Retrograde nailing for subtrochanteric femur fractures.

This study reviews the results of subtrochanteric femur fractures treated with a retrograde nail at a level 1 trauma center. Sixteen patients with 17 fractures were followed up until union. The average Injury Severity Score (ISS) was 19, and average knee range of motion was 127 degrees. Four patients required dynamization of their nail, and three healed over a prolonged period of time. Two patients required exchange nailing: one after failure of union after dynamization and one with a broken nail that was revised to an antegrade nailing. The average varus deformity was 5.06 degrees and only 35

Life-threatening nail gun injuries.

The use of pneumatic and explosive cartridge-activated nail guns is common in the construction industry. The ease and speed of nailing these tools afford enhance productivity at the cost of increased potential for traumatic injury. Although extremity injuries are most common, life-threatening injuries to the head, neck, chest, or abdomen and pelvis may occur. During a 20-month period, eight potentially life-threatening nail gun injuries were admitted to a Level I trauma center, including injuries to the brain, eye, neck, heart, lung, and femoral artery. Mechanism of injury included nail ricoch

Anatomic considerations of locked humeral nailing.

To investigate the risk of axillary nerve injury by the proximal locking screws in antegrade nailing of humeral fractures, the anatomy of the axillary nerve was examined in 20 fresh anatomic specimen humeri, which subsequently were nailed antegrade with specially designed humeral locked nails. The axillary nerve was found to be on average 45.6 mm below the tip of the greater tuberosity; it was jeopardized by insertion of the lower proximal locking screw in one of the 20 specimens. Short humeri, humeri with small heads, or too deeply inserted nails may increase the risk of nerve injury; likewis…

Intramedullary nailing of forearm fractures in children.

Forearm fractures are very common in children. They are usually treated orthopedically. For the last few years, Métaizeau's elastic stable nailing has been used in the authors' hospital. This study is based on 80 children with forearm fractures treated with intramedullary nailing: 64 boys and 16 girls, aged 6 to 16 (23 right sides, 58 left sides: 81 fractures). Nailing was performed 67 times for a displaced fracture, 3 times for a recurrent fracture, 3 times after a secondary displacement, and 7 times in patients with multiple injuries. Sound union was normally achieved in 78 patients, and nor

Skin, hair and nail fungal infections.

Fungal infections of the skin, hair and nails are among the most common skin diseases. The increased incidence of these infections in recent years has been attributed to the large number of immunocompromised patients, life-style changes (greater attendance at gymnasiums, public baths, etc.), the use of chemotherapeutic agents, and the greater presence of elderly people in the general population. Superficial fungal infections were also the most frequent and troublesome dermatological condition occurring among US forces during the war in Vietnam.

Intramedullary nailing of femoral fractures in adolescents.

A prospective study of adolescents with femoral fractures treated with intramedullary nailing was done to identify complications and evaluate the outcome of this method of treatment. Sixty patients with femoral fractures have been treated with antegrade intramedullary nailing. Magnetic resonance imaging scans were obtained to evaluate for subclinical avascular necrosis of the femoral head. The average patient age was 12 years. All patients had open physes at the time of surgery. Implants were removed in 33 patients to date at an average of 10 months after initial nailing. All but two patients

The management of pigmented lesions of the nail bed.

Pigmented lesions of the nail bed, especially without a history of trauma, represent a diagnostic challenge to the clinician. These lesions are often categorized as melanonychia striata (MS), which refers to any linear tan-brown-black pigmentation of the nail bed. The differential diagnosis of MS includes subungual hematomas, onchomycosis nigricans, junctional nevi, melanoma in situ (MIS), and malignant melanoma (MM). Our algorithm at the New York University (NYU) Medical Center for the treatment of pigmented lesions of the nail bed is presented. A histopathologic diagnosis with any evidence o

Squamous cell carcinoma of the nail bed.

Squamous cell carcinoma of the nail bed is a rare disorder. Metastasis is extremely unusual. A case of primary squamous cell carcinoma of the nail bed with metastasis to the regional lymph nodes is reported. The tumor was successfully treated by amputation of the involved finger, block dissection of the regional lymph nodes, postoperative irradiation of the involved axilla, and systemic chemotherapy. Early diagnosis by biopsy, especially in patients with recurrent and persistent disease of the nail bed, is recommended, so that treatment can be instituted early while the tumor is confined to it

Femoral nailing without a fracture table.

This retrospective trail was designed to evaluate the effectiveness and safety of femoral nailing on a radiolucent table with manual traction only. Eighty-three femoral shaft fractures treated by antegrade nailing were included in this study. Group 1 consisted of 24 femur fractures that were reduced and nailed with manual traction. Group 2 consisted of 59 femur fractures treated with the aid of a fracture table. There were 10 patients in Group 1 and 19 patients in Group 2 needing multiple procedures. In Group 1, significantly fewer redrapings and table transfers were necessary. There was no in

Yellow nail syndrome: a rarity in Indians?

Reports of yellow nail syndrome have been few and far between. The classical triad of the syndrome has not been reported in Indian literature. We report a case of yellow nail syndrome in a forty-year-old male, who had yellowish-brown nails from birth. He developed lymphoedema of the legs at the age of twenty years and presented with pleural effusion at the age of forty years. Although a case of yellow nail syndrome has been reported from India, the classical triad of the syndrome is yet to be documented from our country. The condition may be missed because of the long time difference in presen…

Complications of nailing in closed tibial fractures.

Reamed intramedullary nailing was used to treat 102 closed tibial shaft fractures. Although a high rate of union was achieved (98%), we noted a higher incidence of complications than previously reported. Thirty-five (37%) patients exhibited one or more components of malunion. In 17 (18%) patients, further operative procedures were required to achieve union or to correct malalignment. New, postoperative peroneal nerve lesions were found in 19 (19%) patients, of which four (4%) failed to resolve. Acute compartment syndrome, requiring fasciotomy, developed in seven (7%) cases after surgery. Two p
