Can intraoperative facial nerve motor evoked potential monitoring predict postoperative facial synkinesis following vestibular schwannoma surgery? A retrospective study.

BACKGROUND: Facial synkinesis is a distressing late-onset complication of vestibular schwannoma (VS) surgery. The current study aimed to explore the predictive value of intraoperative facial nerve motor evoked potential (FNMEP) monitoring for postoperative facial synkinesis in VS patients. METHODS: The data of 170 VS patients were retrospectively analyzed. The final-to-baseline amplitude ratio (FBR) of mentalis FNMEP was selected as a representative metric to assess FNMEP alterations before and after tumor resection. The presence of facial synkinesis was evaluated one year after surgery. Group

Triple motor mapping: transcranial, bipolar, and monopolar mapping for supratentorial glioma resection adjacent to motor pathways.

OBJECTIVE: Maximal safe resection of gliomas near motor pathways is facilitated by intraoperative mapping. The authors and other groups have described the use of bipolar or monopolar direct stimulation to identify functional tissue, as well as transcranial or transcortical motor evoked potentials (MEPs) to monitor motor pathways. Here, the authors describe their initial experience using all 3 modalities to identify, monitor, and preserve cortical and subcortical motor systems during glioma surgery. METHODS: Intraoperative mapping data were extracted from a prospective registry of glioma resec

Non-Invasive Estimation of Intracranial Pressure by Diffuse Optics: A Proof-of-Concept Study.

Intracranial pressure (ICP) is an important parameter to monitor in several neuropathologies. However, because current clinically accepted methods are invasive, its monitoring is limited to patients in critical conditions. On the other hand, there are other less critical conditions for which ICP monitoring could still be useful; therefore, there is a need to develop non-invasive methods. We propose a new method to estimate ICP based on the analysis of the non-invasive measurement of pulsatile, microvascular cerebral blood flow with diffuse correlation spectroscopy. This is achieved by training

Frontal pathways in cognitive control: direct evidence from intraoperative stimulation and diffusion tractography.

A key aspect of cognitive control is the management of conflicting incoming information to achieve a goal, termed 'interference control'. Although the role of the right frontal lobe in interference control is evident, the white matter tracts subserving this cognitive process remain unclear. To investigate this, we studied the effect of transient network disruption (by means of direct electrical stimulation) and permanent disconnection (resulting from neurosurgical resection) on interference control processes, using the Stroop test in the intraoperative and extraoperative neurosurgical setting.

Invasive monitoring after resection of epileptogenic neocortical lesions in multistaged epilepsy surgery in children.

OBJECTIVE: Incomplete resection of neocortical epileptogenic foci correlates with failed epilepsy surgery in children. We often treat patients with neocortical epilepsy with a staged approach using invasive monitoring to localize the focus, resect the seizure onset zone, and, in select cases, post-resection invasive monitoring (PRM). We report the technique and the outcomes of children treated with staged surgery including PRM. METHODS: We retrospectively reviewed the charts of pediatric patients with neocortical epilepsy who underwent resective surgery with PRM. RESULTS: We identified 71 pa

Safety and feasibility of a novel recurrent laryngeal nerve monitoring technique.

OBJECTIVES/HYPOTHESIS: Continuous intraoperative nerve monitoring (CIONM) during thyroidectomy is promising technology that may reduce the incidence of recurrent laryngeal nerve (RLN) injuries. However, the current technique for CIONM, in which a stimulating probe is placed on the vagus nerve, has not been widely adopted because of concerns regarding its invasiveness and safety. A new method with the lead positioned on the RLN may overcome these objections and promote greater adoption of CIONM. This study was conducted to serve as the first step in determining the safety and feasibility of an

Factors Influencing Diaphragmatic Compound Motor Action Potentials During Cryoballoon Ablation for Atrial Fibrillation.

INTRODUCTION: The diaphragmatic compound motor action potentials (CMAPs) have been used to predict and prevent phrenic nerve injury (PNI) during cryoballoon ablation of right pulmonary veins. We sought to assess factors that influence the amplitude of the surface CMAP recordings. METHODS AND RESULTS: We analyzed CMAPs from consecutive patients undergoing cryoballoon ablation for paroxysmal atrial fibrillation. CMAP recordings were obtained using electrocardiography electrodes positioned in the "modified lead I" method while stimulating the right PN, until loss of capture (ascertained by palpa

Medically resistant pediatric insular-opercular/perisylvian epilepsy. Part 2: outcome following resective surgery.

OBJECTIVE Seizure onset in the insular cortex as a cause of refractory epilepsy is underrepresented in the pediatric population, possibly due to difficulties localizing seizure onset in deep anatomical structures and limited surgical access to the insula, a complex anatomical structure with a rich overlying vascular network. Insular seizure semiology may mimic frontal, temporal, or parietal lobe semiology, resulting in false localization, incomplete resection, and poor outcome. METHODS The authors retrospectively reviewed the records of all pediatric patients who underwent insular cortical res

Risk of perioperative seizures in patients undergoing craniotomy with intraoperative brain mapping.

BACKGROUND: The identification of risk factors associated with perioperative seizures would be of great benefit to the anesthesiologist in managing brain tumor patients undergoing craniotomy with intraoperative brain mapping. METHODS: A series of 316 supratentorial craniotomies for tumor resection, in which intraoperative brain mapping was used, were analyzed. From January 2005 to December 2010 the occurrence of intraoperative and immediate postoperative clinical seizures was prospectively recorded into a database. Demographic data, tumor characteristics, preoperative seizure control, intraop

Preservation of microelectrode recordings with non-GABAergic drugs during deep brain stimulator placement in children.

OBJECT: Deep brain stimulation (DBS) has become accepted therapy for intractable dystonia and other movement disorders. The accurate placement of DBS electrodes into the globus pallidus internus is assisted by unimpaired microelectrode recordings (MERs). Many anesthetic and sedative drugs interfere with MERs, requiring the patient to be awake for target localization and neurological testing during the procedure. In this study, a novel anesthetic technique was investigated in pediatric DBS to preserve MERs. METHODS: In this paper, the authors describe a sedative/anesthetic technique using keta

Preventing position-related brachial plexus injury with intraoperative somatosensory evoked potentials and transcranial electrical motor evoked potentials during anterior cervical spine surgery.

The use of somatosensory evoked potentials (SSEPs) to monitor upper extremity nerves during surgery is becoming more accepted as a valid and … hen neurological examination was performed. This report demonstrates a case in which intraoperative neurophysiological monitoring was useful in identifying and reversing impending nerve injury during cervical spine surgery. Significant changes were seen in SSEPs as well as TCeMEPs, so we recommend that TCeMEP monitoring be considered as an adjunct to SSEPs for prevention of in …

Objective Assessment of the Cough: Listening to the Patient's Voice. A Narrative Review.

Despite its prevalence-affecting up to 10% of adults worldwide-objective measurement of the cough-one of the most common yet least quantifiable respiratory symptoms-remains challenging. Conventional descriptors, such as 'frequent' or 'severe,' are inherently subjective and poorly reproducible, limiting clinical interpretation and standardization. Over the past five decades, technological advances have transformed cough assessment from manual counting and provocation testing to automated acoustic monitoring and neurophysiologic imaging. Modern validated systems, such as the Leicester cough moni

Gestational buprenorphine-naloxone exposure and fetal neurobehavior.

BACKGROUND: Buprenorphine-naloxone treatment may confer substantial benefits for the treatment of opioid use disorder (OUD) during pregnancy including lower risk for overdose/death, less diversion potential and reduced use of other substances. Treatment may also result in less severe Neonatal Abstinence Syndrome (NAS), but little is known about the effects of this medication on fetal neurodevelopment. METHODS: The purpose of the current study is to evaluate neurobehaviors among fetuses exposed to buprenorphine-naloxone at four time points over the second and third trimesters of gestation in p

Monitoring of facial muscle motor evoked potentials during microvascular decompression for hemifacial spasm: evidence of changes in motor neuron excitability.

OBJECT: Hemifacial spasm (HFS) is thought to be due to a hyperactive facial motor nucleus consequent to chronic neurovascular contact. The lateral spread (LS) response is presumed to reflect changes in facial motor neuron excitability. Facial muscle motor evoked potentials (MEPs) use the same efferent pathway as LS, therefore the authors speculated that these potentials should reflect differences consistent with changes at the facial motor nucleus level. METHODS: Monitoring of LS and bilateral facial MEP was performed in 10 consecutive patients undergoing MVD for HFS. Ipsilateral facial MEPs

Quantitative Electroencephalography in Hemodialysis: Scoping Review and Translational Framework.

BACKGROUND: Cognitive impairment and cerebral dysfunction are common among patients with end-stage kidney disease undergoing maintenance hemodialysis. Repeated intradialytic hemodynamic stress, including reductions in cerebral blood flow and episodes of intradialytic hypotension, has been associated with adverse neurological outcomes. Quantitative electroencephalography (qEEG) offers a noninvasive approach for continuous assessment of cerebral activity; however, its potential role in dialysis monitoring remains unclear because of methodological heterogeneity and implementation challenges. Diagnostic Accuracy of Somatosensory-Evoked Potential Changes for Predicting Postoperative Neurological Deficits Following Brain Tumor Surgery.

BACKGROUND: Intraoperative neuromonitoring is widely used during brain tumor surgery to reduce the risk of neurological injury. Somatosensory evoked potentials (SSEPs) are frequently employed to monitor functional integrity of sensory pathways and assist with localization of the central sulcus. However, the diagnostic accuracy of SSEP changes alone in predicting postoperative neurological deficits during brain tumor resection remains incompletely characterized. METHODS: This retrospective study included 511 patients who underwent craniotomy with SSEP monitoring between 2019 and 2023. Postoper
