Clinical and Imaging Characteristics to Discriminate Between Complicated and Uncomplicated Acute Cholecystitis: A Regression Model and Decision Tree Analysis.

Background: Acute complicated cholecystitis (ACC) is associated with prolonged hospitalization, increased morbidity, and higher mortality. However, objective imaging-based criteria to guide early clinical decision-making remain limited. This study aimed to develop a predictive scoring system integrating clinical characteristics, laboratory biomarkers, and computed tomography (CT) findings to facilitate the early identification of ACC in the emergency department (ED). Methods: We conducted a retrospective study at an urban tertiary care center in Taiwan, screening 729 patients who presented to

A case of H-type double gallbladder anomaly: A case report.

RATIONALE: Biliary system anomalies, such as duplicated gallbladders, are rare congenital conditions that present significant diagnostic challenges. Dr. Boyden's classification system, especially the H-type anomaly, offers vital insight into these variations. Failure to detect these anomalies preoperatively can increase the risk of surgical complications, making early identification crucial for surgical planning. PATIENT CONCERN: A 42-year-old male, asymptomatic, was incidentally found to have a gallbladder mass during routine imaging. An upper abdominal magnetic resonance imaging showed gall

Primary immunodeficiencies caused by mutations in actin regulatory proteins.

The identification of patients with monogenic gene defects have illuminated the function of different proteins in the immune system, including proteins that regulate the actin cytoskeleton. Many of these actin regulatory proteins are exclusively expressed in leukocytes and regulate the formation and branching of actin filaments. Their absence or abnormal function leads to defects in immune cell shape, cellular projections, migration, and signaling. Through the study of patients' mutations and generation of mouse models that recapitulate the patients' phenotypes, our laboratory and others have

Lower Gastrointestinal Bleeding.

Lower gastrointestinal bleeding (LGIB) is the most common cause of hospitalization in the United States. Management of LGIB requires a multispecialty team approach and algorithm for identification of the source, as well as the cessation of bleeding. Use of an LGIB scoring system has been shown effective in appropriately classifying patients amenable to early discharge versus those requiring further inpatient management. If a patient experiences recurrent LGIB, treatment algorithms should be restarted. Segmental resection for LGIB should be reserved for localized bleeding. Surgery for unlocaliz

Macrotroponin as a cause of falsely elevated cardiac troponin in systemic sclerosis.

This case report discusses a patient in his late 30s with recent diagnosis of systemic sclerosis who presented with persistently elevated cardiac troponin (cTn) levels, raising concerns for cardiac involvement. Despite elevated troponin-I and troponin-T, clinical investigations, including echocardiogram and cardiac MRI, were unremarkable. The persistence of elevated troponin without corresponding clinical evidence prompted further investigation, leading to the identification of macrotroponin as the cause. This case emphasises the importance of recognising macrotroponin as an interferent in pat

DRESS syndrome: More than just a rash.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is rare but carries significant mortality and morbidity, making early identification and definitive management crucial. The diagnosis of DRESS is made clinically and involves consideration of a broad list of differential diagnoses. Given variable clinical presentations among patients with DRESS syndrome, clinicians should look for common findings and other hallmarks of the syndrome while monitoring for known complications. Additionally, clinicians should maintain a high index of suspicion to avoid missing more mild presenta

Treatment and management of scleral disorders.

Scleral inflammatory disorders, with their extensive differential diagnoses and sometimes as controversial treatment options, pose both diagnostic and clinical challenges for physicians. Targeted laboratory investigations derived from careful history taking and physical examination are crucial in the early identification of patients with associated systemic conditions. Prompt discrimination between episcleritis and scleritis is of therapeutic importance, as management, prognosis, and complications differ for these diseases. Although immunomodulatory and new biologic agents have improved the ma

Candida albicans infection of sternum and costal cartilages: combined operative treatment and drug therapy and 5-fluorocytosine.

Two patients with candidal sternal osteomyelitis have been successfully treated by operative debridement and adjuvant drug therapy with 5-fluorocytosine. One patient had developed postoperative candidal wound infection after sternotomy, and the other acquired candidal sternal osteomyelitis following Candida fungemia. The diagnosis, suggested by culture, was confirmed by identification of Candida pseudohyphae in debrided tissue. Histological confirmation of candidal sternal osteomyelitis indicates the need for operative debridement and specific systemic antifungal therapy. The drug 5-fluorocyto

Development and validation of an early prediction model for cardiac death risk in patients with light chain amyloidosis: a multicenter study.

BACKGROUND: Cardiac involvement is the primary driver of death in systemic light chain (AL) amyloidosis. However, the early prediction of cardiac death risk in AL amyloidosis remains insufficient. OBJECTIVES: We aimed to develop a novel prediction model and prognostic scoring system that enables early identification of these high-risk individuals. METHODS: This study enrolled 235 patients with confirmed AL cardiac amyloidosis from three hospitals. Patients from the first hospital were randomly assigned to the training and internal validation sets in an 8:2 ratio, while the external validatio

A novel multipurpose mini-endoscope for frontal sinus endoscopy "sinus view".

Endoscopic frontal sinus surgery has been proven to enable the treatment of most frontal sinus pathologies but may be challenging for the surgeon in regard to the variable frontal sinus anatomy. Frontal sinus drainage identification and frontal sinus visualization are an essential part of successful frontal sinus surgery. We demonstrate a novel modular mini-endoscopic system for frontal sinus surgery. Fifty-two patients (37 male, 15 female) with a chronic rhino-sinusitis were enrolled. In this study, all patients were subjected to standard endonasal endoscopic sinus surgery with use of the fib

Identification of supernumerary teeth in 2D and 3D: review of literature and a proposal.

Supernumerary teeth occur in both syndromic and nonsyndromic patients, and dental professionals are likely to encounter such teeth in their professional careers. There are three main numbering systems used to identify teeth today: the Universal/ National, the Palmer/Zsigmondy notation, and the Federation Dentaire Internationale (FDI) numbering systems. However, a review of the literature suggests that none of these three consistently addresses the identification of supernumerary teeth. Being able to communicate the location of supernumerary teeth is important for dental professionals, especial

Critical issues using brain-computer interfaces for augmentative and alternative communication.

Brain-computer interfaces (BCIs) may potentially be of significant practical value to patients in advanced stages of amyotrophic lateral sclerosis and locked-in syndrome for whom conventional augmentative and alternative communication (AAC) systems, which require some measure of consistent voluntary muscle control, are not satisfactory options. However, BCIs have primarily been used for communication in laboratory research settings. This article discusses 4 critical issues that should be addressed as BCIs are translated out of laboratory settings to become fully functional BCI/AAC systems that

Candida auris central line-associated blood stream infection in critically ill patients: the worst end of a bad scenario.

BACKGROUND: Candida auris (C. auris) is an emerging aggressive pathogen that causes severe infections in critically ill patients. Therefore, the assessment of this pathogen, characterized by inclination for biofilm formation, elevated colonization rate, and resistance to multiple drugs, holds a paramount importance. There is no data regarding the isolation of C. auris in our tertiary care hospitals' intensive care units (ICUs). The current case study was arranged to assess the incidence of C. auris central line-associated bloodstream infection (CLABSI) problem in our (ICUs). METHODS: Specimen

Acute Kidney Injury to Chronic Kidney Disease Transition.

BACKGROUND: Acute kidney injury (AKI), even if followed by renal recovery, is a risk factor for the future development of chronic kidney disease (CKD) and end-stage renal disease (ESRD). In the previous years, novel insights in the pathophysiology of CKD progression suggested a causal link between AKI and CKD due to a maladaptive repair after severe and repeated injury. SUMMARY: Several pathological mechanisms have been proposed to contribute to the progression of AKI and transition to CKD/ESRD including hypoxia and microvascular rarefaction, alterations of renal resident cell phenotypes and

In vivo neurochemical imaging of olfactory dysfunction in Parkinson's disease.

Olfactory dysfunction is common in Parkinson's disease (PD) and has been attributed to early deposition of α-synuclein pathology in olfactory areas. The pathophysiology of olfactory dysfunction in PD, however, remains poorly understood. Changes in odor identification suggest in part impairment in odor memory, possibly due to hippocampal dysfunction. Olfactory dysfunction occurs also in Alzheimer's disease (AD) and increases with severity of dementia. Cholinergic degeneration is not only a feature of AD but can also occur in PD, at least in a subset of patients with cognitive changes. We report

Evidence of impaired sense of smell in hereditary angioedema.

BACKGROUND: Hereditary angioedema (HAE) is an autosomal-dominant disorder resulting from C1-inhibitor (C1INH) deficiency. Smell impairments were found in patients affected with systemic lupus erythematosus, that, similarly to HAE, is characterized by the activation of the classical complement pathway with C4 consumption. In this study, we aimed at evaluating the sense of smell in patients with HAE. METHODS: Thirty patients with HAE and 30 healthy age- and sex-matched controls were evaluated for olfactory functions using the 3-stages Sniffin'-Sticks kit (threshold, discrimination, and identifi

[Clinical and morphological features of Merkel's carcinoma in elderly patients (literature review).].

Merkels cell carcinoma (MCC) is a rare and aggressive neuroendocrine skin tumor with a high rate of recurrence and local metastasis. The average age at which MCC is diagnosed is 75 years. The identification of the role of somatic mutations and DNA repair in aging mechanisms raises the question of their direct connection with the processes of differentiation, age-related characteristics of the immune system, programmed cell death and malignant growth. In this regard, there remains a need for research aimed at improving the early diagnosis and pathogenetic treatment of elderly patients.

Physiological and biological skin changes in pregnancy.

Pregnancy affects all organ systems in the body, including the skin. Some skin changes are physiological, whereas others such as the dermatoses of pregnancy are pathological. Dermatologists regularly encounter pregnant patients, and the identification of normal skin changes and their differentiation from disease states are essential to insure the well-being of both mother and fetus. This review discusses the physiological skin changes in pregnancy, including alterations in pigmentation, hair, nail, and connective tissue, as well as vascular and hematologic changes and modulation of glandular f

Temporomandibular joint pain as a manifestation of temporal arteritis.

We have described a patient whose temporal arteritis had several unusual manifestations. Except for a number of systemic complaints of longer duration, her predominant symptom was that of temporomandibular joint pain. Subsequent to the development of acute visual loss, her fundi remained normal, suggesting a posterior neuropathy. A high index of suspicion and meticulous history and physical examination should lead to early identification of temporal arteritis so that treatment can be instituted before permanent visual loss occurs.

Acute-on-chronic liver failure.

Acute-on-chronic liver failure combines an acute deterioration in liver function in an individual with pre-existing chronic liver disease and hepatic and extrahepatic organ failures, and is associated with substantial short-term mortality. Common precipitants include bacterial and viral infections, alcoholic hepatitis, and surgery, but in more than 40% of patients, no precipitating event is identified. Systemic inflammation and susceptibility to infection are characteristic pathophysiological features. A new diagnostic score, the Chronic Liver Failure Consortium (CLIF-C) organ failure score, h
