Early surgical management of severe forms of osteogenesis imperfecta.

The early surgical management of severe osteogenesis imperfecta (OI) was studied in three children with autosomal recessive type III form of this disease. Each child had similar clinical and radiographic features at birth. The spine, pelvis, and thorax were osteoporotic but were well formed and free of major fractures. In contrast, the peripheral skeleton was severely affected with numerous new and old fractures. Conservative treatment of fractures was undertaken with a foam mould for the torso and limbs and additional support was provided for specific unstable painful fractures. However, frac…

Collagen metabolism in cultured osteoblasts from osteogenesis imperfecta patients.

Collagen produced in vitro by bone cells isolated from 19 patients with different forms of osteogenesis imperfecta (OI) was analysed. Clinically, four patients were classified as OI type I, 10 patients as OI type III and five patients as OI type IV. Bone cells of 12 of the 19 OI patients produced structurally abnormal type I collagen. Electrophoretically uniformly slower migrating collagen type I alpha-chains were found in one case of OI type I, in seven cases of OI type III and in one case of OI type IV; two cultures of OI type III produced two different populations of collagen type I alpha-c…

Fractures in early childhood: osteogenesis imperfecta or child abuse?

We studied the radiographs of 194 fractures sustained by 39 children less than 5 years of age with osteogenesis imperfecta (OI) and compared them with radiographs of 84 fractures sustained by 69 normal children of the same age. A great variety of fracture patterns was observed in undoubted OI. Spiral and transverse fractures were the most common. Metaphyseal fractures accounted for 15% of the fractures. The results of this study do not support the view that a particular fracture pattern renders the diagnosis of OI unlikely.

Corrective femoral osteotomy and telescopic nailing in osteogenesis imperfecta.

Six cases of osteogenesis imperfecta with femoral deformity treated by Bailey telescopic nailing are discussed. In 4 cases the operation was bilateral. The long-term results show that this nail lengthens without compromising longitudinal growth of the bone, while providing protection from fracture and recurrence of the deformity. Thinning of the diaphyseal cortex was a significant complication in 3 femurs out of 10. Although it is more difficult to apply, the telescopic nail is preferable to traditional nailing because of its better mechanical features which allow for early weightbearing and w…

Abnormal amino acid analyses obtained from osteogenesis imperfecta dentin.

Amino acid analyses were carried out on dentin proteins obtained from 33 normal teeth and 59 teeth from osteogenesis imperfecta patients. The analyses revealed that in the control teeth approximately 95% of the insoluble dentin fraction was collagen. The majority of the analyses for teeth obtained from the O.I. patients were biochemically abnormal (55 out of 59). Specifically, there was a significant increase in the acidic amino acids, with a corresponding decrease in the basic amino acids. A small group of patients showed double peaks in the histidine, hydroxylysine, and lysine areas of the c…

Clinical and radiological features of osteogenesis imperfecta type IVA.

This paper reports a survey of 78 patients with osteogenesis imperfecta (OI) type IVA, the variant that causes the greatest difficulties in differential diagnosis. A subgroup of children aged between five and eleven were compared with a control group from local schools. Among the OI children there were, apart from the high number of fractures, also significantly increased incidences of bruising, nosebleeds, excessive sweating and hypermobility of joints. Although none of the children with OI type IV had the dark blue or grey sclerae of the type I disease, an appreciable number had pale blue sc…

Osteogenesis imperfecta type III: an ancient mutation in Africa?

In a survey of institutions for crippled persons in Zimbabwe, 58 patients with osteogenesis imperfecta (OI) were identified; 42 had the rare OI Type III. The Shona and Ndebele people, who comprise the major tribal groups in Zimbabwe, both had a similar and relatively high gene frequency for this disorder. Both tribes were derived from common progenitors, but until 150 years ago had been geographically separated for 2 millenia. Subsequently, they have remained culturally and socially distinct. The implications are that the mutation for OI III in Africa occurred at least 2,000 years ago.

Lethal osteogenesis imperfecta with amniotic band lesions: collagen studies.

An infant was born with osteogenesis imperfecta (OI) and died after 7 days. In addition, there were amniotic constriction bands and amputations of several digits of the upper and lower limbs. The radiologic picture was suggestive of type III OI. Histomorphometric analysis of the bone showed a trabecular bone volume of 15.1% compared to 26.9% for age-matched controls. This was due to a decreased apposition of matrix by the osteoblasts. Because abnormal collagen synthesis has been suggested as the underlying defect in most forms of OI, collagen studies were undertaken using intact tissues. Bone …

[Genetic heterogeneity of osteogenesis imperfecta. Study of 6 cases].

Osteogenesis imperfecta one of the most common disorders of connective tissue, has been known for centuries. The most characteristic alterations which define it are: osteoporosis, osseous fragility with multiple fractures, blue sclerae, deafness and imperfect dentinogenesis. Important advances in the biochemical, anatomopathological, genetic, therapeutic and prophylactic fields have resulted in a great present-day interest in this disease. In this work we report six cases of osteogenesis imperfecta according to the current classification and we review the most outstanding aspects.

[Prenatal diagnosis of osteogenesis imperfecta, type II, following AID].

Osteogenesis imperfecta is a severe disorder of skeletal formation with nonuniform mode of inheritance. Insufficient bone formation in severe cases (Type II and III) leads to spontaneous intrauterine fractures with shortening of the extremities and thoracic deformities. Sonographical, radiological, pathological-anatomical and clinical data are presented and discussed in a case which was diagnosed at the 19. week of pregnancy.

[Ultrasonic early diagnosis of osteogenesis imperfecta: a case report].

The incorporation of ultrasonic examinations into the usual medical checkup in pregnancies, obliges the diagnostician to specifically look for malformation. The multistage-concept of Hansmann meets the requirements to achieve the highest possible diagnostic accuracy. Provided the multistage-concept functions as expected, it is possible to detect rare malformations at an early stage. The case of a woman, 19 weeks pregnant and ultrasonographically diagnosed as Osteogenesis imperfecta Typ II according to Silence, is presented.

Nonoperative treatment of osteogenesis imperfecta: orthotic and mobility management.

The problem of osteoporosis superimposed on the basic collagen defect of osteogenesis imperfecta has been approached by the use of plastic containment orthoses for the lower limbs, in addition to developmentally staged mobility devices that assist early standing and walking. The purpose of forcing early weight-bearing is to provide stress to the lower limb bones in order to minimize osteoporosis, prevent refracture and deformity, and curb subsequent immobilization osteoporosis, thus breaking a vicious cycle. Management goals are based upon adult needs for independence: efficiency in daily livi…

Spondylolisthesis resulting from osteogenesis imperfecta: report of a case.

Spondylolisthesis resulting from osteogenesis imperfecta has been very rarely documented in the literature. The possibility that osteofragility of the isthmus of the fifth lumbar vertabral can cause spondylolisthesis is noteworthy in the case of a 40-year-old man with trias fragilitas ossium hereditaria.

Altered relation of two collagen types in osteogenesis imperfecta.

To ascertain if the features of osteogenesis imperfecta could be due to an abnormality in two genetically distinct collagens, Type III and Type I, we measured, in pepsin digests of skin, the ratio of the alpha 1, (III) to alpha 1 (I) chains derived from the two types, using a method of interrupted polyacrylamide-gel electrophoresis. In 40 control subjects this ratio decreased from about 0.45 in early fetal life to a mean (+/-2 S.D.) of 0.14 +/- 0.06 in 14 adults. Seven of nine adult patients with mild osteogenesis imperfecta had ratios of more than 4 S.D., above this mean, as did two of five p…
