A new approach to the treatment of experimental septic shock.

Previous work has shown that disseminated intravascular coagulation (DIC) may produce multiple organ failure, including adult respiratory distress syndrome, by obstruction of visceral micro circulation by microclots DIC can be produced by sepsis. This study tests the ability of a plasminogen activator to prevent death from an intravenous injection of killed Escherichia coli by causing lysis of the microclots. Subjects were two groups of 8 pigs each with body weight of 60-70 lbs. Killed Escherichia coli were injected IV in 16 pigs. Invasive monitoring was used to record physiologic data during

Clinical implications of NSAID pharmacokinetics: special populations, special considerations.

Nonsteroidal anti-inflammatory drugs (NSAIDs) are the centerpiece of pharmacologic therapy for most rheumatic disorders and related conditions, and as such are used in great numbers. These drugs are relatively safe and effective, but the pharmacokinetics of NSAIDs can be substantially altered in certain groups of patients, including the elderly and patients with renal and hepatic disease. In these patients, the risk of NSAID toxicity is increased. An understanding of NSAID pharmacokinetics in these groups can help physicians to adjust therapeutic regimens in order to limit the potentially seri

The contribution of the dorsal and ventral roots of the tibial nerve in the propagation of the spinal somatosensory evoked potential in cats.

This study employed selective unilateral dorsal and ventral rhizotomy to examine the contribution of both the dorsal and ventral roots of the tibial nerve in the induction of the spinal somatosensory evoked potential (SSEP) in the cat. The spinal roots of origin of the tibial nerve in the cat were determined by retrograde transport of horseradish peroxidase (HRP). Spinal roots L6, L7, and S1 were found to give rise to the cat tibial nerve. Physiologic monitoring showed that the dorsal roots at these levels primarily contribute to the induction of the SSEP, but the ventral roots also make a sma

Complexity analysis of physiological signals in the wearable era: New insights into chronic diseases and aging.

The human body is a complex and irregular system that generates diverse physiological signals. In the era of wearable technology, an increasing number of physiological signals can be continuously monitored, generating rich time series data that capture the dynamic physiological fluctuations and may reveal subtle signs of dysregulation. For example, continuous glucose monitoring (CGM) systems generate glucose time series data. As such, new metrics harnessing the time series nature of physiological signals may serve as sensitive markers of health. Of them, complexity focuses more on the dynamica

Fixed-distance pH probe placement produces systematic bias in reflux detection: the role of esophageal length.

PURPOSE: Ambulatory esophageal pH monitoring is the reference standard for diagnosing gastroesophageal reflux disease, yet a subset of symptomatic patients demonstrate physiologically nonpositive studies. To evaluate whether variation in esophageal length and relative probe position influences measured reflux exposure and contributes to symptom-physiology discordance during wireless pH monitoring. METHODS: We performed a retrospective analysis of data from 63 patients who underwent Bravo pH monitoring system (Medtronic). Relative probe position was defined as the ratio of capsule placement di

Monitoring the effect of alcohol intake via facial temperature variations using thermography synchronized with heartbeat.

Accurate assessment of alcohol-induced physiological effects is critical for preventing overconsumption and ensuring safe activities such as driving. While breath alcohol concentration (BrAC) measurement is widely used due to its simplicity, it can be confounded by mouth alcohol effects and provides only momentary data. Building on our previously developed Synchro-thermography technique, which synchronizes infrared thermal imaging with heart rate variability to detect vascular-related skin temperature fluctuations, we applied it to monitor physiological changes induced by alcohol. In an alcoho

OPSensing: Embracing Real-Time PPG Monitoring with Intraoral Wearables.

Photoplethysmography (PPG) is a widely used non-invasive optical technique for monitoring physiological parameters such as Pulse Rate (PR) and Blood Oxygen Saturation (SpO2). However, traditional wearables, which typically measure PPG from the wrist or fingertip, encounter shortcomings in motion artifacts, environmental noise, and discomfort during prolonged use. To address these challenges, we propose OPSensing, an intraOral PPG Sensing system designed for real-time physiological monitoring. By exploiting the stable and shielded oral environment, OPSensing achieves high-quality signal acquisi

Toward Patient-Centric Digital Monitoring of Obstructive Sleep Apnea: Mixed Methods Study.

BACKGROUND: Obstructive sleep apnea (OSA) is a sleep disorder characterized by repeated breathing disruptions during sleep. Remote patient monitoring (RPM) of OSA is important, yet contemporary methods are limited. Sensor-based digital health technologies (sDHTs) promise a step advance in OSA RPM, but must provide meaningful, actionable, and usable outputs for patients. While the centrality of considering patient views in sDHT development is widely acknowledged, patient perspectives and priorities are rarely assessed. OBJECTIVE: This study aimed to identify patient-prioritized health aspects

Non-invasive intracranial pressure assessment in adult critically ill patients: A narrative review on current approaches and future perspectives.

Intracranial hypertension (IH) is a life-threatening complication that may occur after acute brain injury. Early recognition of IH allows prompt interventions that improve outcomes. Even if invasive intracranial monitoring is considered the gold standard for the most severely injured patients, scarce availability of resources, the need for advanced skills, and potential for complications often limit its utilization. On the other hand, different non-invasive methods to evaluate acutely brain-injured patients for elevated intracranial pressure have been investigated. Clinical examination and neu

The agreement between oscillometric and intra-arterial technique for blood pressure monitoring in the lower extremities for infants and toddlers undergoing aortic coarctation repair.

OBJECTIVE: Anesthetic management for patients undergoing surgical repair of aortic coarctation (CoA) should include constant blood pressure monitoring of the right upper extremity and a lower extremity. The delayed or absent pulse in the lower limbs often leads to unsuccessful arterial cannulation in infants and the oscillometric technique used for blood pressure measurement. The aim of this study was to evaluate the agreement between the oscillometric method and intra-arterial technique for blood pressure monitoring in the lower limbs of infants undergoing CoA. METHODS: A total of 45 infants

EEG sources of noise in intraoperative somatosensory evoked potential monitoring during propofol anesthesia.

OBJECTIVE: It was hypothesized that somato- sensory evoked potentials can be achieved faster by selective averaging during periods of low spontaneous electroen- cephalographic (EEG) activity. We analyzed the components of EEG that decrease the signal-to-noise ratio of somatosensory evoked potential (SEP) recordings during propofol anesthesia. METHODS: Patient EEGs were recorded with a high sampling frequency during deep anesthesia, when EEGs were in burst suppression. EEGs were segmented visually into bursts, spindles, suppressions, and artifacts. Tibial somatosensory evoked potentials (tSEPs

Investigation of cyanotic/apneic episodes and sleep-related upper airway obstruction by long-term non-invasive bedside recordings.

Over a 4 year period, investigations were performed on 226 infants and children suffering cyanotic episodes and 160 suspected to be the victims of sleep-related upper airway obstruction. These investigations have been considerably aided by long-term tape recordings of multiple cardiorespiratory and neurophysiological variables from non-invasive sensors. In addition to identifying the underlying mechanisms responsible for cyanotic episodes, recordings have identified the presence or absence of airway obstruction, documented its severity by its effects on oxygenation and on sleep-state organizat

[Critical analysis of the response to antihypertensive therapy. Report of 102 cases].

UNLABELLED: One hundred and two patients with mild to moderate hypertension, non responding to a placebo therapy, were given an antihypertensive drug for a mean period of 6 weeks (range: 3-12 weeks). Their blood pressure (BP) was measured before and after active treatment by both a mercury manometer at the clinic and an ambulatory apparatus (Spacelabs 5200 or 90202). The clinic BP figures were poorly correlated with the 24 hour ambulatory BP levels, as well as the magnitude of decrease in clinic BP with the reduction in ambulatory BP. Twenty eight patients who were considered as "clinic respon

Hybrid Feature Learning for Wearable Stress Detection: Combining Domain Knowledge with Supervised Deep Learning.

Accurate stress monitoring is critical for high-risk professions like firefighting, yet existing wearable solutions face challenges balancing accuracy with practical usability. While electrodermal activity (EDA) offers a non-invasive, single-sensor approach, current automated feature extraction methods fail to capture stress-discriminative patterns effectively. We developed a hybrid stress detection pipeline combining 20 hand-crafted physiological features with 32 deep-learned features from a supervised convolutional autoencoder. Unlike traditional unsupervised approaches optimized solely for

Temperature Measurement Inside Protective Headgear: Comparison With Core Temperatures and Indicators of Physiological Strain During Exercise in a Hot Environment.

Non-invasive temperature monitoring with a sensor inside protective headgear may be effective in detecting temperatures that are associated with heat illness. The purpose was to establish the relationship between in-hardhat temperatures (Tih) and core temperature (Tc) as measured by rectal (Tre) and esophageal (Tes) probes. Thirty males (age 24.57 ± 4.32 yrs.) completed two trials: continuous submaximal exercise (CSE) and a series of high intensity 30-s sprints (HIE) with a one-minute rest between each. Exercise in both conditions was in a 36(°)C environment (40% RH) while wearing a standard h

Brief airway obstructions during sleep in infants with breath-holding spells.

We investigated the possibility that infants with breath-holding spells have breathing disorders during sleep. Seventy-one breath holders with a median age of 14 weeks were selected from a well babies clinic because of their histories: 34 infants without loss of consciousness, and 37 with loss of consciousness (21 of the latter had had cyanotic spells, 14 pallid spells, and 2 combined cyanotic and pallid spells). For each breath holder, one control infant without a history of breath holding was chosen from the same clinic. All infants were healthy and had no known cause of disrupted breathing
