Cleft lip, alveolus and palate: Defect or dislocation malformation? Importance of adopting a physiological concept for surgical repair in achieving optimal outcomes in LMICs. Part 1: Physiological processes in facial development.

The conventional surgical procedures for the closure of cleft-lip and palate are based on proven plastic-surgical procedures from tumour surgery or traumatology. These flap surgeries take little account of the fact that the treatment of such malformations takes place during the time of a child's increased growth and is known to lead to pronounced scarring and skeletal growth disorders. It is therefore imperative to develop modified surgical procedures based on physiological growth processes. These physiological procedures must include the reconstruction of all facial structures such as orofaci

Robotic glove port technique for the endowristed rigid instruments in robotic single-site transabdominal and transvaginal surgery.

Although robotic single-site (RSS) surgery employing cross setup of semirigid instruments allows effective triangulation of instruments, it has some limitations in performing RSS transabdominal and transvaginal surgeries. We introduced the robotic glove port technique (RGPT) using parallel setup of endowristed rigid instruments in performing RSS transabdominal surgery and transvaginal surgery in July of 2017. Thirty-five patients underwent RSS surgery with RGPT. Twenty-one patients had RSS transabdominal reconstructive or fertility-preserving surgeries such as myomectomy (12 patients), adenomy

The microbial etiology and resistance patterns of cosmetic tourism-related infections in Ireland.

BACKGROUND: Cosmetic tourism is an increasingly common phenomenon, both in Ireland and internationally. Complications associated with procedures performed abroad may manifest after the patient has traveled back to their home country with patients often presenting to local health services. Such complications can be infectious in nature requiring either antimicrobial therapy and/or source control in the form of wound debridement or radiologically guided interventional procedures. METHODS: Patients who had presented to a regional plastic surgery unit between September 2021 and December 2022 with

Challenging frontiers in neuroplastic cranial reconstruction: addressing neurosurgical wound healing complications through interdisciplinary collaboration - an observational study.

BACKGROUND AND OBJECTIVES: Although rare, complications like skin dehiscence and necrosis after neurosurgery pose significant challenges by increasing the risk of infections spreading to the epidural, subdural, or intracerebral spaces. This retrospective, single-center study aims to assess the prior clinical courses, neuroplastic repair, and outcomes of  patients with skin defects following cranial neurosurgical procedures, and to outline our interdisciplinary reconstructive protocol. METHODS: A retrospective analysis was performed on cranial surgeries conducted at the Department of Neurosurg

Patient and professional use of the root coverage esthetic score (RES) and how it relates to patient satisfaction following periodontal plastic surgery.

BACKGROUND: Following periodontal plastic surgery in the treatment of recession defects, previous studies have reported that patients rate the esthetic outcomes more favorable than dental professionals. The root coverage esthetic score has been developed and suggested to serve as a comprehensive assessment instrument as it addresses several esthetic outcomes following root coverage procedures. However, no study has yet reported on patient use of this instrument. In the present study clinical, esthetic and patient-reported outcomes following periodontal plastic surgery were assessed. The primar

Minimally Invasive Approaches to Complex Abdominal Wall Reconstruction.

Robotic platforms have made it possible to replicate open retromuscular component separation principles through small incisions in a way that was not possible on the laparoscopic platform. However, it is crucial to keep in mind that the optimal repair (not the platform) is key to achieving the best patient outcomes. Robotic abdominal wall reconstruction offers short-term advantages over open repair, including shorter hospital stay and fewer wound complications, albeit with longer operative times, higher procedural costs, and a steep learning curve. Current evidence suggests comparable hernia r

Review of Neurovascular Reconstruction During Robotic-Assisted Laparoscopic Radical Prostatectomy.

PURPOSE OF REVIEW: This manuscript provides a narrative review of techniques of neurovascular reconstruction during robotic-assisted laparoscopic radical prostatectomy (RALP). RECENT FINDINGS: Historic techniques of neurovascular reconstruction, including autologous nerve grafting and neurorrhaphy, have been shown to be feasible in patients in early pilot studies. More recently, perinatal tissue allografts have been studied, with studies supporting earlier recovery of potency or continence. We review historic experimental techniques which are less frequently employed, including nerve grafting

Surgical repair techniques in hypospadias with unfavorable urethral plate: A systematic review and network meta-analysis.

INTRODUCTION: Hypospadias is one of the most common urogenital malformations in male newborns. Numerous techniques exist for repairing hypospadias, including urethral advancement, tubularized incised plate (TIP), flaps, and graft-tubularized incised plate (GTIP). However, it remains unclear which approach is optimal in cases with an unfavourable urethral plate. OBJECTIVE: This systematic review and network meta-analysis compares efficacy, complication rates, and patient outcomes of the various surgical procedures used to repair hypospadias with an unfavorable urethral plate. [Non-transecting anastomotic ureteroplasty].

Ureteral stricture is a significant issue which requires surgical reconstruction in most cases. The main determining factors for the reconstruction type are the length and location of the stricture. The individual steps for each procedure are significantly different, thus the scenario is predetermined regardless of intraoperative data. Preoperative assessment of the stricture length is based on X-ray data. However, it is not always correct and surgeon may encounter a situation where the planned reconstruction turns out to be suboptimal. This clinical case demonstrates intraoperative conversion

100 anastomoses: a two-year single-center experience with robotic-assisted micro- and supermicrosurgery for lymphatic reconstruction.

Robotic-assisted microsurgery has gained significant attention in recent years following the introduction of two dedicated microsurgical robotic systems specifically designed for this purpose. These feature higher degrees of movement and motion scaling which are useful tools, especially when performing surgery in areas of the body which are difficult to access. Robotic-assisted microsurgery has been implemented in lymphatic surgery as well as soft tissue reconstructive surgery at our institution over the past 2.5 years. Our study gives an insight into the details and outcomes of the first 100

Trifold technique for pupil reconstruction in large non-appositional iris defects.

Large iris defects are challenging to close due to shortage of tissue and opposite vector force. By using two pupilloplasty methods, we can close large iris defects with less tractional force or tethering. The trifold technique is a combination of trocar-assisted iris repair and single-pass four-throw (SFT) pupilloplasty for non-appositional large iris defects. The initial two trocar-assisted iris repair sutures oppose the unopposed iris ends and the subsequent SFT seals the defect. The combination procedure is less manipulative with no complications for the iris and pupil diaphragm.

[Urethral reconstruction].

A urethral stricture is an abnormal narrowing of the urethra due to spongiofibrosis of the urethral mucosa and the underlying corpus spongiosum. The diagnostics include uroflowmetry, sonography and radiology. For penile strictures the success rate of endoscopic treatment is low. Therefore, urethroplasty should always be performed, preferably using oral mucosa. Depending on the complexity, reconstruction must be carried out in one or multiple stages. For short bulbous strictures endoscopic treatment can primarily be carried out. In the case of recurrence urethroplasty should be carried out. The

Complex Forefoot Reconstruction with Percutaneous Techniques.

Complex forefoot deformities are challenging to treat and are labor intensive for the surgeon and the patient. New minimally invasive surgery (MIS) shows great potential and in some instances outperforms traditional open surgery. Another advantage MIS has is that it is technically easier to perform (once proficient) and produces less pain and recovery for the patient. This article takes the reader through MIS preoperative planning, soft tissue considerations, the different osteotomy configuration options, rationale, transverse plane correction, sagittal plane correction, revision MIS surgery o

The Lasso Suture: Ex Vivo Testing of an Alternative to the High-Tension Deep Dermal Stitch.

SUMMARY: Wound dehiscence, with an estimated occurrence rate greater than 4% in plastic surgery, is generally underreported, and can be an indicator of increased mortality and remission rates. The authors developed the lasso suture as a stronger alternative to the current standard patterns. The lasso suture takes less time to perform than the standard high-tension wound repair method. The authors dissected caprine skin specimens to create full-thickness wounds for suture repair using simple interrupted, vertical mattress, horizontal mattress, and deep dermal with running intradermal (DDR) sutu

Robotic Ureteral Reconstruction.

It is generally accepted that robotic ureteral reconstruction provides equivalent results to open and laparoscopic approaches while decreasing pain and length of stay. There is a rapid expansion of robotic ureteral reconstructive techniques, platforms, and adjunctive technologies, enabling more efficient, safer, and novel surgical approaches that could not be done in the past. For instance, indocyanine green use allows rapid, precise location of ureteral stenosis and determination of tissue perfusion. Multi-image display allows the surgeon to integrate the robotic field and ureteroscopic image

CONGENITAL SCAPHOID MEGALOURETHRA: REVISITING A RARE ANOMALY.

Megalourethra is an infrequent malformation of the anterior urethra that is caused by the lack of corpus sponigosum and in severe cases is accompanied by the lack of corpora cavernosa as well. We report a five-year-old boy presented to us with scaphoid variety of megalourethra having complaints of ballooning of urethra during voiding. He was investigated with urethrogram and cystoscopy and subsequently repaired with Nesbitt Longitudinal Reduction Urethroplasty. He had smooth recovery postoperatively with normal voiding stream on follow up.

A modified Bilhaut-Cloquet procedure for thumb duplication: Reconstruction of functionality and appearance.

BACKGROUND: The Bilhaut-Cloquet technique is the preferred surgical method for thumb polydactyly with symmetrical caliber and osseous components. However, the traditional Bilhaut-Cloquet technique has many esthetic and functional complications. In this study, we aimed to investigate an effective modified Bilhaut-Cloquet procedure for Wassel type IV thumb duplication. METHODS: The dorsal scar of the thumb was hidden on the ulnar side of the thumb through the flap design. The nail bed was sutured flat by combining the phalangeal bone and nail bed on different planes. The sensory function of the

Pinhole pupilloplasty after previous radial keratotomy.

The multiple radial stromal deep corneal incisions placed in radial keratotomy (RK) lead to higher-order aberrations and pose a surgical challenge to performing any further corrective procedure on the cornea because of fear of inducing an incisional dehiscence. A method to perform pinhole pupilloplasty (PPP) in the setting of previous RK is presented. Application of pinhole optics by performing PPP leads to significant improvement in image quality and helps to optimize visual potential in post-RK cases.
