How Does Obesity Impact Pediatric Anterior Cruciate Ligament Reconstruction?

PURPOSE: To assess the relationship of elevated body mass index (BMI) on postoperative outcomes, including graft rupture, contralateral anterior cruciate ligament (ACL) tear, new meniscus injuries, isokinetic strength testing, and range of motion (ROM) in a large group of pediatric patients. We also sought to calculate the risk of graft rupture in overweight patients with small femoral or tibial tunnels. The secondary objective was to evaluate the association between BMI and concurrent meniscus tears and the need for meniscectomy at the time of primary ACL reconstruction. METHODS: We retrospe…

Treatment strategies for meniscal lesions: from past to prospective therapeutics.

Menisci play an important role in the biomechanics of knee joint function, including loading transmission, joint lubrication, prevention of soft tissue impingement during motion and joint stability. Meniscal repair presents a challenge due to a lack of vascularization that limits the healing capacity of meniscal tissue. In this review, the authors aimed to untangle the available treatment options for repairing meniscal tears. Various surgical procedures have been developed to treat meniscal tears; however, clinical outcomes are limited. Consequently, numerous researchers have focused on differ

Editorial Commentary: Save the Meniscal Root, Why Not?

The repair of the meniscal root in medial meniscal posterior root tears is receiving increasing interest as more and more research highlights the positive effects of this procedure on the biomechanical restoration of the meniscus. As a testament to the findings of these studies, an international consensus statement recently acknowledged, with several supporting findings from both biomechanical and clinical studies, the effectiveness of meniscal root repairs. Various root repair techniques have been developed with the overarching goal of restoring the structure and function of the meniscus. Yet…

Osteochondritis dissecans of the medial femoral condyle associated with malformation of the menisci.

Osteochondritis dissecans is a form of osteochondrosis limited to the articular epiphysis and the relationship of osteochondritis dissecans and discoid lateral meniscus has been clearly identified. This article presents a 10-year-old boy with osteochondritis dissecans of the medial femoral condyle associated with hypoplastic medial and partial deficient lateral menisci. The patient presented with activity-related pain and intermittent swellings of his left knee for 2 months without any evidence of significant knee trauma. Magnetic resonance imaging revealed anomalous medial meniscus mimicking …

Editorial Commentary: Varus Recurrence After Medial Opening-Wedge High Tibial Osteotomy.

Medial opening-wedge high tibial osteotomy (MOWHTO) is a powerful tool in the surgical armamentarium to correct varus malalignment in patients with symptomatic medial compartment disease. This procedure has demonstrated proven results and long-term survivorship. Complications include cortical hinge fracture, symptomatic hardware, infection, delayed or nonunion, and nerve injury. Varus recurrence is a known complication after MOWHTO. Risk factors are multifactorial and thought to include degree of alignment correction, extent of meniscus and cartilage pathology, and effectiveness of soft-tissue…

Editorial Commentary: Moving the Needle: Traditional Inside-Out Meniscal Repair Has Advantages Over All-Inside Repair.

Meniscus repairs for vertical, peripheral tears can be troublesome due to poor tissue quality and/or vascularity that can lead to re-rupture and subsequent removal. The gold standard, inside-out repair technique, has been challenged by all-inside devices for the benefit of improved efficiency and less morbidity but for the sake of expense and potential structural inferiority. Successful meniscus repair requires multiple components, only one of which is deciding the repair construct of choice. I feel the most important aspect will always be the indication based on tear configuration while respe…

Hot Topics in the Multicenter Orthopedics Outcomes Network: Anterior Cruciate Ligament.

Since 2015, the Multicenter Orthopedic Outcomes Network (MOON) has addressed several factors associated with patient outcome after anterior cruciate ligament (ACL) injury. These factors include predictors of high grade knee laxity, the impact of meniscus treatment on joint space narrowing after surgery, graft selection, treating diabetic patients, predictors of clinically significant pain, and outcomes of patients where meniscus tears were left in situ without treatment. The presence of meniscus tears and generalized laxity predict a lax examination under anesthesia prior to surgery. Allograft…

Factors affecting meniscal extrusion: correlation with MRI, clinical, and arthroscopic findings.

The existence of meniscal extrusion is well known in the natural history of the osteoarthritic knee. However, extruded menisci are also seen in non-pathologic knees. To ascertain the prevalence of meniscal extrusion in non-arthritic patients, the MRIs of 100 knees were prospectively studied. The data were correlated both with clinical and operative arthroscopic findings. The results showed 68.5% of the medial menisci to have some degree of extrusion, averaging at 28% of the meniscal size. While the lateral meniscus were extruded in 18.8% of cases at an average of 15% of the meniscal size. Furt…

Fresh meniscal allograft transplantation and autologous ACL/PCL reconstruction in a patient with complex knee trauma following knee dislocation--a case report.

Instability of the knee joint, particularly in combination with the loss of one meniscus, regularly leads to the early development of arthritis. This paper describes the case of a 19-year-old male with ruptures of the anterior (ACL) and posterior cruciate ligament (PCL) along with the loss of the medial meniscus due to knee dislocation. Combined, time-delayed reconstruction of both the ACL and PCL and the allogenic fresh meniscal transplantation of the medial meniscus without bone plugs were performed. The control arthroscopy performed 6 months post-transplantation revealed good vitality and i…

Most errors occurred in the knees with posterior horn lesions of the medial meniscus. Clinical diagnosis was least accurate for lesions of the lateral meniscus (four missed) and arthroscopy was least accurate for lesions of the posterior horn of the medial meniscus (ten missed). Arthrography appeared to provide collateral evidence o …

Biomechanical Comparison of Meniscal Allograft Root Fixation Techniques: Anterograde Interference Bone Plug Fixation Yields Favorable Results Compared to Transosseous Suture Fixation Alone.

PURPOSE: To compare the biomechanical properties of 2 different fixation techniques (interference bone plug fixation vs transosseous suture fixation) of the posterior horn of the medial meniscus using a porcine model. METHODS: Twenty-six matched pairs of fresh-frozen juvenile domestic porcine knees were used in this study. Specimens were randomly distributed among 3 groups: (1) native meniscus groups, (2) interference fixation, and (3) transosseous suture fixation. In each group, the posterior segments of the tested medial menisci were gripped with the freeze clamps and fixed to the tensile t…

[Classification and treatment of modern traumatic meniscal tears].

The menisci have several important biomechanical functions. Traumatic meniscal injuries present a wide spectrum of presentation and several types of lesions. They can concern the body of the meniscus and its fixation. An easy and clear classification of traumatic meniscus tears is essential for helping the physicians to achieve the correct surgical indication and the accurate postoperative rehabilitation protocol. The aim of this current concept review is to clearly describe some classical as well new type of meniscal and menisco-synovial lesions such as ramp lesions, root tears and instabilit…

Ultrasound-Guided Injection of a Corticosteroid Technique for the Treatment of Degenerative Meniscal Tear.

Degenerative meniscal tear is a chronic disorder that presents with knee pain, swelling, and loss of motion. It usually develops slowly on meniscal tissue that already has macroscopic and ultra-structural changes that affect its resistance to load. Conservative management, such as corticosteroid infiltration, is currently advocated as a first-line approach. However, it has been empirically observed that intra-articular injections do not appropriately alleviate pain because they do not target the trigger area of the meniscus and are quickly cleared from joints via synovial capillaries and lymph…

Cyclin B1 protects against meniscal extracellular matrix catabolism and degeneration via the NOD-like receptor pathway.

Meniscus is vital for maintaining knee homeostasis and function, and its degeneration is linked to knee osteoarthritis (OA) progression. However, the pathological mechanism of meniscus degeneration remains elusive. This study aimed to identify the molecular biological role of Cyclin B1 (CCNB1) in meniscus degeneration. Meniscal transcriptome sequencing of OA/non-OA was performed, and CCNB1 expression was verified in human and experimental mouse meniscal samples. The mice were administered with lentiviruses with CCNB1 overexpression via intra-articular injection after anterior cruciate ligament…
