Placentation exhibits remarkable diversity among mammals, with reported correlations in cancer malignancy and invasiveness of placenta. Challenging the antagonistic pleiotropy hypothesis, which links evolution of invasive placentation to cancer malignancy, we had proposed the Evolved Levels of Invasability (ELI) framework for species with non-invasive epitheliochorial placentation. ELI posits that maternal stromal fibroblasts in epitheliochorial species evolved resistance to trophoblast invasion, secondarily limiting cancer dissemination in other tissues. Recent experiments with even-toed ungu…

Human childbirth is widely presumed to be uniquely difficult and dangerous compared to birth in other mammals. Tight fetopelvic proportions can result in obstructed labour and contribute to high rates of maternal and neonatal mortality. Ideas summarised under the 'obstetrical dilemma' have contributed to this assumption by explaining difficult childbirth as the outcome of a unique evolutionary history of bipedalism and encephalisation. However, a comprehensive review of the literature and an analysis of collated data demonstrate that human birth is not truly exceptional in its risk. Dystocia (

Preeclampsia (PE), a hypertensive disorder of pregnancy, is one of the leading causes of maternal and fetal mortality worldwide. Beyond the immediate pregnancy impact, it is also associated with increased long-term risks in both the mother and the offspring. Currently, no causal therapy exists. Consequently, the complex and heterogeneous nature of PE necessitates the exploration of novel therapeutic targets to improve maternal and fetal outcomes. This review summarizes models of pathogenesis in PE, particularly the two-stage model of the development of PE, where the placenta is suggested to be

INTRODUCTION: Placental histopathologic evaluation of pregnancies affected by congenital heart defects (CHDs) can provide insight into the mechanism connecting placental and fetal vascular systems. The objective of this study was to evaluate the prevalence of maternal vascular malperfusion (MVM) and fetal vascular malperfusion (FVM) lesions of the placenta in pregnancies with and without fetal CHD on blinded pathologic examination.
METHODS: This is a case-control study of pregnancies complicated by CHD at our institution from January 1, 2011 to June 25, 2021. Controls were matched to cases in

BACKGROUND: Ischemic placental disease, a syndrome that comprises preeclampsia, placental abruption, and fetal growth restriction, is characterized by ischemic, endothelial, and microvascular dysfunction. These processes, in turn, may serve as sentinel markers of maternal brain vulnerability, resulting in structural abnormalities, altered neuronal connectivity, and impaired neurogenesis.
OBJECTIVE: To estimate the burden of postpartum readmissions for neurological and psychiatric disorders (hereafter referred to as brain disorders) in women associated with ischemic placental disease in the sa

INTRODUCTION: Atypical placental site nodule (APSN) is a rare diagnosis, representing remnants of a previous pregnancy and extravillous trophoblast tissue. These lesions are potential precursor lesions to rare forms of Gestational trophoblastic neoplasia (GTN). Recent data suggest up to a 15% risk of malignancy occurring either concurrently or manifesting within a few months of diagnosis. These patients are often young with future fertility considerations. Prognosis, treatment, and clinical follow-up of APSN cases currently remain a matter of debate. This study aimed to address and explore som

BACKGROUND: Bacterial infection at the maternal-fetal interface contributes to adverse pregnancy outcomes, including preterm birth, chorioamnionitis, and sepsis. Despite evolving obstetric practices that may influence microbial patterns, recommended clinical algorithms for prophylaxis and treatment rely on outdated data. Furthermore, the pathogenic organisms associated with adverse outcomes remain incompletely characterized.
METHODS: We conducted a retrospective study of placental cultures from a high-risk obstetric cohort from a single hospital system between 2014 and 2019 (n = 714). Placent

The placenta plays a crucial role in mediating nutrient and gas exchange between the mother and fetus during pregnancy. Targeting therapeutic agents to the placenta presents significant opportunities for treating placental disorders and enhancing fetal outcomes. However, the unique structural complexity and selective permeability of the placenta pose substantial challenges for effective drug delivery. This review provides a comprehensive overview of current strategies for placental targeting, including lipid nanoparticle (LNP) delivery systems, targeted peptide modifications, specific antibody

Placental lesions rarely occur in isolation and placental lesion multiplicity is associated with poorer pregnancy outcome than that of isolated lesions. As little is known about mutual relations of various patterns of placental injury simultaneously occurring in the same placentas, particularly in relation to gestational age, this retrospective observational analysis was undertaken to study those in a population of 2486 cases of the second half high-risk pregnancy dominated by fetal congenital anomalies. To this end, 23 independent clinical and 48 placental phenotypes were statistically compar

It is known that mineral fibres can be found in placental tissues, but their effect is not known on these tissues. BeWo in vitro model of syncytiotrophoblast, the outer layer of maternal-foetal barrier, is necessary to understand if mineral fibres can alter placental cell turnover and consequently to influence the outcome of pregnancy. We performed in vitro experiments using chrysotile UICC (UICC), chrysotile Valmalenco (VM) and erionite (ERI) to investigate the potential cytotoxic effects of these mineral fibres on BeWo cells. We demonstrated that all fibres are toxic while only UICC fibres c

Human placental-derived allografts are biomaterials categorized as cellular, acellular, matrix-like products (CAMPs) that can serve as wound coverings due to placenta tissue's innate barrier function. The placental membrane consists of three layers, the amnion, the intermediate layer (IL), and the chorion, each contributing distinct functional and biological properties. This study investigates how variations in layer composition influence the Extracellular Matrix (ECM) and growth factor profiles of placental allografts. We compared Dual Layer (amnion-amnion), Full Thickness (amnion-intermediat

The intricate processes of feto-placental development are regulated by both genetics and the nutritional environment. The placenta mediates nutrient transfer to the fetus and waste transfer to the maternal circulation to facilitate proper fetal cell fate specification. Disruptions in these fate-regulatory programmes can lead to maternal and fetal complications. We explore how metabolic-epigenetic mechanisms programme feto-placental development, discussing the roles of metabolic sensing mechanisms. This Review covers the roles of nutrient metabolite-dependent protein modifications, lipid signal

Even in the highest inhabited regions of the world, well above 2500 m altitude, women become pregnant and give birth to healthy children. The underlying adaptation to hypobaric hypoxia provides interesting insights into the physio(patho)logy of the human placenta. Although increasing altitude is regularly associated with fetal growth restriction (FGR), oxygen deficiency does not appear to be a direct cause. Rather, placental oxygen consumption is reduced to maintain the oxygen supply to the fetus. This comes at the expense of placental synthesis and transport functions, resulting in inappropri

Leptin is a well-known metabolic hormone that plays diverse roles in various body functions, including growth, reproduction, and blood pressure regulation. In pregnancy, leptin produced from the placenta is crucial for ensuring proper fetal development and angiogenesis; however, pathological increases in leptin in maternal circulation are strongly associated with vascular endothelial dysfunction and preeclampsia. Leptin has a strong role in fertility and healthy pregnancy; however, numerous clinical reports over the last 2 decades show that leptin levels pathologically increase in patients wit

Preeclampsia (PreE) and placental malaria (PM) are leading causes of perinatal morbidity and mortality in Africa with overlapping pathophysiology. Utilizing data collected during a randomized, controlled clinical trial of intermittent preventive therapy during pregnancy in Malawi, we examined the association between PM and PreE, including severe manifestations, such as eclampsia. All singleton pregnancies with placental histopathology and polymerase chain reaction (PCR) were included. Among 751 pregnancies, 125 (16.7%) had evidence of PM by histopathology or PCR identifying PM parasites, inclu

The changes in serum concentrations of glycodelin A, placental α1-microglobulin, and adiponectin in pregnant women were studied and the possibility of using them for predicting placenta accreta spectrum (PAS) was evaluated. Significantly reduced serum levels of all studied proteins and higher number cases of cesarean sections were found in women with PAS. A formula for predicting PAS was developed. It was found that patient's score ⩾ 8 indicates the presence of PAS with high sensitivity and specificity.

Early placentation relies on temporal changes in intrauterine oxygen tension that regulate trophoblast differentiation events. Studies have highlighted the contribution of bioactive sphingolipids to the pathogenesis of placental disorders, characterized by hypoxia. However, it is unknown whether placental sphingolipid metabolism changes during the switch from a hypoxic to an oxygenated environment in the first trimester of gestation and if sustained hypoxia is causative of sphingolipid alterations seen in preeclampsia. Herein, we performed sphingolipid analysis of first-trimester human placent…
