Radiation-induced cranial neuropathy in patients with nasopharyngeal carcinoma. A follow-up study.

GOAL: The purpose of the current study was to investigate the long-term characteristics of radiation-induced cranial nerve injury in nasopharyngeal carcinoma (NPC) patients. PATIENTS AND METHODS: We studied cranial nerve palsy (CNP) in 328 NPC patients who received radiotherapy between 1994 and 2006. Follow-up was 93.6% complete as of December 2009. A total of 72 patients with CNP were recruited for analysis (56 men and 16 women). Patients with evidence of residual or recurrent tumor accompanied by CNP were excluded. The characteristics of CNP and the relationship with the radiation fields as

Animal Model Considerations for Medical Countermeasure Development for Radiation and Sulfur Mustard Exposures: Animal models for radiation and HD exposures.

Development of medical countermeasures (MCM) to mitigate and/ or treat the pulmonary complications associated with exposure to chemical, radiological, and/ or nuclear weapons is a national, public health preparedness posture priority in the United States (US). Pulmonary exposure to either sulfur mustard vapor or radiation causes oxidative damage, vascular injury, hyperinflammation, and pro-fibrotic signaling cascades that lead to life-threatening and potentially debilitating lung disease. There is no MCM currently approved by the US Food and Drug Administration (FDA) to mitigate and/ or treat

Is spinal computed tomography necessary in pediatric trauma patients?

BACKGROUND: To investigate the prevalence of findings of spinal injury on computed tomography (CT) images of pediatric trauma patients and to define indicators for the possible presence of spinal injuries. METHODS: Spinal CT for pediatric trauma patients (age ≤ 12 years) over a 2-year period was retrospectively evaluated for the presence of findings suggestive of spinal injury. RESULTS: Of the 773 patients reviewed, 19 (2.4%) showed traumatic spinal lesions on their spinal CT images. These patients were significantly older than those without spinal lesions (mean age 7.9 ± 3.3 years vs 6.1 ±

Diagnostic Accuracy of Ultrasound for Adolescent Pelvic Avulsion Fractures: A Blinded Multirater Study.

OBJECTIVE: Pelvic avulsion fractures are common injuries among adolescent athletes and are often evaluated with imaging modalities involving radiation to the pelvis. Ultrasound is a portable, accessible, inexpensive, radiation-free alternative, but its accuracy for diagnosing pelvic avulsions has not been well established. The purpose of this study is to evaluate the diagnostic accuracy of ultrasound detecting pelvic avulsion fractures in adolescents, including among clinicians without prior ultrasound training and radiologists. METHODS: Four clinicians with limited or no ultrasound experienc

[Hazard assessment and occupational safety measures in surgery : Relevant knowledge on occupational medicine].

Surgeons routinely work in an environment with occupational risks and hazards about which they are often uninformed. Based on the currently available scientific literature this review article describes the various hazards in the operating theater and their effects on personnel, particularly from the surgical perspective. A further aim of this article is to describe the occupational safety measures to reduce the burdens and to maintain the long-term health of personnel. Ultimately, surgeons should be equipped with the necessary knowledge for implementing hazard assessments according to the Germ

[Stem cell derived therapy for cutaneous radiation exposure].

Radiation injury to skin results in a variety of deterministic effects including inflammatory reactions and cell depletion leading to distinct clinical symptoms following a defined time pattern. Therapeutic approaches are still limited, a complete restitution of affected areas is so far impossible. In the last few years increasing experimental knowledge about acquisition and administration of autologous stem cells also in the field of radiation injuries has been obtained. Evidence reviewed in this article shows that the beneficial effects of stem cell transplantation are not necessarily due to…

Parks' coloanal pull-through anastomosis for severe, complicated radiation proctitis.

A variety of operations have been proposed for the treatment of radiation injuries of the rectum. In this study, the procedure advocated by the late Sir Alan Parks--resection of the diseased segment down to its points of fixation to the vagina, bladder or prostate; with perianal mucosal stripping of the anorectal segment and primary coloanal (pull-through) anastomosis--was performed in 11 patients. In two others, an attempt at colorectal anastomosis was abandoned because of extreme scarring in the pelvis. The procedures were done following definitive treatment of carcinoma of the rectum (seven…

Unilateral myokymia of the tongue after radiation therapy for cervical nodal melanoma.

Hypoglossal neuropathy with associated myokymia as a delayed effect of radiation is a rare occurrence, presumably due to the relative resistance of cranial nerves to injury from irradiation. The authors describe the first case of myokymia of the unilateral tongue with myokymic discharges on needle electromyography after hypofractionated radiation therapy for an extracranial melanoma of the neck. The earlier onset of myokymia than previous cases may represent more direct radiation exposure due to radiation site or the higher radiation dosage administered for treatment of melanomas.

Ionizing radiation in endovascular interventions.

Ionizing radiation exposure is an inherent component of endovascular surgery. As procedures become more complex, greater radiation exposure is incurred by both staff and patient. Radiation doses during endovascular aneurysm repair are similar to those recorded during percutaneous coronary interventions. Ionizing radiation may cause skin injury, as well as increase the long-term risk of malignancy. Endovascular surgeons need to be aware of radiation hazards and take all reasonable steps to minimize the risks to both patients and staff.

Bilateral phrenic nerve dysfunction: a late complication of mantle radiation.

Neurologic complications from radiotherapy can be immediate or can occur many years after treatment. A known complication of radiotherapy to the supraclavicular and axillary lymph nodes is brachial plexus neuropathy. Although not a common injury, phrenic nerve dysfunction has been reported in association with radiation-induced brachial neuropathy. We describe a patient who developed asymmetric diaphragmatic weakness secondary to phrenic nerve paralysis 37 years after receiving mantle radiation for Hodgkin lymphoma. The patient did not have an associated brachial plexus neuropathy or a secondar

Activation of Neural Modeling-Related Genes in the Heart of Mice after Gamma Irradiation.

Источник пометил публикацию как отозванную

OBJECTIVE: Radiation-induced heart disease (RIHD) is a common sequela of thoracic irradiation. At the same time, nerve remodeling is involved in the progression of heart disease. However, the activation of the nerve remodeling related genes in radiation-induced heart disease is still lacking. METHODS: In this study, C57BL/J mice was anesthetized by intraperitoneal injection with pentobarbital sodium (2%, 40 mg/kg), and radiation was delivered using a cobalt-60 (60Co) teletherapy unit (Cirus). When the mice were anesthetized, none of them showed the signs of peritonitis, pain, or discomfort. T

Alterations in the structure and function of the mammalian cell membrane after exposure to ionizing radiation.

Although the nucleus appears to represent the primary target for the killing of cells by ionizing radiation, the cell membrane may be another important site for radiation injury and modification of the radiation response. In this paper, the effects of radiation on various membrane-associated parameters which are expected to affect cellular viability or function are reviewed. These include the effect of radiation on membrane transport, lectin or hormone receptor sites, cell surface electrokinetics, cell membrane fluidity, cell surface topography, etc. Alternative biochemical mechanisms that may

Ionizing Radiation Induces Extracellular Trap Release from Macrophages.

Macrophages are key innate immune cells in the host defense against pathogens. Ionizing radiation can impair macrophage functions such as phagocytosis and activate them, potentially exacerbating tissue injury. Macrophage extracellular traps (METs) are formed upon stimulation of macrophages with PAMPs or DAMPs. We hypothesized that macrophages exposed to ionizing radiation can release extracellular traps. Peritoneal macrophages were collected from C57BL/6 mice and subjected to 5 Gy radiation. We performed assays to detect METs, including the immunofluorescence of citrullination of histone H3 an

Protective effects of autologous bone marrow-derived mesenchymal stem cell transplantation on acute radioactive enteritis in Beagle dogs.

BACKGROUND: Radiation enteritis is a common complication of radiation therapy in which the surrounding normal intestinal tissue is damaged by ionising radiation, and there is no standard pharmacological prophylaxis or treatment regimen available. Mesenchymal stem cell transplantation can be used for radiation protection and the treatment of acute radiation injury, but its therapeutic mechanism of action remains unclear. AIM: To investigate the protective effects of autologous bone marrow-derived mesenchymal stem cell (ABMSC) transplantation on radiation-induced intestinal injury. METHODS: A

Polypharmacy to Mitigate Acute and Delayed Radiation Syndromes.

There is a need for countermeasures to mitigate lethal acute radiation syndrome (ARS) and delayed effects of acute radiation exposure (DEARE). In WAG/RijCmcr rats, ARS occurs by 30-days following total body irradiation (TBI), and manifests as potentially lethal gastrointestinal (GI) and hematopoietic (H-ARS) toxicities after >12.5 and >7 Gy, respectively. DEARE, which includes potentially lethal lung and kidney injuries, is observed after partial body irradiation >12.5 Gy, with one hind limb shielded (leg-out PBI). The goal of this study is to enhance survival from ARS and DEARE by polypharmac

A further comparison of pathologies after thoracic irradiation among different mouse strains: finding the best preclinical model for evaluating therapies directed against radiation-induced lung damage.

The human lung is among the most sensitive and critical tissues of concern in localized and systemic radiation exposures, and it is a subject of active preclinical research for evaluating mitigating therapies within the radiation countermeasures program. Our previous study comparing C57BL/6, CBA and C57L mice after whole-thorax irradiation pointed to the problems of late pleural effusions that prevented the full development of lung injury in C57BL/6 mice and suggested that the CBA and C57L strains are more favorable for modeling lung injury in humans (Jackson et al., Radiat. Non-targeted effects of ionizing radiation: implications for risk assessment and the radiation dose response profile.

Radiation risks at low doses remain a hotly debated topic. Recent experimental advances in our understanding of effects occurring in the progeny of irradiated cells, and/or the non-irradiated neighbors of irradiated cells (i.e., non-targeted effects associated with exposure to ionizing radiation), have influenced this debate. The goal of this document is to summarize the current status of this debate and speculate on the potential impact of non-targeted effects on radiation risk assessment and the radiation dose response profile.

Occupational radiation exposure: population studies.

Occupational exposure to ionizing radiation in the medical setting differs from the acute exposure received by survivors of atomic bomb blasts. Yet, atomic bomb survivors' disease and mortality outcomes have been the standard data source on the effects of ionizing radiation on humans. Therefore, the prevailing estimated risks of ionizing radiation may not apply to radiologic technologists and other medical radiation workers. Carefully designed epidemiological trials provide evidence that helps determine the strength of association between exposure and onset of disease in selected populations.
