Diagnostic significance of mesenteric lymph node involvement in proximal small bowel Crohn's disease.

BACKGROUND: The diagnosis of proximal small bowel involvement in Crohn's disease (CD) can be challenging at magnetic resonance enterography (MRE). The inflammatory process in CD can be associated with peri-intestinal inflammatory reactions, including the presence of inflamed mesenteric lymph nodes. OBJECTIVES: To evaluate the significance of inflamed mesenteric lymph nodes adjacent to the jejunum at MRE in CD and the association with proximal bowel disease as detected by video capsule endoscopy (VCE). DESIGN: This retrospective study was performed in two tertiary medical centres, and include

Pediatric Psychotropic Polypharmacy: An Evaluation of the Correlates and Prevalence Across Assessment Cycles in the National Health and Nutrition Examination Survey.

Background: Pediatric psychotropic polypharmacy (PPP) is the prescription of more than one medication targeting psychiatric disorders among people younger than 18 years. Recent data suggested that PPP rates may be plateauing. Few studies have evaluated this question in large, nationally recruited samples. Objective: The National Health and Nutrition Examination Survey was used to examine the correlates and prevalence of PPP across assessment cycles. Independent assessments were obtained biannually between 2013 and 2018. Methods: Eleven thousand four hundred thirty-nine participants (4-17 years

A retrospective real-world study of the current treatment pathways for myelofibrosis in the United Kingdom: the REALISM UK study.

BACKGROUND: Myelofibrosis (MF) is a blood cancer associated with splenomegaly, blood count abnormalities, reduced life expectancy and high prevalence of disease-associated symptoms. Current treatment options for MF are diverse, with limited data on management strategies in real-world practice in the United Kingdom. METHODS: The REALISM UK study was a multi-center, retrospective, non-interventional study, which documented the early management of patients with MF. The primary endpoint was the time from diagnosis to active treatment. DISCUSSION: Two hundred patients were included (63% [n = 126/

Idiopathic multicentric Castleman disease treated with siltuximab for 15 years: a case report.

UNLABELLED: Human herpes virus-8 (HHV8)-negative, idiopathic multicentric Castleman disease (iMCD) is a rare lymphoproliferative disorder sustained by pro-inflammatory cytokines, including interleukin-6 (IL-6). According to the international evidence-based criteria developed by the Castleman Disease Collaborative Network (CDCN), siltuximab, which works by inhibiting IL-6, is the recommended choice for iMCD treatment. We report a case of a 63-year-old white male with iMCD who has been on maintenance therapy with siltuximab for 15 years - representing one of the longest treatment periods of any

What are the benefits of the anti-FGF23 antibody burosumab on the manifestations of X-linked hypophosphatemia in adults in comparison with conventional therapy? A review.

UNLABELLED: X-linked hypophosphatemia (XLH) is a genetic disease mostly related to PHEX gene mutations which increases FGF23 serum levels, leading to hypophosphatemia and osteomalacia in adults, while affected children, in addition, develop rickets. Most of adults with XLH suffer from reduced quality of life and physical disability due to chronic bone and joint pain related to limb deformities, early osteoarthritis, delayed-healing of insufficiency fractures, and enthesopathies. Dental infections, muscle dysfunction, and deafness are also frequent. The current treatment consists of 2-5 times d

Characteristics of voluntary reporting of adverse drug events related to antipsychotics in Australia: 14-year analysis.

BACKGROUND: Retrospective analyses of large databases of treated patients can provide useful links to the presence of drug misuse or rare and infrequent adverse effects, such as agranulocytosis, diabetic ketoacidosis or neuroleptic malignant syndrome. The aim of this study is to describe the adverse effects to antipsychotics reported in the Australian Database of Adverse Event Notifications (DAEN). METHODS: Data were collected from the DAEN - a spontaneous reporting database. The database, which covered the period from January 2004 to December 2017, was obtained from the Therapeutic Goods Adm

Flipping death-to-survival event coding shifted pooled risk ratios and GRADE imprecision ratings in 2,071 Cochrane meta-analyses.

OBJECTIVE: When meta-analyzing a dichotomous outcome such as death or survival, reviewers must decide which side of the outcome to code as the event. In a single study, death and survival are two sides of the same dichotomous outcome, and the choice does not change the result. However, in a meta-analysis using the risk ratio, this choice affects the weight given to each study. Pooling deaths and pooling survivors need not give equivalent results. We evaluated how flipping the event coding of death or survival outcomes affects pooled risk ratios, precision, and GRADE imprecision judgments acros

Histologic evaluation of alveolar ridge preservation using aseptically processed large-particle demineralized freeze-dried bone allograft: A case series.

BACKGROUND: Demineralized freeze-dried bone allograft (DFDBA) is widely utilized in alveolar ridge preservation (ARP). However, limited human histologic evidence exists characterizing early healing associated with aseptically processed large-particle DFDBA. Both graft processing methodology and particle size may influence regenerative healing dynamics, graft integration, and tissue maturation. METHODS: Three systemically healthy patients underwent ARP following tooth extraction using aseptically processed large-particle DFDBA (850-2000 µm) and dense polytetrafluoroethylene (d-PTFE) membrane c

Real-world predictors of progression to type 2 diabetes among adults with prediabetes.

AIMS: Assess predictors of progression to type 2 diabetes (T2D) among patients with prediabetes. MATERIALS AND METHODS: This case-control study assessed adults with prediabetes using US claims and EMR-linked data from January 1, 2016, through January 31, 2024. T2D cases were patients with prediabetes who progressed to T2D (index: date of first T2D diagnosis); controls had prediabetes but did not progress to T2D. Control index dates were randomly assigned from cases. Demographic and clinical characteristics in the year before progression to T2D were assessed to identify progression predictors.

Social prescribing and supported self-management: A scoping review of the potential for implementation in non-surgical oncology services.

BACKGROUND: Social prescribing and supported self-management are two components of the National Health Service (NHS) personalised care model and have proven effectiveness in primary care settings. Social prescribing is a range of non-clinical services that address the psychological, psychosocial, and environmental issues affecting a person's well-being. Patients undertake supported self-management tasks to increase their knowledge, skills, and confidence in managing their health and medical care. However, these practices are not yet routinely integrated into radiation therapy. With 53.5 millio

Evaluating empirical calibration of P values under unmeasured confounding bias: a simulation study and real-world application.

OBJECTIVES: To evaluate whether empirical calibration of P values using negative controls can effectively control type I and type II errors under unmeasured confounding bias in both simulated and real-world observational settings. STUDY DESIGN AND SETTING: A simulation study was conducted under five settings reflecting different degrees of adherence to the U-comparability assumption-that is, the extent to which negative controls share the same unmeasured confounding structure as the exposure of interest. These included three primary scenarios (ideal, realistic, and violation of U-comparabilit

Evaluation of grading of recommendations assessment, development, and evaluation approach adherence in clinical practice guidelines: a systematic survey of criteria and their applications.

OBJECTIVES: Many clinical practice guidelines (CPGs) claim to follow the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach, yet their actual adherence remains uncertain. We aimed to identify and evaluate existing criteria for evaluating adherence to GRADE in CPGs and describe the extent of adherence revealed by their application. STUDY DESIGN AND SETTING: We systematically searched PubMed, Embase, Web of Science, and the Cochrane Library from January 2004 to November 2024, supplemented by manual reviews of the GRADE handbook and related websites.
