Churg-Strauss Sindrome or allergic granulomatosis is a small vessel systemic vasculitis characterized by asthma, hypereosinophilia and necrotizing vasculitis with extravascular eosinophil granulomas. We describe a case assisted in our hospital who presented pulmonary infiltrates, fever, peripherical neuropathy, weight loss, myalgia, rhinosinusitis, with antecedents of nasal polyposis, and facial edema. Our aim is to review the main otolaryngologic manifestations of this rare illness that is treated with oral corticosteroids, immunosupresor drugs like ciclofosfamide and plasmapheresis.

A 67-year-old man presented with myalgia, muscle atrophy, and a history of seronegative polyarthritis. Blood tests showed inflammation but no hematologic or immunologic abnormalities. Muscle biopsy revealed no vasculitis or myositis but Tropheryma whipplei was detected by polymerase chain reaction in muscle, blood, and duodenum specimens; this was confirmed by immunohistochemistry. Ceftriaxone led to clinical improvement. Although rare, Whipple's disease should be considered in the differential diagnosis of diffuse myopathy.

A case of polymyositis associated with primary cytomegalovirus infection in a 17-y-old girl is reported. The girl exhibited fever, sore throat, progressive myalgia and muscle weakness with elevated creatine kinase, atypical lymphocytosis and myopathic features in the electromyogram. Histopathologically, biopsied muscle met the criteria for polymyositis. Primary cytomegalovirus infection was proven by seroconversion of IgG as well as IgM antibodies. This is the first report of an association between cytomegalovirus infection and polymyositis.

Cutaneous polyarteritis nodosa is a benign form of the disease with cutaneous involvement and occasional systemic manifestations. We report a 51 years old man presenting with malaise, arthralgia, myalgia and nodular, erythematous and painful skin lesions in both legs. The pathological study of these lesions showed a polyarteritis. During hospital stay there was a mild remission of the disease without treatment, but its reappearance during follow lead to the use of prednisone.

Glycogenosis type V (McArdle disease) is a serious metabolic disorder with an exercise intolerance, myalgia, early fatigue and stiffness of exercising muscles, relieved++ by rest. The authors present a case report of patient with McArdle's disease, and diagnostic procedures which can be used in different diagnostic of metabolic myopathies, especially between myoadenylate deaminase deficiency and different types of gly(geno)lytic myopathies. The importance of "ischemic forearm test" and muscle biopsy is emphasized.

A 71 year old woman with typical giant-cell arteritis experienced myalgia of the legs immediately at the end of steroid therapy, 3 months after the onset of her illness. Muscle biopsy showed a focal necrotising panarteritis whereas the muscular fibres remained unchanged. The patient is now asymptomatic after 3 years' follow-up on prednisolone therapy (10 mg per day). This case demonstrates that giant cell arteritis can also involve the muscular arteries.

An Irish family with an unusual periodic syndrome is described. Attacks consist of fever with localized myalgia and painful erythema. Other features include abdominal pain and pleurisy, with leucocytosis and a high ESR. The syndrome resembles classical familial Mediterranean fever (FMF) but differs from it in its prompt response to steroids and its autosomal dominant pattern of inheritance. The disease appears to have a benign course and no patient has developed amyloid.

Two patients had a delayed syndrome of benign fasciculation after complete recovery from paralytic poliomyelitis and a third had the syndrome after an attack of purely motor myelitis. Myalgia was prominent in all three, and two had frequent cramps. Denervation hypertrophy occurred in two. The patients were observed for at least three years, and no new weakness was seen. These cases suggest that some chronic diseases of motor neurons may be benign.
