Outbreaks of COVID-19 variants in US prisons: a mathematical modelling analysis of vaccination and reopening policies.

BACKGROUND: Residents of prisons have experienced disproportionate COVID-19-related health harms. To control outbreaks, many prisons in the USA restricted in-person activities, which are now resuming even as viral variants proliferate. This study aims to use mathematical modelling to assess the risks and harms of COVID-19 outbreaks in prisons under a range of policies, including resumption of activities. METHODS: We obtained daily resident-level data for all California state prisons from Jan 1, 2020, to May 15, 2021, describing prison layouts, housing status, sociodemographic and health chara…

Evaluating the Prevention Benefit of HCV Treatment: Modeling the SToP-C Treatment as Prevention Study in Prisons.

BACKGROUND AND AIMS: Between 2014 and 2019, the SToP-C trial observed a halving in HCV incidence in four Australian prisons following scale-up of direct-acting antiviral (DAA) therapy. However, the contribution of HCV treatment to this decline is unclear because the study did not have a control group. We used modeling to consider this question. APPROACH AND RESULTS: We parameterized and calibrated a dynamic model of HCV transmission in prisons to data from each SToP-C prison on incarceration dynamics, injecting drug use, HCV prevalence trends among prison entrants, baseline HCV incidence befo…

Does distance decrease healthcare options for pregnant, incarcerated people? Mapping the distance between abortion providers and prisons.

OBJECTIVES: Pregnant, incarcerated people retain the constitutional right to abortion, but evidence suggests that many cannot access abortion services. State and federal prisons are often located in remote areas and there is a known shortage of abortion providers across the U.S., particularly in remote areas. The goal of this study was to determine the proximity of state and federal prisons to the nearest abortion clinic. STUDY DESIGN: We used publicly available information to identify and geocode abortion clinics in the U.S., as well as state and federal prisons that house at least 10 female…

[Assessment and Management of the Risk of Exposure to COVID-19 of Healthcare professionals in Italian Prisons].

BACKGROUND: The COVID-19 pandemic had a relevant health impact in a large part of the planet and there are several studies aimed at understa … ey. RESULTS: 204 nurses participating in the survey (response rate 49.27%). Most nurses working in prisons (92.65%) were exposed to the risk of Covid-19 virus infection, since they came directly into contact with positive patients (90.69%) and in situations where they were exposed during care procedures in which aerosol was produced by the patient (56.21 …

ADVERSITY, MENTAL HEALTH, AND SUBSTANCE USE DISORDERS AS PREDICTORS AND MEDIATORS OF RULE VIOLATIONS IN U.S. PRISONS.

This study explores how typologies of adversity and mental health/substance use disorders impact rule violations during incarceration. Data come from the 2004 cross-sectional Survey of Inmates in State and Federal Correctional Facilities (SI-SFCF). Logistic regression and structural equation modeling were used for analysis. Results link history of adverse experiences to rule violations during incarceration and demonstrate how mental health and substance use disorders mediate this relationship. Incarcerated people with severe histories of adverse experiences had the highest odds of rule violati

How many drug rehabilitation places are needed in prisons to reduce the risk of bloodborne virus infection?

Transmission of HIV and hepatitis B virus infection has been recognised in prisons, and injecting drug use is a major route of infection. Combined results of two pilot health care surveys showed that 47% of prisoners with a history of injecting drug use wanted help to give up class A drugs but only 11% of non-injecting drug users expressed a similar wish. It would therefore seem appropriate for prisons to estimate the number of inmates with a history of injecting drug use and provide drug rehabilitation places for half that number (47% rounded up). Data from three prisons in England and Scotla…

Using nominal group technique to identify perceived barriers and facilitators to improving uptake of the Prison Needle Exchange Program in Canadian federal prisons by correctional officers and healthcare workers.

BACKGROUND: Elimination of bloodborne viruses including HIV and hepatitis C virus from prisons requires high coverage of evidence-based interventions that prevent bloodborne virus transmission, including needle and syringe programs. Canada launched a Prison Needle Exchange Program (PNEP) in nine federal prisons in 2018; however, uptake among people who inject drugs in prison remains low. We aimed to explore barriers and facilitators to improving PNEP uptake identified by correctional officers and healthcare workers. METHODS: Participants from nine federal prisons with PNEP completed focus gro…

Brief Report: HCV Universal Test-and-Treat With Direct Acting Antivirals for Prisoners With or Without HIV: A Prison Health Care Workers-Led Model for HCV Microelimination in Thailand.

BACKGROUND: This study investigated the sustained virologic responses (SVRs) among prisoners with hepatitis C virus (HCV) using universal test-and-treat approach by prison health care workers in a central male prison in Thailand. METHODS: A universal HCV screening was conducted in a maximum-security central prison (Klong Prem Central Prison) in Thailand. HCV RNA-confirmed prisoners were treated with generic sofosbuvir/velpatasvir by prison health care workers, regardless of their HCV genotypes and duration of prison sentences. We evaluated the SVR rates at 12 weeks after completing direct act

Methods for delivering the UK's multi-centre prison-based naloxone-on-release pilot randomised trial (N-ALIVE): Europe's largest prison-based randomised controlled trial.

INTRODUCTION AND AIMS: Naloxone is an opioid antagonist used for emergency resuscitation following opioid overdose. Prisoners with a history of heroin use by injection have a high risk of drug-related death in the first weeks after prison-release. The N-ALIVE trial was planned as a large prison-based randomised controlled trial (RCT) to test the effectiveness of naloxone-on-release in the prevention of fatal opiate overdoses soon after release. The N-ALIVE pilot trial was conducted to test the main trial's assumptions on recruitment of prisons and prisoners, and the logistics for ensuring that…

Prevalence and Factors Associated With Hepatitis C in Danish Prisons, 2022-2024: A Multicentre Cross-Sectional Study.

Prisons offer a critical opportunity for hepatitis C virus (HCV) elimination, yet current data from Danish correctional facilities are sparse. We conducted a cross-sectional study in 16 prisons across Eastern Denmark between October 2022 and August 2024, enrolling 651 incarcerated individuals. All participants underwent HCV antibody and RNA testing using dried blood spots and completed a bio-behavioural risk survey. The prevalence of HCV antibody and HCV RNA was 4.2% (n = 26) and 2.0% (n = 13) respectively. HCV exposure was most prevalent among individuals with a history of injecting drug use …

"Everyone Hears Everyone's Business": Health Care Provider Perspectives on Privacy and Confidentiality in U.S. Prisons and Jails.

Incarcerated people suffer stigmatized conditions at higher rates than nonincarcerated people, making upholding privacy and confidentiality ethically essential in prisons and jails. Yet these health professional values are widely acknowledged to be challenging to maintain in carceral settings. Robust qualitative data on how physicians and nurses navigate and perceive limitations to privacy and confidentiality in U.S. prisons and jails is lacking. This study reports on 55 semistructured interviews with physicians and nurses who have worked in federal or state prisons and/or jails across the Uni…

Transgender identity and psychiatric care in Italian prisons: a thematic analysis of systemic gaps and institutional challenges.

BACKGROUND: Transgender and gender-diverse (TGD) individuals face high levels of stigma, discrimination, and psychological distress, which are amplified in carceral settings. Italian prisons have introduced specialized sections for TGD inmates; however, limited empirical evidence exists on their function and the psychiatric, social, and institutional challenges emerging within them. METHODS: A case-informed qualitative design was applied to four TGD inmates housed in a specialized prison section in Northern Italy. Data were derived from routine psychiatric and endocrinological assessments, ad…

Screening for tuberculosis in prisons: efficacy of a breath-based diagnostic approach in a high prevalence setting.

OBJECTIVES: Tuberculosis (TB) remains a major health threat in prisons, where overcrowding and limited access to care facilitate transmission. We evaluated the diagnostic accuracy and feasibility of the AeoNose for TB screening in Paraguayan prisons. METHODS: In this cross-sectional diagnostic study, people deprived of liberty and prison staff underwent symptom screening, chest X-ray with CAD4TB (v6), and AeoNose breath sampling. Individuals with presumptive TB underwent GeneXpert and mycobacterial culture. Breath signals were analyzed using a convolutional neural network. Two subsets were ev…

Are Supervision Violations Filling Prisons? The Role of Probation, Parole, and New Offenses in Driving Mass Incarceration.

Advocates for reform have highlighted violations of probation and parole conditions as a key driver of mass incarceration. As a 2019 Council of State Governments report declared, supervision violations are "filling prisons and burdening budgets." Yet few scholarly accounts estimate the precise role of technical violations in fueling prison populations during the prison boom. Using national surveys of state prison populations from 1979 to 2016, the authors document that most incarcerated persons are behind bars for new sentences. On average, just one in eight people in state prisons on any give…

Risk Reduction Policies to Reduce HIV in Prisons: Ethical and Legal Considerations and Needs for Integrated Approaches.

The United States has the fastest growing prison population in the world, and elevated incarceration rates, substance use, and human immunodeficiency virus (HIV) prevalence are fueling each other. Yet without a national guideline mandated for HIV care within the prison system, standards for state and federal prisons vary greatly.

Incidence and molecular epidemiology of hepatitis C virus reinfection in prisons in Catalonia, Spain (Re-HCV study).

Hepatitis C virus (HCV) reinfection may hamper HCV elimination in prisons. We aimed to (i) determine the reinfection rate in people treated for HCV in Catalan prisons, (ii) measure reinfection in people entering prisons, and (iii) characterize the molecular epidemiology of HCV in prisons and people who inject drugs (PWID) in the community. Re-HCV was a prospective study in eight prisons (2019-2020) including two groups: (1) people cured with treatment in prison and followed-up every 6 months, and (2) people testing HCV-RNA positive at incarceration. Bio-behavioral data were collected. HCV isol…

[Independent monitoring of prisons: The work of the European Committee for the Prevention of Torture in Switzerland].

Prisons concentrate people with multiple vulnerabilities who are exposed to violations of human rights, without any exception, including Switzerland. Independent monitoring bodies play an important role in preventing ill-treatment in detention. The most effective monitoring body worldwide is the European Committee for the Prevention of Torture (CPT), which bases its work on a European Convention that guarantees unrestricted access to detention facilities and all documents, including medical documents, in the member states of the Council of Europe. This article summarizes the main recommendatio…
