Epileptic seizures, movement disorders, and breathing disturbances in Rett syndrome: diagnostic relevance of video-polygraphy.

Epileptic seizures, movement disorders and breathing disturbances may be observed in Rett syndrome, and correct diagnosis is mandatory for the management. We evaluated the usefulness of video-polygraphy in the differential diagnosis between epileptic and non-epileptic paroxysmal events in eight patients with Rett syndrome. Based on video analysis, myoclonic seizures were usually misdiagnosed as movement disorders and stereotypies; the events identified by parents as generalized tonic-clonic seizures included episodes of motor activity and breathing abnormality. Myoclonic seizures aggravated by…

Characterization of movement disorders in patients with familial Creutzfeldt-Jakob disease carrying the E200K mutation.

BACKGROUND: While myoclonus and ataxia are considered common in patients with familial Creutzfeld-Jakob disease (fCJD), other movement disorders are less prevalent. OBJECTIVES: To systemically evaluate the frequency of extrapyramidal signs and movement disorders in patients with fCJD. METHODS: A detailed neurological examination, with special emphasis on movement disorders and extrapyramidal signs, was conducted in 43 consecutive symptomatic CJD patients (26 males and 17 females; mean age 58.7 +/- 8.9 yrs, range 43-77 years) carrying the E200K mutation in the PRNPgene. RESULTS: Limb or gait…

Education research: patient telephone calls in a movement disorders center: lessons in physician-trainee education.

OBJECTIVE: Telephone medicine is part of clinical practice, but there are no published data on the volume, nature, and time allocation of patient-related telephone calls received in a movement disorders center. Such data might provide insights which augment patient care, and may be instructive regarding medical education, since patient-related telephone calls are often addressed by physicians-in-training. METHODS: Characteristics of patient-related calls to a movement disorders center were prospectively recorded during a 2-month period. RESULTS: A total of 633 calls were generated by 397 pat…

Reduction in neuroleptic-induced movement disorders after a switch to quetiapine in patients with schizophrenia.

BACKGROUND: Persistent neuroleptic-induced movement disorders limit effective pharmacological management of psychotic disorders. Although antipsychotic switching is a common strategy for managing extrapyramidal side effects (EPSs), there is insufficient empirical support to guide the clinician. We designed the present study to examine whether patients with preexisting EPS switched to quetiapine would show greater reduction in EPS compared with control patients. METHODS: Twenty-two patients with schizophrenia meeting clinical criteria for tardive dyskinesia or coexisting parkinsonism were rand…

Movement disorders at a university hospital emergency room. An analysis of clinical pattern and etiology.

We prospectively evaluated the clinical features and etiologies of all common categories of movement disorder seen in the emergency room (ER) of an urban university hospital over a period of 12 months. We divided movement disorders according to the presenting phenomenology likely to dominate the clinical presentation, that is, gait disorder, tremor, dystonia, myoclonus, and acute akinetic crisis and classified a specific etiology in the individual phenomenology. In one year, there were 60,002 ER visits; of these, 58 (0.09 %) were diagnosed as a primary movement disorder. The most common clinic…

Spectrum of movement disorders associated with glutaric aciduria type 1: a study of 16 patients.

Glutaric aciduria type 1 (GA1) is an autosomal recessive neurometabolic disorder due to glutaryl CoA dehydrogenase deficiency. Comprehensive descriptions of GA1-associated movement disorders are rare. In order to refine the description of the motor phenotype, we prospectively studied 16 consecutive pediatric and adult GA1 patients, focusing on the movement disorders and their time course. In most patients, generalized dystonia, superimposed on baseline axial hypotonia, remained the predominant feature throughout the disease course. With aging, it tended to evolve from mobile to fixed dystonia …

Pathophysiology of the basal ganglia and movement disorders: from animal models to human clinical applications.

Electrophysiological studies in control and MPTP treated primates have played a major role in our understanding of the physiology of the basal ganglia and the pathophysiology of Parkinson's disease (PD). Early models emphasized discharge rate and viewed the basal ganglia as a network of boxes (nuclei) connected by excitatory or inhibitory connections. More recent studies view the basal ganglia as neural networks with weak and non-linear interactions in and between the different nuclei. Microelectrode electrophysiological recording enables the high resolution-both in the temporal domain (spike)

[Treatment of laryngeal movement disorders with botulinum toxins: part 1: History and mode of action].

Laryngeal dystonia (spasmodic dysphonia) is a movement disorder characterised by involuntary contractions of the laryngeal muscles involved in vocalisation. The introduction of botulinum toxin (BTX) in the treatment of laryngeal dystonia had a major clinical impact due to the striking improvement of symptoms. Most patients with severe types of spasmodic dysphonia are treated with injections of botulinum toxin type A. For patients with a resistance against type A toxin there is a new hypercleaned type A toxin or type B available. Research on type F toxin is also underway. In this article, the h

Natural and nosocomial infection in a patient with West Nile encephalitis and extrapyramidal movement disorders.

Since its first recognition in North America in 1999, West Nile virus (WNV) has spread rapidly across the continent, but in many communities, rapid diagnostic tests for detection of WNV infection are not fully available. We describe a patient with extrapyramidal movement disorders and changes in the basal ganglia noted on magnetic resonance images that are characteristic of other flavivirus encephalitides and may help in the recognition of patients with West Nile encephalitis. Detailed molecular analysis suggested that, although our patient received a blood transfusion infected with WNV, the v…

[Biomechanical gait analysis after elongation of the Achilles tendon in patients with cerebral movement disorders].

The biomechanical gate analysis makes possible an exact interpretation of single locomotion in all details. Even by patients with cerebral palsy an improvement of objective and self-critical clinical assessment of therapy was achieved. We report on the results of biomechanical gate analysis after elongation of the Achilles tendon. The effects of the operation especially concerning coordination of the gate have been analysed.

Glymphatic system activity in functional movement disorder - a link to widespread pain.

AIM OF THE STUDY: The study aimed to evaluate glymphatic function in functional movement disorder (FMD) and examine its clinical correlates. CLINICAL RATIONALE FOR THE STUDY: The glymphatic system plays a key role in brain homeostasis by facilitating perivascular clearance of interstitial fluid and metabolic waste, particularly during sleep. The diffusion tensor imaging analysis along the perivascular space (DTI-ALPS) index provides a non-invasive magnetic resonance imaging-derived (MRI-derived) proxy of glymphatic system activity. The involvement of the glymphatic system in FMD is unknown.

What if it is the eyes? The effect of eye movement disorders on cognitive testing in MS.

BACKGROUND: Visual cognitive tests that rely on saccades may be affected by internuclear ophthalmoplegia (INO), irrespective of cognitive ability. INO and cognitive decline are common in people with multiple sclerosis (PwMS). We assessed the impact of INO on visual versus non-visual cognitive tests in PwMS. METHODS: PwMS (n=197) completed auditory and visual neuropsychological tests, and demographically corrected Z-scores were obtained. INO was quantified using the DEMoNS infrared oculography protocol, applying the versional dysconjugacy index (VDI) of area under the saccadic trajectory (AUC)

Aberrant preparation of hand movement in schizophrenia spectrum disorder: an fMRI study.

Schizophrenia spectrum disorder is linked to impaired self-other distinction and action feedback monitoring, largely stemming from sensory-motor predictive mechanisms. However, the neural correlates of these predictive processes during movement preparation are unknown. Here, we investigated whether patients with schizophrenia spectrum disorder exhibit aberrant sensory-motor predictive processes reflected in neural activation patterns prior to hand movement onset. Functional MRI data from patients with schizophrenia spectrum disorder (n = 20) and healthy controls (n = 20) were acquired during a

Hypoxia ameliorates neurodegeneration and movement disorder in a mouse model of Parkinson's disease.

Parkinson's disease (PD) is characterized by inclusions of α-synuclein (α-syn) and mitochondrial dysfunction in dopaminergic (DA) neurons of the substantia nigra pars compacta (SNpc). Patients with PD anecdotally experience symptom improvement at high altitude; chronic hypoxia prevents the development of Leigh-like brain disease in mice with mitochondrial complex I deficiency. Here we report that intrastriatal injection of α-syn preformed fibrils (PFFs) in mice resulted in neurodegeneration and movement disorder, which were prevented by continuous exposure to 11% oxygen. Specifically, PFF-indu
