The effect of two new filaricidal compounds, CGP 6140 and CGP 20376, on the microfilariae of O. volvulus was tested in vitro. Culture medium consisted of a 1:1 mixture of IMDM/NCTC 135 supplemented with 20% heat-inactivated foetal calf serum and gentamycin. For pulse experiments the microfilariae were exposed to the drugs for one or three hours in medium without serum and then replaced to serum supplemented medium. A 63-year-old woman with treatment-resistant depression was started on trazodone and subsequently developed erythema multiforme (EM), with lesions predominant on the distal parts of the limbs and involvement of the hands and feet, as well as her palms and soles. The patient's treatment course and subsequent remission are described. It is suggested that trazodone therapy be discontinued immediately upon notice of EM. This patient was also taking lithium, which has not been implicated in erythema multiforme.

We report on a patient with systemic blastomycosis who presented with acute epididymitis that was followed by the appearance of typical skin and pulmonary lesions. An excellent clinical response was obtained with ketoconazole, a new oral antifungal agent. Only 2 cases of systemic blastomycosis that were treated with ketoconazole have been reported and neither patient had genitourinary involvement. The diagnosis and treatment of systemic blastomycosis with genitourinary involvement, and ketoconazole as the treatment of fungal infections are discussed.

Report of a case of histoplasmosis observed in Zaïre with lymphatic, pulmonary and intestinal localizations. Histoplasma duboisii was abundant in the stools, sputum and lymphatic tissue. With no amphotericine B available, treatment first started with high dosage of miconazole which had no effect. Ketoconazole have been, then, tried, giving a clinical and biological improvement, but patient still in a very bad condition, died. Ketoconazole seems to have a rapid but not lasting effect, even with increased dosages.

Eighty-two patients with pityriasis versicolor were treated orally with ketoconazole in tablet form (200 mg). The tablets were taken at least 90 min before a meal, and patients who received only one tablet daily were told to refrain from bathing except immediately before ingestion of the drug and only once a day. Dosage of ketoconazole varied from a single dose of 400 mg to 200--400 mg per day for four weeks. Seventy-seven patients reported no adverse effects. Ovarian carcinoma is the most lethal gynecologic malignancy. Resistance to platinum is considered the major problem affecting prognosis. Our recent study established that microRNA-506 (miR-506) expression was closely associated with progression-free survival and overall survival in two independent patient cohorts totaling 598 epithelial ovarian cancer cases.
