possible relevance of quarrels to understanding overdose behaviour is discussed. Nearly a third were receiving non-psychiatric

Living with Crouzon syndrome: how do young adults with Crouzon syndrome handle their life situation?

BACKGROUND: Recent research has been focused on those attributes that appear to buffer a person against the stresses and strains of living with a visible difference. AIM: To provide some insight on how young adults with Crouzon syndrome handle their life. DESIGN: Telephone interviews were carried out with eight Crouzon syndrome individuals (six males, two females, mean age 25.4 years) and data were analysed according to the qualitative method of grounded theory. RESULTS: The informants' main concern was to make the best of their situation, showing that even in adverse conditions, as in Crou

Risk Factors for Depression: Differential Across Age?

INTRODUCTION: The occurrence of well-established risk factors for depression differs across the lifespan. Risk factors may be more strongly associated with depression at ages when occurrence, and therefore expectance, is relatively low ("on-time off-time" hypothesis). This large-scale study examined absolute and relative risks of established risk factors for depression across the lifespan. METHODS: Participants were 2,215 currently or never depressed adults aged 18 to 93 years from two cohort studies: NESDA and NESDO. The occurrence of 19 established risk factors (absolute risk) was examined

Oral history and Hurricane Katrina: reflections on shouts and silences.

In the immediate aftermath of Hurricane Katrina in August 2005, many oral historians throughout the nation began to consider the role their methodology could serve in documenting the storm and its aftermath. Interviewing so soon after such a traumatic event creates new considerations for oral history as an approach to recording experience. The problems and possibilities of oral history as such a moment initiated a vibrant discussion on H-Oralhist and at professional meetings in the fall of 2005. This article reflects on many of the topics raised in that dialogue, including issues of historical

The moderating impact of childhood adversity profiles and conflict on psychological health and suicidal behaviour in the Northern Ireland population.

Childhood adversities are key etiological factors in the onset and persistence of psychopathology. In Northern Ireland the Troubles also impacted on the population's psychological health. This study used data from the Northern Ireland Study of Health and Stress a collaborative epidemiological study which used the WMH-CIDI to assess mental health disorders in a nationally representative sample (Part 2, n = 1986). The aims of the study were to assess co-occurrences of childhood adversities and investigate the impact of adversity profiles and conflict experience on psychopathology and suicidal be

Associations between adverse childhood experiences and acquired brain injury, including traumatic brain injuries, among adults: 2014 BRFSS North Carolina.

Adverse childhood experiences (ACEs) can negatively affect lifelong health and opportunity. Acquired brain injury (ABI), which includes traumatic brain injury (TBI) as well as other causes of brain injury, is a health condition that affects millions annually. The present study uses data from the 2014 North Carolina Behavioral Risk Factor Surveillance System to examine the relationship between ACEs and ABI. The study sample included 3454 participants who completed questions on both ABI and ACEs. Multivariable logistic regression models were used to determine the relationship between ACEs and AB

Exploration of life experiences of positive growth in long-term childhood cancer survivors.

PURPOSE: The aim of this study was to explore experiences of positive growth in long-term childhood cancer survivors, from their perspective. METHODS: Fifteen long-term survivors of childhood cancer provided descriptions of their experiences. Data were collected through face-to-face interviews and the analysis was based on Giorgi's phenomenological research method. RESULTS: The analysis of positive growth experienced by long-term childhood cancer survivors revealed three themes: self-directed life, normalcy in life, and inner maturity. Long-term survivors defined positive growth as a success

Psychosocial and sociocultural correlates of depressive symptoms among diverse African American women.

African American women are faced with many challenges regarding their historical, cultural, and social structural position in the United States that may heighten their vulnerability for depression, one of the most prevalent disorders that can engender poor functionality. The purpose of this cross-sectional pilot study was to foster greater understanding about the occurrence and correlates of depressive symptoms among a diverse convenience sample of 63 African American women recruited from a comprehensive primary health care clinic (n = 23), a small private academic institution (n = 25), and an

[Cultural and migration aspects in functional abdominal pain].

Compared to Europe's mean immigrant contingent of 7.3 to 8.6 % Switzerland holds the highest contingent of foreign population with 23.5 %. Therefore it is of utmost importance that physicians have a knowledge of the specific characteristics of immigrant patients. The influence of personality factors (experience, behavior) is not independent from the influence of culturally-related environmental factors (regional differences in diet, pollutants, meanings, etc.). In addition, different cultural groups rate their quality of life differently. Psychological reasons for recurrent abdominal pain are

The impact of major trauma: quality-of-life outcomes are worse in women than in men, independent of mechanism and injury severity.

BACKGROUND: The importance of gender differences in quality of life and psychologic morbidity after major trauma is a newly recognized focus of trauma outcomes research. The Trauma Recovery Project is a large, prospective, epidemiologic study designed to examine multiple outcomes after major trauma, including quality of life (QoL), and psychologic sequelae such as depression and early symptoms of acute stress reaction (SASR). The specific objectives of the present report are to examine gender differences in QoL outcomes and the early incidence of combined depression and SASR after injury, cont

Life events, familial stress, and coping in the developmental course of schizophrenia.

Recent studies have isolated some socioenvironmental factors that seem to predict the onset of schizophrenic episodes in vulnerable persons. In particular, stressful life events have been found to cluster in the 3- to 4- week period preceding a schizophrenic episode in some patients. Many persons with a schizophrenic disorder also seem to contribute to additional stressful life change events--for example, by high geographic mobility--thereby playing an active role in precipitating the onset of illness episodes. Within the family environment, hostile, critical, and emotionally overinvolved atti…
